TMS vs. tDCS: How the Brain Stimulation Treatments Compare

TMS and tDCS are different treatments, not interchangeable names for the same therapy. Transcranial magnetic stimulation (usually delivered repetitively as rTMS) uses rapidly changing magnetic fields from a coil near the head. Transcranial direct current stimulation (tDCS) sends low-intensity direct electrical current between electrodes on the scalp. Which is more appropriate depends on the diagnosis, device, protocol, age, prior treatment, safety screening and country-specific authorization—not on the modality’s name alone.

In the United States, rTMS has the longer-established clinical and regulatory role for several defined indications. The regulatory picture for electrical stimulation also changed recently: the FDA approved the Flow FL-100 cranial electrotherapy stimulator on December 8, 2025, but only for a specific adult major depressive disorder (MDD) population. That decision does not approve every tDCS product, protocol or patient use.

What is the difference between TMS and tDCS?

Both treatments attempt to influence activity in brain regions without surgery, but they deliver different forms of energy and use different treatment parameters.

How TMS works

A TMS system positions an electromagnetic coil against or just above the scalp. The coil produces rapidly pulsed magnetic fields that induce electrical currents in targeted areas of the cerebral cortex. In clinical care, sessions commonly use repetitive TMS (rTMS), meaning a programmed series of pulses delivered at a selected frequency and intensity. The coil position, pulse pattern, intensity, number of trains and treatment schedule are part of the protocol; changing them can change the treatment and the evidence supporting it.

TMS is administered by trained staff using a device-specific targeting and safety procedure. A treatment described simply as “TMS” therefore does not identify one universal dose or one guaranteed mechanism. Current evidence does not establish that TMS “resets” the brain or allow a clinician to predict an individual response from the modality alone.

How tDCS works

tDCS applies a low-intensity direct electrical current through electrodes placed on the scalp. The direction and location of the current are intended to modulate activity in underlying cortical networks rather than deliver magnetic pulses. Protocols differ in electrode placement, current strength, session duration, number of sessions, spacing between sessions and whether stimulation is combined with medication, psychotherapy or another rehabilitation program.

Those details are clinically important. Two studies both labeled “tDCS” may use different montages, currents and schedules, so their results cannot automatically be combined. A consumer product, research headset and clinician-supervised medical device may also have different labeling, controls and evidence. Do not treat an unsupervised gadget as equivalent to a prescribed, monitored protocol.

Side-by-side comparison

Feature TMS/rTMS tDCS
Energy delivered Rapidly changing magnetic fields from a coil induce currents in targeted cortex. Low-intensity direct electrical current flows between scalp electrodes.
What varies by protocol Target location, coil type, pulse frequency and pattern, intensity, train length and session schedule. Electrode placement and size, current, session length, number and spacing of sessions, and combination with other treatment.
Typical administration Usually delivered in a clinic by trained personnel with device-specific targeting and monitoring. May be delivered in research or clinical settings; the appropriate supervision depends on the device label and protocol.
Regulatory position in the United States Multiple device- and indication-specific FDA clearances, including depression and severe OCD; an adjunctive adolescent depression clearance was reported in March 2024. FDA approval is not universal. Flow FL-100 received a December 8, 2025 PMA approval for a narrowly defined adult MDD indication; that decision does not cover every tDCS device or protocol.
Evidence interpretation Results depend on indication, device, target and pulse protocol. Results depend particularly on montage, current, schedule, population and whether treatment is combined with another intervention.
Commonly discussed tolerability issues Headache and scalp or application-site pain; hearing protection and screening are considered because of the device’s sound and rare serious events such as seizure. Reviews report mostly mild to moderate adverse events, including events occurring more often in active-stimulation groups in some trials; device-specific labeling determines the relevant warnings.

Does one work better for depression?

There is no scientifically sound universal winner for every person with depression. A treatment’s likely benefit depends on the exact diagnosis, severity, age, previous treatment, protocol, comparison condition and outcome being measured.

A 2024 systematic review and dose-response meta-analysis examined 110 studies involving 4,820 participants across mental disorders. It found associations between outcomes and treatment parameters for both TMS and tDCS, but the authors called for prospective randomized, sham-controlled studies to validate those patterns and test additional parameters. The result supports a practical conclusion: the label “TMS” or “tDCS” by itself is not enough to predict a response.

A separate 2025 systematic review of transcranial electrical stimulation included 88 randomized trials and 5,522 participants. In its tDCS analysis, improvement was associated with treatment in some groups of people with depression and comorbid conditions and in some analyses where medication was combined with stimulation. The review did not find the same conclusion in its MDD-only subgroup, and several estimates were rated low or very low quality. Mild to moderate adverse events were reported more often in active-stimulation groups. These findings do not justify either “tDCS works for depression” or “tDCS does not work”; they describe specific populations and uncertain estimates.

For rTMS, U.S. clearances and a larger clinical infrastructure mean that clinicians have more established pathways for certain depressive disorders. That regulatory history is not proof that every rTMS protocol is superior to every tDCS protocol. It means the device, indication and protocol have to be checked against the applicable authorization and evidence.

What conditions are authorized or studied?

rTMS in the United States

The National Institute of Mental Health reports that rTMS devices have been cleared for several forms of depression since 2008 and for severe obsessive-compulsive disorder (OCD) in 2018. It also reports an FDA clearance in March 2024 for adjunctive treatment of depression in adolescents age 15 and older. “Cleared for depression” is not a blanket authorization for every device, age, severity level or treatment history. A clinic must use a device and protocol whose labeling matches the patient’s indication.

tDCS-related authorization

The FDA PMA record for Flow FL-100 identifies it as a cranial electrotherapy stimulator for adults 18 and older with moderate-to-severe MDD in the current episode who are not considered treatment refractory to medication. The record permits use as monotherapy or as an adjunct to another treatment. Its decision date is December 8, 2025.

That is a narrow device-specific indication. Cranial electrotherapy stimulation is a regulatory category and should not be treated as proof that every tDCS headset, electrode arrangement or home protocol has the same authorization. A July 14, 2026 supplement record lists design, component and documentation changes for the device. Because supplements and labeling can alter the description or indication, prescribers and patients should check the current FDA device record and labeling before relying on eligibility details.

Outside the United States, terms such as approved, authorized, registered or available can have different meanings. The country, edition of the device and current label all matter.

Safety: how the risks differ

rTMS safety considerations

Headache and pain or discomfort at the application site are among the commonly assessed outcomes. The equipment can also produce loud clicking, so hearing protection and targeted hearing assessment may be part of a clinical protocol.

FDA safety guidance for rTMS studies calls for monitoring serious events such as worsening depression, suicidal ideation or behavior, mania, seizure, death and device malfunction. This list is a safety-monitoring framework, not a statement that each event is common. Seizure is a rare but serious potential risk that requires qualified screening and a protocol-appropriate treatment plan.

tDCS safety considerations

The 2025 electrical-stimulation review described mostly mild to moderate adverse events and found them more often in active-treatment groups in the analyzed trials. The type and frequency of symptoms can vary with current, electrodes, skin contact, session length and the underlying condition. A device’s own warnings and instructions take priority over a generic description of tDCS.

Information to discuss before either treatment

Bring a complete medical and treatment history to the prescribing clinician. Topics may include:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Seizures or factors that could alter seizure risk.
  • Implanted electronic devices, metal in or near the head, or other hardware relevant to the device label.
  • Current and recent medicines, including changes that could affect mood, sleep or seizure threshold.
  • The confirmed diagnosis, symptom severity, past treatments and any history of mania, psychosis or suicidal thinking.
  • Pregnancy, breastfeeding or plans for pregnancy when the specific device labeling asks about them.
  • How symptoms and adverse effects will be monitored and when treatment will be paused or reassessed.

Do not select intensity, electrode placement, pulse settings or session frequency from an internet guide. Those are treatment parameters, not general self-care instructions.

What a treatment course involves

Clinic workflow for rTMS

A clinic generally confirms the indication, reviews medical and medication history, identifies the treatment target, determines device-specific settings and explains expected sensations and sound. Each visit includes positioning the coil and delivering the programmed trains while staff monitor comfort and symptoms. The number of visits, session length and schedule vary by protocol and indication; the label and treatment plan—not a generic daily count—determine what is appropriate.

Workflow for tDCS

The clinician or trained operator selects the electrode montage, current and session schedule based on the device and intended use. Skin preparation, electrode contact and symptom checks can affect both tolerability and the validity of the protocol. If stimulation is paired with medication, therapy or rehabilitation, the combined plan should be documented rather than assuming that either component produced the outcome alone.

Access and burden

Ask how many appointments are expected, where treatment is administered, who is responsible for targeting and monitoring, and what happens if a session is missed. Availability is highly local. A treatment that is authorized in principle may not be offered by a nearby clinic, and a research protocol may not be available outside a study.

A decision framework for patients and clinicians

Use these questions to compare two proposed treatment plans without assuming that the modality decides the answer:

  1. Indication and eligibility: Is this exact device authorized or approved for my diagnosis, age, current episode and treatment history?
  2. Protocol: Which brain target, pulse or electrode parameters, session duration and total schedule will be used?
  3. Evidence: What studies used this protocol in people most like me, and how certain are the findings?
  4. Safety: What screening is required, which symptoms should be reported immediately, and who will monitor me?
  5. Goals: Which outcome will be measured—symptom score, response, remission, functioning or quality of life—and when will it be reviewed?
  6. Alternatives: What medication, psychotherapy, other stimulation treatment or watchful-monitoring options remain?
  7. Practical burden: How many visits are required, how long is each visit, and what travel or work disruption should I plan for?
  8. Coverage: What will the provider and insurer confirm in writing for this device and indication?

FDA guidance for rTMS clinical studies recommends validated depression outcomes, response and remission measures, patient-reported quality of life and adverse-event tracking. Asking a clinic how it measures these outcomes is more useful than relying on claims such as “rebalances” or “reprograms” the brain.

Costs, insurance and availability

Prices, insurance coverage and appointment availability vary by country, device, indication, clinic and payer. The evidence available here does not establish a universal price or coverage rule. Before starting, request a written estimate that separates evaluation, treatment sessions, follow-up visits and any facility fees. Ask whether prior authorization is required and what happens if the insurer denies a particular device or indication.

Do not infer coverage for tDCS from coverage for rTMS, or coverage for one rTMS indication from another. A device may be authorized while a specific use remains outside a payer’s policy.

Common misunderstandings

“TMS and tDCS are the same treatment.”

No. One uses magnetic pulses from a coil; the other uses direct electrical current through electrodes. Their parameters, evidence and regulatory labels differ.

“FDA approval of one tDCS-related device approves all tDCS.”

No. The December 2025 Flow FL-100 decision applies to that device and a defined adult MDD indication. It does not authorize every product, protocol or patient population.

“A clearance guarantees success.”

No. Clearance or approval establishes that a particular device may be used for a defined indication under specified conditions. It does not predict an individual’s response or establish superiority over every alternative.

“More sessions or a stronger setting must be better.”

Not necessarily. Dose-response relationships are protocol-specific, and the 2024 meta-analysis specifically called for better prospective validation. Changing settings without clinical supervision can alter both benefit and risk.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

“Home stimulation is automatically equivalent to clinic treatment.”

No. A home-use device, if authorized for a particular purpose, still has a specific label, training requirements and monitoring plan. An unregulated or off-label product should not be substituted for clinician-directed care.

How to prepare for an appointment

  • Write down your diagnosis, current symptoms, previous treatments and what helped or caused problems.
  • Bring a complete medication and supplement list, including recent dose changes.
  • Ask for the exact device name, indication, patient-age range and current labeling.
  • Request the planned target, session schedule, monitoring process and criteria for changing or stopping treatment.
  • Discuss seizure history, implants or metal, hearing concerns, pregnancy-related questions and any history of mania or suicidal thinking.
  • Agree on a measurable outcome and a date for reviewing progress.

The most defensible comparison is therefore individualized: match the device and protocol to the indication, confirm the evidence for the relevant patient group, weigh safety and practical burden, and review alternatives with a qualified clinician.
