The Super Mario Bros. 2 most players in North America remember was not the Japanese Super Mario Bros. 2. Nintendo adapted a separate Japanese game, Yume Kōjō: Doki Doki Panic, for the Western release; Japan’s own sequel later reached Western audiences as The Lost Levels.
Why were there two different games called Super Mario Bros. 2?
After the first Super Mario Bros., Nintendo made a direct sequel for Japan. That game kept the familiar side-scrolling style but was substantially more difficult. When Nintendo prepared a sequel for Western release, it instead adapted Yume Kōjō: Doki Doki Panic, replacing that game’s characters with Mario’s cast.
As a result, the name depended on where and when you encountered the games:

So “swapped in” describes Nintendo’s regional release choice: the Western sequel was an adaptation of another game, not Japan’s original sequel under a different name. Nintendo’s Iwata Asks archive identifies Doki Doki Panic and its later Japanese release as Super Mario Bros. USA.

What was Super Mario Bros. 2 originally?
The Western game began as Yume Kōjō: Doki Doki Panic, a Fuji Television tie-in connected to Yume Kōjō (“Dream Factory”). Nintendo made it for Japan’s Famicom Disk System, and it was released in July 1987. Nintendo’s archive identifies the game, platform and release month; WIRED’s interview-based account of its development reports that Nintendo was asked to build a game around characters associated with the event.
How did the game’s unusual mechanics take shape?
Director Kensuke Tanabe recalled an early prototype built around two players climbing vertically by stacking blocks or throwing one another upward. In WIRED’s 2011 report, Tanabe said “the vertical-scrolling gimmick wasn’t enough to get us interesting gameplay.” Shigeru Miyamoto encouraged the team to move toward a more conventional side-scrolling game.

Tanabe connected actions from that prototype—lifting objects and tossing characters—to the finished game’s emphasis on picking up and throwing enemies and objects. This is Tanabe’s recollection as reported by WIRED, rather than a claim based on a surviving design document. WIRED’s account also quotes Tanabe explaining the character change: “We knew these Fuji TV characters wouldn’t be popular in America, but what would be attractive in America would be the Mario characters.”
Why does the Western game feel so different?
In the first Super Mario Bros., jumping on enemies is the central way to deal with them. In the Western Super Mario Bros. 2, lifting and throwing enemies and objects is central instead. The game also lets players choose among characters whose different handling affects play:

Those character traits were part of the original cast and remained in the Mario adaptation, according to Nintendo Life’s retrospective. Together, the throwing-focused action and selectable characters make the game less like a straightforward continuation of the first game, while giving it a distinct identity in the Mario series.
When did the adapted game come to Japan?
Nintendo released the adapted game in Japan as Super Mario Bros. USA in September 1992, according to Nintendo’s Iwata Asks archive. The name makes the regional history visible: this was the game Western players had received as Super Mario Bros. 2, now arriving in Japan under a title that marked its American connection.

What happened to Japan’s original sequel?
Japan’s direct sequel was later released outside Japan as Super Mario Bros.: The Lost Levels. It is the game to look for if you mean the challenging Japanese follow-up—not the Western Super Mario Bros. 2 adapted from Doki Doki Panic.
