Targeted therapy attacks a cancer-related molecule or signal; immunotherapy uses or changes immune activity so the body can recognize and attack cancer. Those are the primary distinctions, but the labels are broad. Some targeted medicines recruit immune cells, and different immunotherapies work in very different ways. A treatment’s name alone cannot tell you whether it is appropriate, effective, or safer for a particular person.

The useful comparison is between the specific medicine, the cancer’s features, the evidence for that use, and the patient’s health and treatment history. Biomarker testing can help identify options, but a matching result is not a promise that treatment will work.

What is the basic difference?

Question Targeted therapy Immunotherapy
Primary action Blocks or disrupts a selected protein, mutation, pathway, or other cancer-associated feature that helps cells grow, divide, survive, or spread. Activates, releases, redirects, or supplies immune activity to help recognize and attack cancer.
Typical selection clue A tumor has the molecular target that the drug is designed to affect. The cancer has features, such as certain proteins or mutation patterns, that may make an immune approach relevant; some treatments do not require a known biomarker.
How benefit is evaluated Imaging, examinations, laboratory results, symptoms, and sometimes a molecular test showing that the target remains present. The same clinical assessments, interpreted with awareness that immune responses can have unusual timing and that inflammation can affect scans.
Main safety concern The drug can affect healthy cells that share the target or rely on a related pathway. An activated immune system can attack healthy tissues as well as cancer.
Can resistance occur? Yes. The cancer may lack the target from the start or acquire changes that bypass the drug or alter the target. Yes. Some tumors avoid immune recognition, suppress immune activity, or evolve after an initial response.

These are categories, not two completely separate boxes. A monoclonal antibody used as a targeted treatment can block a growth signal, mark a cancer cell for immune destruction, or carry a toxic payload to a cell bearing the target. In that sense, a targeted treatment may involve the immune system even though its defining feature is the selected target. “Immunotherapy” is also an umbrella term: an immune checkpoint inhibitor and a cell-based treatment change immunity through different mechanisms and have different monitoring needs.

How targeted therapy works

Finding and blocking a cancer dependency

Cancer cells often carry changes or produce unusually high amounts of proteins that control growth, division, repair, blood-vessel formation, or survival. A targeted drug is designed to interfere with one of those features. The target may be a mutated gene product, a receptor on the cell surface, an enzyme inside the cell, or a signal that cancer cells use to communicate.

Small-molecule targeted drugs are able to enter cells and act on internal targets. Monoclonal antibodies are larger laboratory-made proteins that bind selected targets on the outside of cells or in the tissue around them. Depending on the medicine, an antibody can block a growth message, flag a cancer cell for immune attack, or deliver a toxin or radioactive substance to cells carrying the target. The actual drug mechanism matters more than the broad label.

Why the target matters

A targeted drug generally has a plausible chance of helping only when the cancer has the feature the drug is designed to affect, although the presence of a target does not ensure benefit. A tumor can contain the target in too few cells, use an alternate growth pathway, or have other biological characteristics that prevent a response. The cancer may also change during treatment, allowing it to grow despite continued dosing.

Examples of effects and limitations

Depending on the pathway involved, targeted medicines can produce effects such as skin or nail changes, diarrhea, high blood pressure, liver or other organ problems, or changes in blood counts. These are examples across the category, not a prediction for a particular drug. The prescribing information for the exact medicine and the oncology team’s instructions determine which symptoms require urgent attention.

How immunotherapy works

Releasing immune brakes

Some cancers use checkpoint signals to make immune cells stand down. Checkpoint inhibitors block those inhibitory interactions, allowing T cells or other immune components to remain active against tumor cells. This does not mean every tumor will be recognized or that an immune response will be strong enough to shrink it.

Supplying or redirecting immune cells

Other immunotherapies provide immune-stimulating substances, antibodies that recruit immune activity, or cells prepared to recognize cancer. Cell-based approaches can involve collecting immune cells, modifying or expanding them, and returning them to the patient. Each approach has its own eligibility rules, preparation, possible complications, and follow-up.

Why immunotherapy is not one treatment

The word “immunotherapy” covers multiple families of treatment. A checkpoint inhibitor, a cell-based therapy, and an immune-stimulating drug do not have identical response patterns or side effects. Even within one family, the dose, combination, cancer type, and treatment sequence can change the balance of benefit and risk.

Does biomarker testing matter?

What a biomarker can show

The National Cancer Institute states: “Some cancer treatments, including targeted therapies and immunotherapies, may only work for people whose cancers have certain biomarkers.” A biomarker is a measurable feature of a tumor or, in some cases, the patient that can provide information about diagnosis, prognosis, or treatment selection.

For targeted therapy, testing may look for the molecular change or protein that a drug is designed to affect. The NCI gives an EGFR gene change as an example: finding that change may make an EGFR inhibitor an option in the appropriate cancer setting. For immunotherapy, testing may assess features that can inform whether some checkpoint inhibitors or other immune treatments are reasonable options. The relevance of a result depends on the exact test, cancer type, stage, treatment line, and medicine.

How testing is performed

Most biomarker tests use tumor tissue obtained during a biopsy or surgery. In some situations, a liquid biopsy analyzes tumor-related material circulating in blood. A tissue sample can be limited, degraded, or unrepresentative of every tumor site, while a blood test may fail to detect a feature that is present at a low level. The team may recommend one test, repeat testing, or a different specimen when the result does not fit the clinical picture.

What a result can and cannot authorize

A result can identify a possible approved treatment, suggest an off-label option, or support consideration of a clinical trial. It is not itself a prescription. The result also does not guarantee that the cancer will respond, that the benefit will last, or that the expected risks are acceptable for a particular patient. Biomarkers can change as cancer evolves, and testing does not provide a useful answer for every person.

Questions to ask about a test report

  • What alteration, protein level, or other feature was detected, and in which sample?
  • Is the result required for the proposed medicine, or does it only provide supporting information?
  • Was the test validated for this cancer and treatment decision?
  • Does the result correspond to an approved use, an off-label use, or a clinical trial?
  • Could the biomarker differ in a recurrence or another tumor site?

How side effects differ

Targeted-treatment toxicities

Targeted drugs are selective, not perfectly selective. A protein that drives cancer may also be present in healthy tissue, or a normal organ may depend on the same pathway. As a result, side effects vary widely by medicine. Across targeted therapies, clinicians may monitor skin and nail changes, diarrhea, blood pressure, liver function, heart function, blood counts, or other organ effects.

The schedule matters. Some effects begin soon after treatment; others appear after cumulative exposure or when two medicines are combined. Dose changes, treatment pauses, supportive medicines, or switching therapy may be considered, but the correct response depends on the drug and the severity of the problem. Do not stop a prescribed treatment without contacting the treating team.

Immune-related side effects

Immunotherapy can cause inflammation when activated immune cells affect healthy organs. Possible sites include the skin, bowel, liver, lungs, endocrine glands, kidneys, nerves, heart, or other tissues. Symptoms can be mild or life-threatening, and they may occur during treatment or after a dose has ended. The risk varies with the immunotherapy, dose, combinations, cancer context, and the patient’s health.

New or worsening diarrhea, severe abdominal pain, cough or shortness of breath, yellowing of the skin, unusual fatigue, severe rash, confusion, weakness, changes in urination, or other unexpected symptoms should be reported according to the team’s instructions. Some immune-related effects require prompt evaluation and medicines that suppress inflammation. The patient information for the actual medicine gives the most reliable warning list and contact guidance.

Neither category is automatically gentler

It is inaccurate to call all targeted therapy mild or all immunotherapy dangerous. A targeted drug can cause serious organ toxicity, and an immune-related reaction can be severe even when the treatment initially seemed easy to tolerate. Conversely, many people receive either category without a serious complication. Risk is treatment-specific and patient-specific, so monitoring plans should be discussed before the first dose.

Response, resistance, and what happens if treatment stops working

Assessing response

The oncology team may combine scans, physical examinations, symptoms, blood tests, and other disease-specific measurements to judge whether treatment is helping. A biomarker result can support the rationale for treatment but cannot substitute for follow-up. The timing and interpretation of scans are set by the treatment protocol and the cancer’s behavior.

Why targeted therapy can fail

If the intended target is absent, the drug may have little or no effect. Even when the tumor initially shrinks, cancer cells can acquire additional changes, activate a bypass pathway, or alter the target so the medicine no longer binds effectively. A later biopsy or liquid biopsy may sometimes identify a resistance mechanism, but testing is not informative in every case.

Why immunotherapy can fail

Some tumors prevent immune cells from entering, create an immunosuppressive environment, or fail to display features that immune cells can recognize. Other cancers evolve after an initial response and become resistant. Response varies considerably among patients, and there is no universal response-rate winner between the two categories across all cancers.

Options after progression

If the cancer grows or side effects become unacceptable, the team may review whether the diagnosis or biomarker should be reassessed, change the dose or schedule, switch to another treatment class, combine treatments, or consider a clinical trial. The available choices depend on cancer type and subtype, where it has spread, previous therapies, current organ function, other medicines, and the person’s goals.

How doctors choose between the categories

A category-level comparison cannot select treatment. The decision usually integrates the following factors:

  • Cancer identity and extent: the organ of origin, molecular subtype, stage, spread pattern, and whether the disease is newly diagnosed, recurrent, or resistant.
  • Evidence for the exact use: results from studies and regulatory approvals for that medicine in that cancer and treatment setting.
  • Biomarker information: whether a required or informative feature is present, how reliable the test is, and what uncertainty remains.
  • Previous treatment: earlier drugs, radiation, surgery, response duration, resistance findings, and lingering toxicities.
  • Health context: organ function, autoimmune disease, transplant history, infections, other medicines, pregnancy potential, and ability to attend monitoring visits.
  • Practical and personal priorities: infusion or pill schedules, travel, support at home, symptom tolerance, and the patient’s goals.

Two people with the same broad cancer type may therefore receive different approaches. A treatment that is reasonable after one biomarker result or treatment history may be unsuitable after another. The care team weighs expected benefit against known and uncertain risks rather than choosing based on the category name.

A practical comparison for an oncology appointment

Use the following framework to compare the actual options offered to you:

  1. Name the treatment. Ask for the generic name, whether it is a small molecule, antibody, checkpoint inhibitor, cell-based treatment, or combination, and how it acts in your cancer.
  2. Identify the decision-driving evidence. Ask which biomarker or clinical feature supports the option, whether it is required, and how the test was performed.
  3. Clarify expected benefit. Discuss the goal—shrinkage, disease control, symptom relief, preventing recurrence, or another endpoint—and how success will be measured.
  4. Map the safety plan. Ask which blood tests, scans, examinations, and specialist reviews are needed; when side effects usually occur; and which symptoms require a same-day call or emergency care.
  5. Review alternatives. Ask what other approved treatments, combinations, supportive-care approaches, or clinical trials fit the diagnosis and treatment history.
  6. Plan for nonresponse. Ask when the first assessment will occur and what could be considered if the cancer progresses or the medicine must be stopped.

Questions worth taking to the care team

  • What is the exact name of the treatment, and what does it target or change in my cancer?
  • Which biomarker results support or rule out this option, and what are the limits of those results?
  • What benefits and risks are known for my diagnosis, stage, and prior treatments?
  • Which symptoms should prompt a call, whom should I contact after hours, and when should I seek emergency care?
  • How will you tell whether the treatment is working, and when will that assessment occur?
  • What could be considered if the cancer does not respond, later progresses, or side effects become unacceptable?

Bottom line for the comparison

Targeted therapy is defined mainly by the cancer feature it attacks; immunotherapy is defined mainly by the way it engages or changes immune activity. Biomarker testing can make either type more relevant, but a result is not a guarantee. Both categories contain treatments with distinct benefits, toxicities, monitoring requirements, and resistance patterns. The appropriate choice comes from the specific medicine’s evidence, the cancer’s current biology, and the patient’s health and goals—not from assuming that one label is inherently better or safer.
