The White House announced a one-time $90 payment for more than 20 million Medicare Part B enrollees, intended to help with their premiums. It is not a check for all Americans: the administration says people whose premiums are paid by Medicaid and people who pay an income-related monthly adjustment amount (IRMAA) are excluded. Trump called the funding source a “slush fund,” but its formal name is the Medicare Improvement Fund.
What the $90 payment is—and who may get it
The White House says most people enrolled in Medicare Part B qualify for a one-time $90 payment. The stated aim is to help pay Part B premiums. Its fact sheet names two exclusions: people whose premiums are paid by Medicaid and people who pay IRMAA, an income-based adjustment to their Medicare premiums. Because the administration says “most” enrollees qualify, enrollment in Part B alone does not establish that a particular person is eligible. The White House fact sheet gives the announcement’s eligibility details.
The announced reach is more than 20 million enrollees, not 20 million Americans chosen without regard to Medicare coverage. The payment is also distinct from Trump’s proposed $5,000 “Trump Dividend” for every adult. The Associated Press reported that House Speaker Mike Johnson said congressional approval would be needed for that separate proposal. AP’s report addresses the dividend proposal, not the Medicare payment.
When and how the payment is supposed to arrive
The White House said eligible recipients should receive the payment in early October 2026. It described two delivery routes: direct deposit for people set up to receive one, or a paper check sent to the mailing address registered with Medicare for people without direct deposit. This is the announced schedule, not confirmation that every payment has arrived or that distribution is complete. The fact sheet lists 1-800-MEDICARE (1-800-633-4227) for eligibility questions and the Social Security Administration at 1-800-772-1213 for payment-status questions.

What “slush fund” means in this announcement
“Slush Fund” is Trump’s political label, not the formal name of a new benefit program. The White House identifies the source as the Medicare Improvement Fund. Its fact sheet says Congress provided $2 billion for improvements to the Medicare fee-for-service program and says the fund had not previously been used. Those statements about the fund’s history and purpose are the administration’s account. The available reporting does not provide an independent legal analysis establishing whether this specific payment is authorized under the fund’s governing rules.
Trump’s social post described the checks as “nearly $100”; the White House specifies $90. Axios’s same-day report notes the difference and reproduces the post. The official fact sheet is the clearest source for the announced payment amount.

How much $90 covers
Axios cited a standard monthly Part B premium of $202.90 and reported that $90 offsets less than half of that amount for one month. The payment is a one-time contribution, not a recurring monthly benefit and not enough to cover a full standard monthly premium. Individual premium costs can differ; the comparison here is specifically with the standard premium cited in that report. Axios’s coverage provides the premium figure.
