A peer-reviewed case report describes a 16-year-old girl whose abrupt behavioral change, falling school performance and social withdrawal were followed by vivid auditory and visual hallucinations—including a flying green dinosaur. Doctors identified the condition as pediatric anti-neuronal intermediate filament (anti-NIF) autoimmune encephalitis, a rare immune-mediated brain disorder. The case illustrates why sudden psychosis-like symptoms in a young person can require neurological and autoimmune evaluation, not only a psychiatric assessment.
What happened to the teenager
The girl’s symptoms appeared abruptly rather than as a long-established pattern. The case description includes bizarre behavior, reduced performance at school and withdrawal from other people. She then reported both seeing and hearing things that were not present.

“The patient reported auditory and visual hallucinations of a flying green dinosaur, hearing voices that told her to hurt herself, and a sense that people were following her.”

That combination is clinically important for two reasons. The hallucinations were multimodal—visual and auditory—and they occurred alongside a rapid change in behavior and functioning. Voices telling someone to self-harm also represent an immediate safety concern, regardless of whether the eventual cause is psychiatric, neurological or immune-mediated.

The report identifies the diagnosis as pediatric anti-neuronal intermediate filament autoimmune encephalitis. It should not be relabeled as anti-NMDA receptor encephalitis: anti-NIF and anti-NMDA receptor encephalitis are different antibody-associated conditions within the broader autoimmune-encephalitis group.
What anti-NIF autoimmune encephalitis means
Autoimmune encephalitis occurs when an abnormal immune response inflames or disrupts brain function. Depending on the subtype, inflammation can affect behavior, perception, memory, speech, movement, consciousness and the autonomic nervous system. Anti-NIF autoimmune encephalitis is defined by antibodies directed at neuronal intermediate filament proteins. The teenager’s case is notable because the first prominent manifestations looked psychiatric, even though the reported diagnosis was neurological and immune-mediated.
“Autoimmune” does not mean that every hallucination is caused by an immune disorder. Hallucinations have many possible causes, and no single symptom proves autoimmune encephalitis. The diagnostic clue in a case like this is the overall pattern: a previously functioning young person develops a rapid, severe change in behavior and perception, prompting clinicians to look for brain inflammation and other medical explanations.

Why a first psychosis-like episode can need neurological testing
Clinicians generally examine the time course and search for accompanying neurological signs rather than relying on the content of a hallucination. Features that can raise concern for autoimmune encephalitis include:
- Very rapid onset of confusion, bizarre behavior or hallucinations.
- Seizures or episodes of impaired awareness.
- New problems with speech, memory or level of consciousness.
- Abnormal movements, severe agitation or catatonia.
- Autonomic instability, such as major fluctuations in heart rate, blood pressure or temperature.
These findings are warning signs, not a checklist that confirms a diagnosis. A psychiatric disorder can include unusual behavior or hallucinations, and autoimmune encephalitis can begin without every neurological feature listed above. Doctors integrate the history, examination and test results.
How clinicians investigate the possibility
When the presentation is abrupt or medically unusual, evaluation may extend beyond a standard psychiatric assessment. Reported autoimmune-encephalitis workups can include cerebrospinal-fluid analysis and antibody testing, along with neurological assessment and other investigations selected for the patient’s symptoms. The purpose is to look for evidence of inflammation, infection, seizures or another brain disorder while also assessing immediate risks such as self-harm.

Immunotherapy is commonly used for confirmed autoimmune encephalitis in published clinical reports. That general treatment principle should not be read as a description of this teenager’s care. The available case summary does not establish her complete antibody panel, cerebrospinal-fluid findings, brain imaging, treatment sequence or long-term recovery.

Anti-NMDA receptor encephalitis is the better-known autoimmune-encephalitis subtype and provides useful clinical context, but it is not the diagnosis reported for the flying-dinosaur case. In anti-NMDA reports, patients may develop delusions, hallucinations, agitation, insomnia, disorganized thinking, catatonia, seizures, memory problems, speech disturbance, dyskinesia and autonomic dysregulation. Cerebrospinal-fluid antibody testing is used in reported cases, and treatment commonly involves immunotherapy.
| Feature | Teenager’s reported anti-NIF case | Features described in related anti-NMDA reports |
|---|---|---|
| Age and presentation | 16-year-old girl with abrupt behavioral change, reduced school performance and social withdrawal | Neuropsychiatric presentations reported across case literature |
| Perception and behavior | Visual and auditory hallucinations, including a flying green dinosaur; persecutory feeling; voices urging self-harm | Hallucinations, delusions, agitation, insomnia and disorganized thinking |
| Neurological or physical features | The available case summary does not establish seizures, abnormal movements, speech changes or autonomic findings | Seizures, speech disturbance, dyskinesia, catatonia and autonomic dysregulation are described |
| Immune marker | Identified as anti-neuronal intermediate filament (anti-NIF) autoimmune encephalitis | Anti-NMDA receptor antibodies in the related syndrome |
A 2022 anti-NMDA case report by Chamlagain and colleagues gives a figure of one case per 1.5 million. That number belongs to the related anti-NMDA syndrome and is not a prevalence estimate for anti-NIF autoimmune encephalitis.

What the published account does—and does not—establish
The case supports a clear clinical lesson: a dramatic new psychosis-like state in an adolescent can have an immune or neurological cause. It does not, by itself, establish how often anti-NIF encephalitis occurs, which test result confirmed the diagnosis, what scans showed, which medicines were given or whether the patient made a complete recovery. Those details require the full case report rather than a short description.
It is also unsafe to infer that the dinosaur image has a special diagnostic meaning. Hallucination content varies from person to person. The medically relevant facts are the sudden onset, the combination of visual and auditory symptoms, the functional decline and the associated behavioral change.
When urgent assessment is warranted
A young person who suddenly begins hallucinating, becomes severely confused, has a seizure, develops abnormal movements, cannot speak normally, becomes unresponsive or hears commands to self-harm needs urgent medical attention. Families should tell clinicians when the change began, how quickly it progressed, any recent infection or medication exposure, sleep loss, substance use, seizures or unusual movements, and whether the person’s memory or speech has changed.

Those steps do not diagnose autoimmune encephalitis. They help emergency and specialist teams decide how quickly to investigate dangerous medical, neurological and psychiatric causes while protecting the patient from immediate harm.
The central lesson
The flying-green-dinosaur report is a reminder that “psychosis” can describe an appearance, not a final explanation. In this teenager, the reported diagnosis was anti-NIF autoimmune encephalitis, a distinct immune-mediated brain disorder. Abrupt hallucinations and behavioral decline should therefore prompt a broad medical evaluation, with anti-NIF kept distinct from anti-NMDA and with conclusions based on documented testing rather than on one striking symptom.
