Tailbone Pain vs. Lower-Back Pain: How to Tell the Difference

The quickest clue is location. Tailbone (coccyx) pain sits at the very bottom tip of the spine, near the top of the buttocks, and is usually provoked by sitting or moving from sitting to standing. Lower-back pain is higher, between the bottom ribs and the buttocks; it is more often linked to lifting, bending or pain that travels into a buttock or leg.

Those patterns are clues, not a home diagnosis. Numbness, tingling, weakness, fever, unexplained weight loss, major trauma, worsening pain, or bladder or bowel changes need medical assessment. Inability to move or suspected spinal-cord injury is an emergency.

At a glance: coccyx pain versus lumbar pain

Feature More typical of tailbone pain More typical of lower-back pain
Where it is felt At the lowest point of the spine, just above the cleft of the buttocks Between the lower edge of the ribs and the buttocks
Common triggers Sitting, sitting down or standing up, leaning forward, or having a bowel movement Lifting, bending, prolonged activity, or certain spinal movements
Radiation Usually remains local around the coccyx May spread into a buttock, hip, leg or foot
Typical causes Backward fall, pressure from a hard seat, pregnancy or childbirth, joint changes, posture or prolonged strain Often mechanical or nonspecific; nerve, disc, joint and other specific causes are possible
Neurologic features Not expected with isolated coccyx soreness Numbness, tingling or weakness can indicate nerve involvement
Initial evaluation History and examination; imaging only when a fracture, infection, tumor or another serious cause is suspected History and examination first; scans are guided by red flags, neurologic findings or persistent symptoms

A person can have both problems at once, and pain can be referred from nearby muscles or joints. Use the pattern to decide what to monitor and when to seek care rather than trying to label the condition with certainty at home.

Where exactly is the pain?

Tailbone (coccyx) location

The coccyx is the small bone at the bottom of the backbone. Pain is centered over the bony tip or immediately beside it, near the top of the buttocks. MedlinePlus describes pain in the tailbone area and pain on sitting as characteristic symptoms. The NHS calls the main symptom pain and tenderness at the base of the spine, near the top of the bottom.

You may be able to identify a very specific tender point. Pressing directly on that area, sitting on a firm chair, or leaning backward can make it obvious. Pain can also be felt slightly to either side because the ligaments and pelvic-floor muscles around the coccyx become irritated.

Lower-back (lumbar) location

Low-back pain is higher: the World Health Organization defines it as pain between the lower edge of the ribs and the buttocks. It may occupy one side or the center of the lumbar region and can feel aching, tight, sharp or burning. Discomfort may continue into the buttocks, hip, thigh or lower leg when a nerve or another structure is involved.

If the most painful spot is several inches above the buttock crease, especially with stiffness across the waist, a lumbar source is more likely than an isolated coccyx problem. Location alone still cannot identify a disc, joint, muscle or nerve cause.

Which movements and activities make it worse?

Patterns that point toward the coccyx

Coccyx pain is commonly aggravated by direct pressure. Typical situations include sitting for a long time, sitting on a hard or narrow surface, moving from sitting to standing, leaning backward or forward, and passing a bowel movement. Pregnancy and the strain of childbirth can alter the coccyx and surrounding tissues. The NHS also lists poor posture, prolonged sitting or strain, and extremes of body weight among associated factors.

A backward fall onto a hard surface is the most common injury mechanism described by MedlinePlus. Even without a fracture, the impact can bruise the bone, ligaments or muscles. Pain may start immediately after a fall or become more noticeable over the following day.

Patterns that point toward the lumbar spine

Low-back pain often changes with lifting, bending, twisting or repeated activity. Stiffness after rest is common. A lumbar nerve problem may produce an electric, burning or shooting line of pain into the buttock or leg, with symptoms changing when the spine or hip moves. NIAMS notes that low-back pain can radiate to the buttocks, legs or abdomen and may occur with numbness or weakness in the legs or feet.

There is no single movement that proves the source. A painful sit-to-stand can occur with hip or lumbar conditions, and bending can load the coccyx as well as the lower back. Keep a short record of the activity, position, duration and exact route of pain to show a clinician.

What usually causes each type?

Common coccyx causes

  • A backward fall or direct blow.
  • Repeated pressure from a hard seat, cycling or another activity that loads the coccyx.
  • Pregnancy or childbirth.
  • Arthritis-related or other changes in coccyx-joint mobility.
  • Posture, prolonged sitting, or strain affecting the pelvic-floor and nearby tissues.

Mayo Clinic notes that tailbone pain can follow falls, prolonged sitting on hard or narrow surfaces, arthritis-related joint changes and childbirth. In many cases, symptoms improve over weeks or months, but persistent or worsening pain deserves an examination.

Common lumbar causes

Most low-back pain is mechanical or nonspecific: a precise pain-generating structure cannot be identified even though the symptoms are real. The WHO reported that about 90% of low-back-pain cases are nonspecific. Muscle or ligament strain, joint irritation and disc-related pain are among the possibilities; a nerve-root disorder is considered when pain, altered sensation or weakness follows a leg distribution.

The global burden is substantial: the WHO estimated 619 million people had low-back pain in 2020 and projects 843 million cases by 2050. Those figures describe low-back pain overall, not tailbone pain, and do not indicate what is causing an individual episode.

Numbness, weakness and other warning signs

Neurologic symptoms

Isolated coccyx soreness generally causes local pain and tenderness, not numbness or weakness in a leg. Tingling, reduced sensation, loss of strength, or pain traveling below the buttock makes lumbar nerve involvement or another condition more concerning. Arrange medical review promptly if these symptoms develop, especially if they are new or progressing.

Systemic and serious symptoms

Healthdirect advises assessment for pain that worsens over weeks, severe distress, restricted movement, numbness or tingling, fever, unexplained weight loss, urinary or bowel problems, or significant trauma. These findings should not be assumed to be ordinary postural pain. Infection, fracture, tumor and other serious causes are uncommon but need to be excluded when the pattern warrants it.

When it is an emergency

Call emergency services if a spinal-cord injury is suspected, the person cannot move, or pain is severe after major trauma. New loss of bladder or bowel control, inability to pass urine, or rapidly worsening weakness also warrants emergency care rather than home treatment.

How a clinician tells them apart

History and physical examination

Healthdirect says clinicians begin by asking where the pain is, what brought it on, which movements change it, and whether other symptoms are present. They examine the spine, hips, legs and neurologic function. A localized tender coccyx with pressure-related pain supports a tailbone source; restricted lumbar movement, radiating pain, altered reflexes or weakness points the assessment toward the lower back or nerves.

For ongoing coccyx pain, Mayo Clinic notes that a clinician may perform a rectal examination to assess coccyx movement and tenderness. This is a clinical decision, not a test to perform yourself.

When imaging is useful

Routine X-rays, CT scans or MRI are usually unnecessary for straightforward coccyx pain. Healthdirect says they may be used when a fracture, infection, tumor or another serious cause needs to be ruled out. Mayo Clinic says persistent symptoms may justify MRI to examine a fracture, joint changes or, rarely, a tumor.

Low-back imaging is likewise guided by the examination and red flags rather than ordered automatically for every new ache. A scan can show age-related changes that are not the source of pain, so results must be interpreted alongside symptoms and physical findings.

What you can do for suspected tailbone pain

  1. Reduce direct pressure. Limit long sitting sessions and change position frequently. Lean slightly forward with your back supported rather than reclining onto the coccyx.
  2. Use side-lying rest. Lying on your side can unload the painful point. When sleeping, choose the position that keeps symptoms lowest.
  3. Try a coccyx cushion. A pressure-reduction or wedge-shaped cushion can off-load the coccyx while you sit. It may reduce symptoms but cannot confirm the diagnosis or repair an injury.
  4. Apply cold or heat. Use whichever gives better relief, protecting the skin and limiting sessions to a comfortable duration.
  5. Keep bowel movements comfortable. Avoid straining and discuss constipation treatment with a pharmacist or clinician if passing stool increases pain.
  6. Consider pain medicine only if suitable. The NHS and Mayo Clinic list an appropriate non-steroidal anti-inflammatory medicine, such as ibuprofen, or acetaminophen as options. Follow the package directions and ask a clinician or pharmacist first if you are pregnant, have kidney, liver, stomach or heart disease, take blood thinners, or use other medicines.

Stop an activity that sharply increases pain. A fall, severe pain or symptoms that are not improving should prompt an assessment instead of progressively stronger self-treatment.

What you can do for suspected lower-back pain

  1. Stay gently active. Short walks and normal daily movement are generally preferable to prolonged bed rest unless a clinician gives different advice.
  2. Modify the aggravating load. Temporarily reduce heavy lifting, repeated bending and twisting. When lifting is unavoidable, keep the load close and use help.
  3. Change positions regularly. Alternate sitting, standing and walking; use a chair that supports the lower back without forcing a slumped posture.
  4. Use heat or ice according to response. Protect the skin and stop if either worsens symptoms.
  5. Seek guidance for persistent or neurologic symptoms. Rehabilitation and exercise should be matched to the cause and examination findings, particularly when pain radiates or strength changes.

Because most low-back pain is nonspecific, a single product, stretch or posture cannot be expected to cure every episode. A clinician or physiotherapist can identify movement limitations and build a progressive plan when symptoms do not settle.

Choosing and using a coccyx cushion

Search for a coccyx cushion or pressure-reduction cushion rather than a generic flat seat pad. Wedge-shaped designs with a rear cut-out are intended to reduce contact at the tailbone while supporting the thighs. NHS and Mayo Clinic both mention specially designed or pressure-reduction cushions as optional measures.

  • Stays in Place + Easy to Clean: The non-slip rubber bottom ensures your cushion won’t slide on slick surfaces. Comes with a machine-washable, removable cover for easy cleaning—perfect for daily use in cars, offices, and planes.
  • Place the cushion on a stable chair so the cut-out or lower-pressure area sits beneath the coccyx, following the product instructions.
  • Keep both feet supported and avoid sliding forward into a slumped posture.
  • Stand and move at regular intervals; a cushion should not be used to tolerate hours of uninterrupted sitting.
  • Stop using it if hip, pelvic or leg symptoms increase.

A cushion can make sitting more tolerable while you arrange care. It cannot distinguish coccyx pain from lumbar pain, rule out a fracture, or substitute for evaluation of red flags.

When symptoms need a medical appointment

Arrange routine care

  • Pain persists, repeatedly returns, or is getting worse over days or weeks.
  • Sitting, walking or ordinary activities become difficult.
  • You develop numbness, tingling, pain into a leg, or any weakness.
  • The episode followed a fall, childbirth or another event that may have injured the coccyx or spine.
  • Self-care measures do not provide a clear trend toward improvement.

Seek same-day or urgent assessment

  • Fever, unexplained weight loss or feeling systemically unwell accompanies the pain.
  • There is significant trauma, severe distress or rapidly declining mobility.
  • You have new urinary or bowel problems.

Use emergency services

Do so for suspected spinal-cord injury, inability to move, severe pain after major trauma, or rapidly progressing weakness and loss of bladder or bowel control.

What treatment may involve when pain persists

Persistent coccyx pain

The NHS lists physiotherapy and corticosteroid with local-anesthetic injections after simpler measures have not worked. Surgery to remove part or all of the coccyx (coccygectomy) is reserved for cases that fail conservative treatment. Mayo Clinic also lists physical therapy, manipulation and injections, with surgery rarely needed for chronic symptoms.

Persistent lower-back pain

WHO states that treatment depends on whether pain is specific or nonspecific and identifies rehabilitation as a major beneficial intervention. A plan may combine education, graded exercise, activity modification and treatment of an identified nerve, joint or systemic problem. The appropriate approach follows the examination rather than the pain location alone.

A practical decision guide

  1. Point to the center of pain. At the lowest spinal tip favors coccyx; higher across the waist favors lumbar pain.
  2. Identify the strongest trigger. Sitting and bowel movements favor coccyx; lifting, bending or radiating activity-related pain favors lumbar involvement.
  3. Check for nerve or systemic signs. Leg weakness or altered sensation, fever, weight loss, major trauma, or bladder and bowel changes move the decision toward prompt medical care.
  4. Choose low-risk relief. Reduce pressure for a suspected coccyx problem; keep gently active and modify loads for a typical mechanical back episode.
  5. Set a review point. If symptoms worsen, limit movement, persist or develop any warning sign, arrange an examination rather than relying on a cushion or self-diagnosis.

No authoritative source provides a single home maneuver that reliably separates coccyx from lumbar pain, and no precise percentage is established for how often each individual tailbone cause occurs. Location, triggers, neurologic findings and a clinician’s examination are more reliable together than any one symptom.
