Utah has authorized Nolla Health to use an AI system to issue initial prescriptions and renewals for certain topical acne treatments to eligible adults in the state. It is a limited, time-bound pilot—not broad approval of AI doctors—and physicians review every prescription before it reaches a pharmacy in the initial stage.
What Utah authorized
The Utah Office of Artificial Intelligence Policy and Division of Professional Licensing signed a regulatory mitigation agreement with Magic Health, Inc. and Magic Health Services, LLC, which do business as Nolla Health. The agreement covers the company’s Nolla Skin App and allows it to select and issue specified first-time topical acne prescriptions and renewals, then transmit them to Utah-licensed pharmacists.

The state listing gives the pilot an active term of October 5, 2026, through October 5, 2027. The agreement says the term begins when the technology is made available to customers and the company gives written notice to the state. The authorization applies only to the service, participants and requirements described in the agreement. It does not constitute state endorsement or approval of Nolla’s use of AI, and it is not a nationwide authorization.
Who can use it, and what can it prescribe?
The pathway is for Utah residents aged 18 or older with mild-to-moderate acne. The system assesses structured intake information and standardized facial images, then selects from a defined list of topical options. The agreement’s formulary includes tretinoin, adapalene, clindamycin with benzoyl peroxide, benzoyl peroxide and azelaic acid. Some options are available as retail prescription or over-the-counter equivalents; some compounded combinations are available only through compounding. Where offered, patients can choose a local-pharmacy option or compounded delivery.

The AI cannot prescribe outside that formulary. Severe or nodulocystic acne, or symptoms that suggest a need for oral, systemic or isotretinoin treatment, are outside its prescribing scope. Pregnancy, trying to become pregnant, breastfeeding, certain medical conditions, prior adverse reactions to formulary drugs and delivery outside Utah are among the listed exclusions or hard stops. Depending on the reason, the system must route a patient to a physician or direct them to seek in-person care.

- Initial stage: Two Utah-licensed physicians review each prescription before it is sent to a pharmacy.
- Later stage: Prescriptions may be submitted directly, with a physician retrospectively reviewing all cases at least weekly.
- Further stage: Physicians may review a monthly sample of at least 10% of prescriptions, while reviewing every escalation and adverse-event case.
The agreement allows the state to pause or reverse progression. The existence of these later stages does not mean Nolla has already reached them: the state must approve each transition.
What safeguards and disclosures apply?
The agreement requires Nolla to tell users they are interacting with generative AI and to tell pharmacies that prescriptions were generated by AI. It also requires adverse-event reporting within 24 hours, monthly reporting that includes clinician agreement rates, professional liability insurance, and restrictions on selling or transferring patient data. These are contractual requirements; they do not by themselves establish that the treatment is effective or safe.

Nolla describes the app as using a medical intake, a five-angle face scan and constrained treatment protocols, with licensed physician messaging available. Those are company-described features, not independent validation of the system.
What is known about performance?
Nolla reported roughly 96% clinician agreement in prior app cases in its 2026 materials, describing the remaining differences as minor strength adjustments. That is a company-reported figure for prior app cases, not an independently verified result from Utah pilot patients.

The Utah agreement sets at least 95% clinician agreement, zero missed hard stops after quality-assurance review and zero serious adverse events as targets for progression between stages. Those are thresholds, not reported outcomes of the Utah pilot. The sources available for this pilot do not provide independently validated patient outcome statistics.

Nolla advertised a $4.99-per-month pilot subscription in its 2026 launch announcement. Medication may cost extra, and the amount varies by fulfillment route; the subscription figure should not be read as the total cost of treatment.

Why “AI doctor” needs a qualifier
The pilot is notable because it covers initial topical acne prescriptions as well as renewals, but the phrase “AI doctor” can overstate what Utah authorized. The system operates within a narrow formulary and eligibility rules, with physician review required for every prescription at launch. Nolla’s claim that it is the first service of its kind is the company’s characterization; independent coverage has distinguished its initial-prescription authority from pilots limited to AI-assisted refills.
