TMS Side Effects: What’s Common and When to Contact Your Clinician

Transcranial magnetic stimulation (TMS), including repetitive TMS (rTMS), most often causes short-lived scalp discomfort or headache. Facial tingling or twitching and lightheadedness can also occur. A seizure is uncommon but serious, and new suicidal thoughts, unusually elevated or irritable mood, major behavior changes, hearing changes, fainting, or symptoms that are severe or worsening require prompt contact with the clinician supervising treatment. Call emergency services or your local crisis service if someone may act on suicidal thoughts or is in immediate danger.

What TMS is and why side effects vary

TMS is a clinician-administered treatment in which a coil is positioned against the scalp to deliver magnetic pulses to a targeted area of the brain. Treatment schedules, pulse patterns, coil designs, stimulation intensity, and the condition being treated differ between protocols. Your medical history, medications, sleep, and sensitivity to headache or sound also influence what you experience. Therefore, a side-effect rate from one device or protocol should not be treated as a prediction for every patient.

Common TMS side effects

Most expected reactions are mild to moderate and improve shortly after a session. Some people notice that they become less troublesome as treatment continues. Tell the technician or prescribing clinician during the appointment rather than waiting until the next visit; stimulation settings or positioning may sometimes be adjusted.

Scalp discomfort or pain

The tapping sensation of the coil and repeated muscle contractions can make the scalp tender or painful at the treatment site. Pennine Care NHS Foundation Trust describes this as the most common side effect and says it generally becomes less noticeable over the course of treatment. The 2024 consensus update from the National Network of Depression Centers, Clinical TMS Society, and International Federation of Clinical Neurophysiology reports significant scalp or stimulation-site pain or discomfort in 39% of summarized studies. That figure combines study results and does not indicate your personal probability.

Headache

A tension-type headache during or after treatment is frequently reported. The same 2024 consensus update gives a range of 28% to 65% for tension-type headache across summarized studies. Mayo Clinic notes that headaches usually improve soon after a session and may decrease with additional sessions, but frequent headache or migraine can be triggered in some people and may require a treatment change or pause.

Facial tingling, twitching, or spasms

Magnetic pulses can stimulate nearby facial muscles or nerves, causing tingling or brief twitching around the eye, cheek, or jaw. Tell the operator if the sensation is painful, persistent, or interfering with the session so the coil position or intensity can be reviewed.

Lightheadedness or dizziness

Some patients feel lightheaded during or shortly after treatment. Pennine Care also describes rare faintness or dizziness. Sit or remain supported until the sensation passes, and report it before leaving the clinic. The 2024 consensus update reports dizziness or lightheadedness in 4% to 9% of summarized studies.

Nausea, fatigue, and sleep changes

The 2024 consensus update lists nausea in 7% to 11% of summarized studies, fatigue in 7% to 8%, and insomnia in 5% to 7%. These figures are ranges from pooled professional evidence, not guarantees for an individual. Record when the symptom starts, how long it lasts, and whether it affects eating, driving, work, or sleep.

Effect What patients may notice Reported frequency in the 2024 consensus update
Scalp or stimulation-site pain/discomfort Tenderness or pain where the coil rests 39%
Tension-type headache Pressure or aching during or after a session 28–65%
Nausea Queasiness during or after treatment 7–11%
Dizziness or lightheadedness Feeling faint or unsteady 4–9%
Fatigue Unusual tiredness after treatment 7–8%
Insomnia Difficulty falling or staying asleep 5–7%

The percentages summarize studies cited by the 2024 professional consensus update. The indexed summary was available, but the full text was not accessible for verification here, so the figures should be read as approximate evidence ranges rather than a personal risk calculator.

When an expected symptom deserves a call

Contact the prescribing or supervising clinician promptly when pain, headache, dizziness, nausea, fatigue, or another reaction is significant, lasts longer than your team expects, worsens from session to session, or does not settle after treatment. Also call about any new symptom that concerns you, even if it is not on the clinic’s handout. Mayo Clinic says treatment may need to stop when frequent headaches or migraine are triggered.

Do not change the schedule, skip prescribed medicines, take an over-the-counter pain reliever, or alter another treatment without asking the clinician. Mayo Clinic notes that a provider may adjust stimulation or suggest an over-the-counter pain medicine; the safe choice and dose depend on your health, other medicines, allergies, and pregnancy status.

Symptoms that need urgent or emergency attention

Seizure or seizure-like activity

A seizure during TMS is rare but serious and requires immediate clinical response. Possible signs include loss of awareness, uncontrolled jerking, stiffening, collapse, or confusion afterward. Alert clinic staff immediately if an event occurs during treatment. After leaving the clinic, seek urgent medical assessment for a first seizure, a prolonged event, repeated events, injury, breathing problems, or failure to return to normal awareness. Follow the emergency instructions supplied by your treatment center.

Worsening depression or suicidal thoughts

Tell the treatment team promptly about new or worsening suicidal thoughts, a plan, increased hopelessness, or a sudden deterioration in depression. U.S. Food and Drug Administration (FDA) labeling guidance calls for warnings about worsening depression, suicidality, and unusual behavior changes, with close observation. If the person may act on suicidal thoughts, cannot stay safe, or is in immediate danger, call emergency services or use the local crisis service now; do not leave the person alone while waiting for help.

Mania, hypomania, or marked behavior change

Report unusually elevated or irritable mood, greatly reduced need for sleep, racing thoughts, unusually rapid speech, impulsive or risky behavior, severe agitation, or behavior that is out of character. These changes can emerge during treatment, particularly in people with bipolar-spectrum illness, and need clinical review rather than self-monitoring alone.

Hearing symptoms, fainting, or serious injury

TMS produces loud clicking. A sudden hearing change, ringing that does not settle, ear pain, or concern that hearing protection failed should be reported immediately to the treatment team and assessed urgently when severe. Fainting, a fall, chest pain, breathing difficulty, or a significant injury also warrants urgent assessment appropriate to the severity of the event.

Rare risks and required hearing protection

Seizure, mania or hypomania, and hearing injury are the principal serious concerns highlighted in the cited guidance. Their likelihood depends on the device, protocol, patient factors, and screening process; no single rate applies to every form of TMS. The FDA’s Class II Special Controls Guidance for Industry and FDA Staff states: “Patients and those operating the device should always wear earplugs or similar hearing protection devices with a rating of 30dB of noise reduction during rTMS treatment.” Mayo Clinic also describes patients wearing earplugs during treatment.

Use the hearing protection supplied or approved by your clinic and follow its device-specific instructions. Do not substitute consumer earplugs on your own if the clinic specifies another product, fit, or rating. The need is modest and practical: hearing protection is part of the treatment procedure, not an optional accessory to buy instead of following clinic guidance.

What to tell the clinic before your first session

Accurate screening helps the team decide whether TMS is appropriate, what precautions are needed, and how to set the device. Tell the clinician about:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Any implanted or metallic device, especially an implanted stimulator in or near the head, a cochlear implant, aneurysm clip, or other device that could be affected by magnetic fields. Compatibility must be checked for the exact device and TMS system.
  • Personal or family history of seizures, fainting, or unexplained episodes.
  • Bipolar disorder, psychosis, substance-related concerns, or previous manic or hypomanic symptoms.
  • Brain tumor, stroke, traumatic brain injury, or other neurological condition.
  • Frequent or severe headaches, migraine, or a history of unusual sensitivity to sound.
  • Every prescription medicine, nonprescription medicine, vitamin, and supplement, including doses and recent changes.
  • Pregnancy, plans to become pregnant, or any other medical condition that could affect treatment.

These items are not an automatic contraindication for every person or device. The treating clinician must assess the exact implant, medication, diagnosis, and protocol. Never conceal a device or medication because you think it is unrelated.

How to prepare for and monitor sessions

  1. Confirm the protocol. Ask which device and coil are being used, the target area, the planned number and frequency of sessions, and which symptoms should be reported during the appointment.
  2. Arrive with an up-to-date medication list. Include over-the-counter products and supplements. Tell the clinic about dose changes before treatment begins.
  3. Use the clinic’s hearing protection. Put it in before pulses start and tell the operator if it is loose, uncomfortable, or displaced.
  4. Report discomfort in real time. Describe the location and severity of scalp pain, headache, facial twitching, nausea, or dizziness. The operator may be able to pause, reposition the coil, or discuss an intensity adjustment.
  5. Remain seated until you feel steady. If you are lightheaded after treatment, ask staff for help before standing or driving.
  6. Keep a simple symptom record. Note session date, onset, duration, severity, sleep, headache or migraine symptoms, mood changes, and any medicine taken after discussing it with your clinician.
  7. Use the agreed contact route. Keep the clinic’s daytime and after-hours numbers, emergency instructions, and crisis resources accessible at home.

Questions that clarify your personal risk

  • Which side effects are expected with this specific coil and pulse protocol?
  • What severity or duration of headache should trigger a same-day call?
  • Who should I contact after hours, and which symptoms require emergency services?
  • What hearing protection does this device require, and will the clinic provide it?
  • Could my current medicines, supplements, seizure history, migraine, pregnancy, or implanted device change the plan?
  • How will you monitor mood, suicidal thinking, mania, sleep, and other changes during the course?

Why protocol-specific advice matters

TMS is not one identical procedure. Device and coil design, stimulation intensity, pulse frequency, session length, treatment target, and the patient’s health history all affect tolerability and risk. A symptom that is mild and brief for one protocol may need review in another context. Use general information to recognize patterns, but use your own clinic’s written instructions for scheduling, medicines, hearing protection, driving, and emergency escalation.

Bottom line for patients

Scalp discomfort and headache are the effects most often reported with TMS, and facial tingling or twitching, lightheadedness, nausea, fatigue, and insomnia can also occur. They commonly ease after a session or over the treatment course, but persistent, worsening, or significant symptoms should be reported. Seizure, hearing injury, severe fainting, worsening suicidality, and manic or markedly unusual behavior require urgent clinical attention. Full disclosure of medical history, medicines, seizure risk, mood history, and implants allows the supervising team to make the treatment as safe as possible.
