What does 0 sanity do in The Forest?
The Forest Wiki’s community-maintained Sanity reference describes sanity as a percentage from 0% to 100% and says it has no effect on the player beyond allowing effigy access below 90%. It does not describe reaching zero as causing death, a forced game over, impaired crafting, increased vulnerability, or permanent damage.

The same page mentions auditory hallucinations only as an unconfirmed theory. Treat claims that zero sanity makes the character hallucinate or suffer lasting consequences as unestablished, rather than as documented mechanics. The wiki is not direct developer documentation, and no gameplay test is being presented here.

No. The cited Stats page identifies the relevant threshold as falling below 90%, not reaching 0%. The distinction matters: the stat’s minimum is not the same thing as the reported effigy unlock condition.

There is also a version issue. The community-maintained Effigy page says effigies were removed from the crafting/build menu in version 1.0, the full release dated April 30, 2018. So an older unlock description does not guarantee that effigies can be crafted in the version you are playing. Check your game version and build menu rather than relying on the sanity threshold alone.
The Effigy page describes effigies as intended to frighten or deter cannibals, but also notes inconsistent behavior. That is secondary wiki guidance, not a guaranteed defensive result.

What raises or lowers sanity?
The Sanity page lists the following actions as affecting the stat. These are reported mechanics from a community-maintained reference, not independently tested results:

How should you interpret claims about “losing all sanity”?
For the game-specific mechanics described in these references, the useful distinction is between a low stat and a documented consequence. The wiki describes a 0–100% value and an effigy-related condition below 90%; it does not substantiate a special event at zero. Likewise, its mention of auditory hallucinations is explicitly unconfirmed. If you see stronger claims, check whether they identify a specific game version and provide evidence beyond speculation.
