Ask the surgeon who performed your operation when healing is sufficient for intercourse and attempts to conceive. The answer depends on the procedure, what was found or treated, whether one or both ovaries were involved, any complications, and your recovery.
After clearance, make a time-limited fertility plan with your clinician. Age, pain, previous operations, ovarian reserve, other infertility factors, partner testing, tubal considerations, preferences, and the Endometriosis Fertility Index (EFI) all matter. Surgery can relieve disease but may also reduce ovarian reserve, so delaying a plan indefinitely is not automatically the safest approach.

What the guidance actually says about timing
Neither the 2022 European Society of Human Reproduction and Embryology (ESHRE) endometriosis guideline nor current NICE endometriosis recommendations sets one rule such as “wait six weeks,” “wait three cycles,” or “wait until the first period.” Those milestones may be appropriate for an individual, but they are not universal medical instructions.

Do not assume that the date of your first period is a safety clearance. Conversely, do not continue avoiding conception solely because a generic online rule says you must wait a certain number of cycles. The relevant date is the one your surgeon gives you after reviewing your operation and recovery.
Why operations lead to different advice
An operation may involve draining an endometrioma, removing its cyst wall, treating endometriosis elsewhere, separating adhesions, or procedures on one or both ovaries. The operative report may also reveal extensive disease or findings that change the fertility plan. A person with an uncomplicated procedure on one ovary may receive different advice from someone who had bilateral disease, repeat ovarian surgery, extensive adhesiolysis, or a postoperative complication.
Contact the surgical team rather than waiting for a routine appointment if you have fever, worsening or severe pain, heavy bleeding, foul-smelling discharge, persistent vomiting, wound redness or drainage, fainting, shortness of breath, or a rapidly enlarging abdomen. These symptoms need medical assessment and are not questions to solve by changing your conception schedule.
What to cover at the postoperative visit
Bring your questions and request a plain-language explanation of the operative findings. The following checklist keeps the appointment focused on both healing and fertility:
- What exactly was removed, drained, or treated?
- Were one or both ovaries involved, and was ovarian tissue unintentionally affected?
- Were there adhesions, tubal concerns, deep endometriosis, or other findings that matter for conception?
- Is healing adequate for intercourse and attempts at pregnancy now?
- Are there activity, lifting, intercourse, or follow-up restrictions?
- Does my prior surgery or this operation make ovarian-reserve assessment useful?
- Given my age, previous attempts, symptoms, partner factors, and findings, how long should we try before reassessing?
- Should we calculate an EFI and combine it with other fertility-test results when deciding about assisted reproductive technology (ART)?
These are discussion prompts, not a mandatory test panel or a fixed follow-up timetable. ESHRE recommends counseling about the chance of pregnancy after surgery and using EFI together with other fertility investigations when identifying people who may benefit from ART.

Building a fertility plan after clearance
Once the surgeon confirms that trying is medically permissible, decide what you will do and when you will review the plan. “Try naturally” is not the same as “wait without a plan.” A defined review point protects time, particularly when age, reduced reserve, previous infertility, or repeat ovarian surgery could make delay consequential.
Expectant management and natural attempts
Natural attempts may be reasonable when the tubes and sperm factors are acceptable, symptoms are manageable, ovarian reserve and age are reassuring in context, and there has not been a long period of infertility. The duration should be agreed with a clinician rather than copied from a single rule. Track the start date of attempts and any symptoms so the review is based on an accurate timeline.
Surgery does not guarantee pregnancy. It may remove a cyst or treat pain while other factors—ovulation, tubal function, sperm quality, age-related egg quality, adhesions, or disease elsewhere—still influence the outcome.
Fertility evaluation and ART
ESHRE could not formulate a single recommendation identifying exactly who must have ART after endometrioma surgery. Instead, it recommends individualized counseling about pregnancy chances and use of EFI alongside other results. A fertility evaluation commonly considers ovulation and menstrual history, the uterus and tubes where relevant, partner semen analysis, prior pregnancies and treatments, endometriosis extent, and ovarian-reserve information.
ART may deserve earlier discussion when there is older reproductive age, a prolonged period of trying, bilateral or repeat ovarian surgery, significant reserve concern, tubal or male-factor infertility, severe disease, or a personal preference to prioritize time. “Discuss ART” does not mean “start IVF immediately”; it means obtaining a specialist assessment before an avoidable delay.

Do not schedule repeat surgery simply to improve IVF success
For someone already pursuing ART, ESHRE advises against routine endometrioma surgery for the purpose of improving live-birth rates. Current evidence has not demonstrated that benefit, and ovarian surgery can negatively affect reserve. Surgery may still be considered for substantial pain, suspicious features requiring evaluation, or when the cyst prevents access to follicles. The reason for another operation should therefore be explicit rather than an assumption that removal automatically improves fertility treatment.

How ovarian reserve fits into the decision
Endometriomas and their treatment can affect ovarian reserve. ESHRE highlights particular concern with severe endometriosis and bilateral endometriomas, and notes that surgery can further reduce reserve. A previous operation on the same ovary may also be relevant.
What AMH can and cannot tell you
Anti-Müllerian hormone (AMH), antral follicle count, and related tests can help a fertility clinician estimate expected response to ovarian stimulation and plan treatment. They do not provide a yes-or-no answer about whether you can become pregnant naturally. The American Society for Reproductive Medicine (ASRM) describes ovarian-reserve tests as poor predictors of fertility, while still recognizing their value for guiding fertility treatment.
A low or declining result deserves context, not a prediction that pregnancy is impossible. A result that appears reassuring does not erase age, tubal, sperm, ovulatory, or endometriosis-related factors. ESHRE’s fertility-preservation guidance also notes that the usefulness of pretreatment AMH for predicting later pregnancy or the need for fertility preservation remains unclear.
Ask whether testing would change your decision. Repeating a test without a clinical question can create anxiety without improving the plan; interpreting it with your history and other findings is more useful.

Hormonal treatment has a different purpose
Postoperative hormone treatment is often discussed as though it were a fertility treatment. The guidelines distinguish its purpose.
When pregnancy is not being pursued immediately
ESHRE recommends offering long-term postoperative hormonal treatment after ovarian endometrioma surgery when a person is not seeking pregnancy right away. The goal is to reduce endometrioma and symptom recurrence. The choice should account for contraindications, side effects, preferences, and the intended duration before trying to conceive.
When you are actively trying
NICE advises against offering hormonal treatment alone or alongside surgery to people who are trying to conceive, because suppression does not improve spontaneous pregnancy rates. Hormonal suppression can prevent ovulation or conception while it is being used, so it should not be presented as a way to boost natural pregnancy chances during active attempts.
Tell your clinician which outcome you want now: pregnancy, pain and recurrence control while postponing pregnancy, or evaluation of another problem. Medication decisions make sense only after that goal is clear.
Do these 3 things before closing this tab:
1Repair Windows errors before they cause bigger problems2Scan for outdated or missing drivers - takes under a minute3Clear out junk files and repair common Windows errorsWhen to seek fertility help
Contact the operating team whenever you are unsure whether recovery permits sex or attempts, or when postoperative symptoms are concerning. Seek fertility advice on an individualized schedule if pregnancy has not occurred within the timeframe you and your clinician agreed on. Do not wait for a generic interval when delay could matter.
Best Value
Arrange earlier specialist input when any of these apply:
- older reproductive age or concern about age-related time sensitivity;
- previous infertility or a long period of trying before surgery;
- bilateral endometriomas, repeat ovarian operations, or a reserve concern;
- known tubal, ovulatory, uterine, or sperm-factor issues;
- severe or extensive endometriosis, persistent pain, or symptoms suggesting recurrence;
- a strong preference to understand ART options before attempting naturally.
A reproductive endocrinologist or fertility specialist can combine your operative report, symptoms, age, ovarian-reserve information, EFI, and partner testing into a time-based plan. Referral is a care pathway, not a commitment to IVF.
Comparing the main paths
| Path | When it may fit | What to weigh |
|---|---|---|
| Natural attempts with a review date | Recovery is complete, no urgent infertility factor is known, and the clinician considers delay reasonable. | Age, duration of infertility, symptoms, tubal and sperm factors, reserve, and the agreed reassessment point. |
| Early fertility evaluation or ART discussion | Time sensitivity, prior infertility, bilateral or repeat surgery, reserve concern, or other known factors make waiting costly. | EFI plus the rest of the evaluation, treatment burden, preferences, and the possibility that ART may be useful without further surgery. |
| Additional endometrioma surgery | Pain, suspicious findings, complications, or a cyst obstructing follicle access creates a separate medical reason. | Potential symptom benefit versus further ovarian-reserve loss; routine surgery solely to improve live birth before ART is not recommended by ESHRE. |
| Postoperative hormonal suppression | Pregnancy is being deferred and recurrence or symptom reduction is the priority. | Medication effects, contraindications, preferences, and the planned date for stopping suppression before conception attempts. |
The intended outcome must stay explicit. A strategy that is sensible for recurrence prevention while postponing pregnancy is not the same as a strategy intended to achieve pregnancy now.
A practical appointment-to-conception sequence
- Confirm recovery. Ask the operating surgeon whether intercourse and pregnancy attempts are permitted and whether restrictions remain.
- Obtain the operative facts. Record which ovary or ovaries were treated, the procedure performed, disease extent, complications, and any tubal or adhesion findings.
- Identify time-sensitive factors. Review age, prior attempts, previous surgery, pain, cycle history, partner semen factors, tubal considerations, and any reserve results.
- Choose a route. Agree whether to attempt naturally, obtain an immediate fertility evaluation, or discuss ART; do not assume another operation is required.
- Set a reassessment date. Write down when you will contact the clinician if pregnancy has not occurred or symptoms return.
- Revisit the plan if circumstances change. New pain, a suspected recurrence, a reserve result, or a change in family-building preferences can justify earlier review.
Questions that should not be answered by a universal rule
“Do I have to wait for my first period?”
Not as a general requirement established by the reviewed guidelines. Ask your surgeon when healing permits attempts; the first period is not a universal clearance test.
“Does surgery mean I should go straight to IVF?”
No. ESHRE recommends individualized counseling, using EFI and other fertility findings to identify who may benefit from ART. Some people may reasonably try naturally; others benefit from early specialist planning.
“Does AMH tell me whether I can get pregnant?”
No. AMH and other reserve tests can guide treatment, but ASRM considers ovarian reserve a poor stand-alone predictor of fertility.
“Should I take hormones while trying?”
Hormonal suppression is used for recurrence and symptom control when pregnancy is being deferred. NICE advises against using it to improve spontaneous pregnancy rates while actively trying.

A safe plan starts with surgical clearance, not an internet countdown. It then links your recovery, operative findings, age, ovarian reserve, EFI, other fertility factors, and preferences to a review date. The absence of a universal waiting interval does not mean there is no guidance; it means the clinically important information is individual rather than calendar-based. Discuss each decision with the surgeon or fertility specialist who knows your medical history.
