Bill Lee announced Saturday, October 3, 2026, days after the failed execution attempt involving Christa Pike. Lee has ordered an independent review, but its findings are not yet available. The state has not named a successor or specified Strada’s final work date.
Why is Tennessee’s prison chief resigning?
Strada said he was resigning in the state’s interests while the independent review proceeds. He said, “I believe this review is entirely appropriate and necessary, and I look forward to doing everything I can to help determine exactly what occurred.” The announcement makes his departure the first major personnel change following the September 30 execution attempt; it does not say he was found responsible for what happened.
Lee said carrying out a lawful sentence is among the state’s most serious responsibilities and that Tennesseans expect it to be legal, constitutional and effective. His announcement did not give an exact departure date or identify who will lead the department next.
What happened during Christa Pike’s execution attempt?
The attempt took place on September 30, 2026. In a same-night advisory, the Tennessee Department of Correction said Pike had been taken to an off-site medical facility. The agency also asserted that it “followed every step of the State’s lawful, established execution protocol approved by the Attorney General’s Office.” That is the department’s account, not an independent finding about why the attempt failed. The Associated Press, citing Pike’s attorneys and court filings, reported that the state administered two doses of pentobarbital and that Pike was taken to a hospital. Her lawyers later said she was unconscious and on a ventilator. Those details are attributed to counsel; the available reporting does not provide a completed independent medical account or establish a cause for the failure. AP’s September 30 report
What will the independent review establish?
Lee ordered a third-party review to determine what happened and halted the remaining scheduled executions for the rest of 2026. As of October 3, the review had not reported findings. The accounts currently available should therefore be kept distinct: TDOC says it followed its established protocol, while Pike’s attorneys and court filings provide the reported two-dose account. Neither account, on its own, settles the cause. AP’s report on the review order

How does this fit Tennessee’s recent execution history?
Tennessee’s official death-penalty history says a December 2024 lethal-injection revision adopted the single-drug pentobarbital protocol and executions resumed in May 2025. That policy background does not explain the Pike case; the state’s assertion about following protocol and the attorneys’ account of two doses remain separately attributed pending the review. Tennessee’s death-penalty history
The Pike attempt was Tennessee’s second unsuccessful execution attempt in four months, according to AP’s October 2 report. Earlier in 2026, officials called off Tony Carruthers’ execution after they could not establish an IV. Tennessee had also paused executions after the 2022 Oscar Smith case before later revising its protocol. The reported episodes have different known circumstances, and the available information does not establish a shared cause. AP’s October 2 report
