There’s Another Cat Parasite That Can Infect Humans. A Huge Study Shows How Common It Is

The parasite is Toxocara cati, the feline roundworm—not Toxoplasma gondii. Cats can contaminate soil and other environments with eggs that eventually infect people, but owning or touching a cat does not automatically cause infection, and most human infections cause no symptoms.

What the large cat study found

A 2024 systematic review and meta-analysis combined 289 studies published from 1973 through 2023, covering more than 150,000 cats and other animals. Using coproparasitological tests—methods that detect eggs or worms in feces—it estimated a pooled T. cati prevalence of 17.0% (95% confidence interval, 16.2–17.8%).

Only four studies used PCR, which detects parasite DNA. Their pooled estimate was lower, at 4.9% (95% confidence interval, 1.9–7.9%). These figures are not interchangeable: they come from different tests, populations and study designs.

Group or method Estimated prevalence Qualification
All cats and populations, coproparasitological methods 17.0% Pooled estimate from the 2024 review
PCR-based studies 4.9% Pooled estimate from four studies
Feral cats 42.6% Pooled estimate by cat type
Stray cats 29.9% Pooled estimate by cat type
Shelter cats 20.1% Pooled estimate by cat type
Nepal 94.4% Highest country-level pooled estimate in the review

The 17.0% figure is therefore not the infection rate for every household cat. Geography, access to veterinary care, whether cats are feral or owned, and the diagnostic method can change the result substantially.

How Toxocara cati reaches people

Cats shed unembryonated eggs in feces. In the environment, those eggs generally need one to four weeks to embryonate and become infective, according to CDC DPDx. A person is an accidental host: larvae can migrate through tissues, but they do not mature into adult worms in humans.

Main exposure routes

  • Swallowing infective eggs in contaminated soil, gardens, sandpits, playgrounds or on unwashed produce.
  • Ingesting larvae in undercooked meat or viscera from paratenic animals.
  • Transferring contaminated material from litter, soil or surfaces to the mouth through unwashed hands.

Direct pet contact is considered a possible route, but touching a cat is not the same as swallowing an infective egg. Freshly deposited feces are less likely to contain fully infective eggs than feces left in the environment long enough to mature.

What illness can cause

Most infections are asymptomatic or mild. When disease develops, CDC identifies two principal syndromes.

Visceral larva migrans

Visceral larva migrans (VLM) occurs when larvae travel through organs. It is seen mostly in preschool children and can involve fever, cough, muscle aches, weight loss, rash, an enlarged liver or spleen, and eosinophilia—a high level of eosinophil white blood cells.

Ocular larva migrans

Ocular larva migrans (OLM) affects the eye and may cause uveitis, retinitis or endophthalmitis. Vision can be impaired, and severe cases can lead to permanent blindness. New visual symptoms, especially in a child, require prompt medical evaluation.

What the human 19% figure actually means

A separate 2019 global meta-analysis reviewed 250 studies involving 265,327 people in 71 countries. It estimated that 19% of participants were seropositive for Toxocara antibodies. Regional estimates were 37.7% in Africa and 8.2% in the Eastern Mediterranean region.

Seropositivity means that an antibody test found evidence of exposure or infection at some point. It is not a count of people who are currently ill, and it does not show that 19% have active toxocariasis. Available human tests generally cannot distinguish T. cati from T. canis, the dog roundworm, so the proportion attributable specifically to cats remains unknown.

Who faces higher exposure risk

Risk follows environmental contamination, sanitation, socioeconomic conditions, age and geography rather than cat ownership alone. In a U.S. NHANES analysis covering 2011–2014, age-standardized Toxocara seroprevalence among people aged six and older was 5.0% (95% confidence interval, 4.2–5.8%), compared with 13.9% in NHANES III from 1988–1994.

The later analysis found higher seropositivity among older people, non-Hispanic Black and Hispanic participants, males, people below the poverty level, people living in crowded households, people with less than a college education, and people born outside the United States. These are population-level associations, not a prediction that any individual in one of these groups is infected.

How to reduce the risk at home

  1. Remove feces promptly. Eggs need time in the environment to become infective, so daily litter cleaning and prompt removal from yards or gardens limits maturation.
  2. Wash hands thoroughly. Use soap and water after cleaning litter, handling soil or gardening, and before eating or preparing food.
  3. Protect children from contaminated ground. Keep sandboxes covered when not in use and prevent children from putting soil or dirty hands in their mouths.
  4. Wash produce. Rinse raw vegetables and herbs that may have contacted contaminated soil.
  5. Cook meat fully. Undercooked meat or viscera from infected paratenic animals can contain larvae.
  6. Arrange veterinary care. A veterinarian can diagnose and treat roundworms and advise on parasite control. Cornell notes that reinfection after treatment is common, so prevention and follow-up matter.
  7. Limit hunting where feasible. Preventing cats from eating infected prey can reduce exposure to paratenic hosts.

When a test result needs medical interpretation

A positive antibody result should be discussed with a clinician alongside symptoms, eye findings, blood counts and exposure history. By itself, it does not establish active disease or identify whether exposure came from T. cati or T. canis. Anyone with unexplained symptoms such as persistent fever, abdominal or respiratory complaints, marked eosinophilia, or visual changes should seek medical care rather than self-diagnose from a cat’s parasite status.

The key distinction from toxoplasmosis

Toxocara cati is the cat roundworm highlighted by the large feline review. It is distinct from Toxoplasma gondii, the parasite most people associate with cats and pregnancy. The numbers above concern Toxocara; they should not be presented as rates of toxoplasmosis. Likewise, the global 19% antibody estimate cannot be converted into a percentage of people sick from cat roundworms.
