Testosterone replacement therapy (TRT) is appropriate only when a clinician confirms hypogonadism with compatible symptoms or signs and consistently low morning testosterone on repeat testing. If obesity, a medication, or another reversible factor is the likely cause, correcting that cause—often beginning with weight loss—is usually preferred. TRT is not an anti-aging treatment, and over-the-counter “testosterone boosters” are not equivalent to prescription testosterone.
What TRT treats—and what it does not
TRT supplies testosterone for men whose bodies do not produce enough because of a medical disorder affecting the testes, pituitary gland, or related systems. The U.S. Food and Drug Administration (FDA) approves testosterone products only for men with low testosterone together with an associated medical condition. Approved forms include topical gel, a transdermal patch, a buccal system applied to the gum, and injections.

The Endocrine Society’s diagnostic standard requires both parts of the diagnosis:
- Symptoms or signs that fit testosterone deficiency.
- “Unequivocally and consistently low” serum total testosterone and/or free testosterone, confirmed with repeat morning, fasting testing.
Symptoms by themselves are not diagnostic. Low energy, reduced libido, mood changes, sleep problems, and changes in body composition have many possible causes. A clinician should assess the underlying testicular or pituitary problem and look for reversible contributors before prescribing TRT.

That distinction matters because taking testosterone when production is normal can expose a person to treatment risks without treating the reason for the symptoms. It can also interfere with fertility and create a monitoring obligation that continues for as long as treatment continues.
What “natural treatment” should mean
In this context, natural treatment means correcting an identifiable cause or health factor rather than replacing testosterone with an unproven supplement. It is cause-directed care, not a promise that lifestyle changes can restore testosterone in every case.
Weight loss when excess weight is the likely cause
The Endocrine Society’s 2026 statement says that for hypogonadism associated only with overweight or obesity (body mass index above 27) and with no other identified cause, weight loss is typically first-line therapy. The appropriate plan depends on a person’s health, medications, eating pattern, activity, and ability to sustain changes. The goal is to address the condition associated with the low measurement, then reassess symptoms and testosterone with a clinician.

Reviewing medication and medical contributors
Clinicians should investigate contributors such as opioid or corticosteroid use, both of which the Endocrine Society identifies as potentially reversible factors. Do not stop a prescribed medicine on your own; ask the prescriber whether a dose change, alternative, or treatment of the underlying condition is possible.
Why “testosterone boosters” are a separate category
Over-the-counter products marketed as testosterone boosters are not FDA-approved testosterone replacement. The authoritative sources summarized here do not establish an equivalent benefit or endorse a specific booster. A supplement label, “natural” claim, or online symptom quiz cannot establish hypogonadism or replace laboratory confirmation.
TRT and natural, cause-directed care compared
| Decision factor | TRT | Natural or cause-directed care |
|---|---|---|
| Who it is for | Men with compatible symptoms or signs plus repeatedly and unequivocally low testosterone linked to a medical condition | Men whose low result or symptoms may be related to reversible contributors such as obesity, opioid use, or corticosteroid use |
| Expected benefit | Can induce and maintain male secondary sex characteristics and correct symptoms of confirmed deficiency | May improve testosterone and symptoms when the underlying contributor responds; not a guaranteed replacement for treatment of testicular or pituitary disease |
| Fertility | Requires a specific fertility discussion; guidelines advise against starting exogenous testosterone in men trying to conceive | Addresses the contributor without intentionally supplying exogenous testosterone, but fertility still needs individual evaluation |
| Reversibility | Stopping requires medical guidance, and the effects and recovery of the body’s own production vary by cause and individual | Targets a changeable contributor, although the underlying condition may persist |
| Monitoring | Symptoms, adverse effects, adherence, serum testosterone, hematocrit, and prostate-cancer risk assessment during the first year | Follow-up testing and treatment of the identified cause; the schedule depends on the condition being managed |
| Contraindications and cautions | Several situations require avoiding TRT or completing further evaluation first, including certain prostate or breast findings, high hematocrit, severe untreated sleep apnea, recent cardiovascular events, and near-term fertility plans | Still requires medical assessment; changing weight or medication without supervision can be unsafe |
| Evidence basis | Prescription therapy supported for appropriately diagnosed hypogonadism | Evidence depends on the cause; weight loss and correction of contributors are not interchangeable with treatment for organic hypogonadism |
How clinicians determine whether you qualify
Qualification is a clinical process, not a checklist. A sensible evaluation usually follows these steps.

Mention libido, sexual function, energy, mood, sleep, medication use, previous testicular or pituitary problems, and cardiovascular history. Symptoms guide the evaluation but do not prove testosterone deficiency.
- Obtain an initial morning, fasting testosterone test. Testosterone varies during the day and can be affected by illness, sleep, food intake, and other factors. The Endocrine Society recommends morning fasting measurement.
- Repeat the measurement if it is low. A single low result is not enough. Repeat morning testing is recommended to establish that the concentration is consistently low. Depending on the clinical situation, the clinician may assess total testosterone, free testosterone, or both.
- Look for the cause. The clinician should distinguish a testicular problem from a pituitary or other medical cause and review potentially reversible contributors, including obesity and medicines such as opioids or corticosteroids. Further testing is selected according to the history and examination.
- Establish safety before prescribing. Discuss fertility plans, prostate symptoms and risk, sleep apnea, blood counts, heart failure, previous heart attack or stroke, clotting risk, and all medicines. Baseline findings can make TRT inappropriate or require specialist evaluation.
- Agree on a monitoring plan. Before starting, clarify which formulation is being considered, how results will be checked, what side effects require a call, and when treatment will be reconsidered if symptoms do not improve.
Testing should be interpreted in context. A result obtained during an acute illness, after an atypical night of sleep, or without the recommended morning and fasting conditions may need confirmation rather than an immediate treatment decision.

Who should not start TRT without further evaluation
The 2018 Endocrine Society guideline recommends against starting TRT in men with any of the following situations unless the issue is addressed through appropriate evaluation:
- A plan to pursue fertility in the near term.
- Breast cancer or prostate cancer.
- A palpable prostate nodule or area of induration.
- Prostate-specific antigen (PSA) above 4 ng/mL.
- PSA above 3 ng/mL combined with high prostate-cancer risk when urological evaluation has not been completed.
- Elevated hematocrit.
- Untreated severe obstructive sleep apnea.
- Severe lower urinary tract symptoms.
- Uncontrolled heart failure.
- Myocardial infarction or stroke within the previous six months.
- Thrombophilia.
The American Urological Association also says exogenous testosterone should not be prescribed to men who are currently trying to conceive. If topical testosterone is used, the AUA recommends discussing the possibility of transfer to other people through skin contact; application and covering instructions therefore matter.
Potential benefits when hypogonadism is confirmed
For appropriately diagnosed hypogonadism, the Endocrine Society recommends testosterone to induce and maintain male secondary sex characteristics and to correct symptoms of testosterone deficiency. The relevant question is whether a documented deficiency and an appropriate cause make treatment more likely to help than to harm—not whether testosterone can produce a general “optimization” effect in someone with normal levels.
Before treatment, ask which symptoms are plausibly related to low testosterone and which may have another explanation. Low mood, fatigue, reduced sexual interest, and poor sleep can arise from depression, sleep disorders, medication effects, chronic disease, relationship stress, or other causes. Treating testosterone alone may leave those problems unaddressed.

Risks and what current safety evidence shows
Common treatment-related effects
The Endocrine Society guideline reports acne or oily skin, breast tenderness, fluid retention, and erythrocytosis among commonly reported effects. Erythrocytosis—an abnormal rise in red blood cell concentration—is the most frequent adverse event reported in randomized trials cited by the guideline. That is why hematocrit is part of routine monitoring and why an elevated baseline hematocrit is a reason to evaluate before starting.
Fertility and reproductive planning
Exogenous testosterone is not a fertility treatment. Because fertility can be impaired during treatment, men who want a child soon should tell the clinician before any prescription is written. The guideline recommendation against TRT for near-term fertility plans is a qualification issue, not a minor side-effect warning.
Cardiovascular outcomes
The FDA’s TRAVERSE safety summary followed more than 5,200 men. Major adverse cardiovascular events occurred in 7.0% of men receiving AndroGel 1.62% and 7.3% of men receiving placebo, with no meaningful difference in serious cardiovascular events over the trial period. These figures describe that trial population and duration; they do not make TRT risk-free for every patient.
The Endocrine Society continues to emphasize individualized cardiovascular risk assessment. Its patient information also says that long-term cardiovascular and prostate effects remain uncertain. A history of heart failure, a recent heart attack or stroke, uncontrolled cardiovascular disease, or multiple risk factors should be discussed before treatment.
Best Value
Blood clots and pulmonary embolism
The Endocrine Society’s 2026 statement discusses a roughly 50% relative increase in pulmonary embolism among testosterone-treated men in the TRAVERSE safety discussion. The statement also notes the limitation and trial-duration context of that figure. Relative risk should not be converted into an individual prediction without knowing baseline risk, clotting history, and the study’s population and follow-up.
Prostate health
Prostate cancer and certain prostate findings are reasons not to begin TRT without evaluation. For men who do start, the guideline calls for prostate-cancer risk assessment during the first year. Long-term prostate effects remain uncertain, so urinary symptoms, examination findings, and PSA results should be reviewed according to the clinician’s monitoring plan.
Monitoring after treatment begins
TRT is an ongoing therapy decision, not a prescription that ends the evaluation. Monitoring should cover:
Do these 3 things before closing this tab:
1Fix the driver behind crashes, sound loss and screen glitches2Repair Windows errors before they cause bigger problems3Scan for outdated or missing drivers - takes under a minute- Symptoms and function: whether the problems that prompted treatment are improving and whether new symptoms have appeared.
- Adverse effects: acne or oily skin, breast tenderness, fluid retention, changes in urination, breathing problems during sleep, and other concerns.
- Adherence and administration: whether the prescribed gel, patch, buccal system, or injection is being used as directed, and whether topical product could transfer to another person.
- Serum testosterone: whether the measured concentration is appropriate for the prescribed formulation and dosing schedule.
- Hematocrit: to detect erythrocytosis and guide decisions if the blood count rises.
- Prostate-cancer risk: assessment during the first year, with follow-up tailored to baseline risk and findings.
Ask in advance what result or symptom would lead to a dose change, a pause, a different formulation, or referral to urology or endocrinology. If the original symptoms do not improve despite appropriately monitored levels, reconsidering the diagnosis is more useful than simply escalating the dose.
A practical decision framework
TRT is more likely to be appropriate when
- You have symptoms or signs that fit testosterone deficiency.
- Two properly timed morning tests, interpreted by a clinician, show consistently low testosterone.
- An underlying medical condition explains the deficiency.
- Fertility is not a near-term goal, or a specialist has provided an alternative plan.
- Prostate, blood-count, sleep-apnea, cardiovascular, and clotting risks have been evaluated.
- You accept regular monitoring and understand the possible adverse effects.
Cause-directed care is the better first move when
- Overweight or obesity is the only identified contributor, particularly with a body mass index above 27.
- An opioid or corticosteroid may be suppressing testosterone and can be reviewed safely with the prescribing clinician.
- The low result has not been repeated under appropriate morning, fasting conditions.
- Symptoms are nonspecific and another medical or psychological cause has not been assessed.
- You are trying to conceive in the near term.
Pause and obtain specialist evaluation when
- PSA, prostate examination, hematocrit, sleep-apnea status, or cardiovascular history raises a contraindication.
- The cause could be pituitary or testicular disease that needs targeted investigation.
- You are considering an online prescription based only on symptoms or a single laboratory result.
Questions to take to an appointment
- Were both testosterone samples drawn in the morning and while fasting?
- Do my symptoms and results meet the diagnostic criteria for hypogonadism?
- What condition is causing the low level, and which reversible contributors have been checked?
- Could treatment affect my ability to conceive, and what alternatives exist if fertility is a priority?
- What are my baseline hematocrit, PSA, prostate findings, sleep-apnea status, and cardiovascular risks?
- Which formulation is being considered, how is it administered, and how can transfer to another person be prevented?
- When will testosterone, hematocrit, symptoms, adverse effects, and prostate risk be reassessed?
- What finding would make us stop, change, or reconsider TRT?
Bottom line for choosing between TRT and natural treatment
Use TRT for confirmed hypogonadism after repeat morning testing and evaluation of the cause and safety risks. Use weight loss and correction of medication or obesity-related contributors first when those are the likely explanation, while recognizing that lifestyle change cannot replace treatment for every testicular or pituitary disorder. Neither symptoms alone nor an over-the-counter booster establishes a diagnosis. The safest choice is the one made with documented laboratory evidence, a fertility and prostate discussion, and a monitoring plan that continues after the prescription is written.
