Stem cell therapy vs. gene therapy: how the approaches differ

Stem-cell therapy uses cells as the treatment; gene therapy changes genetic material, gene expression, or the biological properties of living cells. The distinction matters because the two approaches involve different products, manufacturing steps, eligibility rules, evidence, and risks. They are not always separate: an ex vivo gene therapy can modify a patient’s stem or other cells in a laboratory and return them to the body.

Neither label identifies the best treatment by itself. A meaningful comparison requires the exact disease, product, indication, delivery method, regulatory status, clinical evidence, eligibility criteria, risks, and follow-up plan. The discussion below uses the United States regulatory framework, where the relevant regulator is the Food and Drug Administration (FDA).

What is the difference between stem cell therapy and gene therapy?

“Stem-cell therapy” is commonly used for treatments in which living cells are administered or otherwise used as the therapeutic product. It is a broad, sometimes imprecise phrase. Cellular therapy can involve hematopoietic stem cells, adult or embryonic stem cells, immune cells, cancer vaccines, and other autologous (the patient’s own) or allogeneic (donor-derived) cells.

Gene therapy is defined by what is changed rather than simply by the presence of cells. FDA describes human gene therapy as seeking to “modify or manipulate the expression of a gene or to alter the biological properties of living cells for therapeutic use.” A product may replace a disease-causing gene, inactivate a problematic gene, or add a new or modified gene that gives cells a therapeutic function.

Comparison Stem-cell or cellular therapy Gene therapy
Therapeutic material Cells, or a cell-based product, are administered or used as the intervention. Genetic material or gene expression is changed, or living cells are given altered biological properties.
Cell source Cells may come from the patient or a donor; the source, processing and matching requirements depend on the product. Gene therapy can use a vector or other delivery system in the body, or cells modified outside the body.
Where modification occurs The cells may be selected, expanded, differentiated or otherwise processed without genetic modification. Modification may occur in vivo (inside the patient) or ex vivo (outside the patient before reinfusion).
Typical biological idea The administered cells or their products carry out the intended repair, replacement, immune activity or support. Changed genetic instructions alter how target cells function, survive, produce a protein or attack disease.
Regulatory question Is this exact cellular product approved for this indication, and is its collection, handling and administration controlled? Is this exact gene-therapy product approved for this indication, and are its delivery, genetic effects and long-term monitoring appropriate?

The categories therefore describe different therapeutic ideas, not mutually exclusive product shelves. A genetically modified cell product can be both a cellular therapy and a gene therapy.

Can gene therapy use stem cells?

Yes. In an ex vivo approach, clinicians collect cells from a patient, send them to a controlled laboratory process, introduce or edit genetic material, test the resulting product, and return the modified cells. The cells can be stem cells or another relevant cell type. The genetic change is the defining feature of the gene therapy; the returned cells are the delivery vehicle and therapeutic material.

  1. Collection: Cells are obtained from the patient or a donor using the method specified for that product.
  2. Manufacturing: A laboratory introduces, removes, replaces or regulates genetic material. The process may use a viral vector or another approved technology, depending on the product.
  3. Quality testing: The manufactured cells are checked for identity, potency, viability, contamination and other product-specific release criteria.
  4. Patient preparation: Some treatments require conditioning or other preparation before the cells are infused.
  5. Administration and monitoring: The modified cells are returned to the patient and followed for immediate reactions, treatment effect and delayed effects.

This workflow is different from an in vivo gene therapy, in which a delivery system introduces genetic material directly into the patient’s body. It is also different from a conventional cellular therapy in which cells are administered without being genetically modified for the treatment.

How do the two approaches work?

How cellular therapy may work

A cell-based product can be intended to replace depleted blood-forming cells, provide immune activity, release biologically active substances, support damaged tissue, or perform another disease-specific function. The exact mechanism depends on the cell type, source, preparation, dose, route and indication. “Stem cells” does not imply a single mechanism: blood-forming stem cells, mesenchymal cells, embryonic stem-cell-derived products and immune-cell products behave differently.

Autologous products use a patient’s own cells and may reduce some donor-matching issues, but collection, manufacturing time and the patient’s underlying disease can affect feasibility. Allogeneic products come from another person and can provide an available cell source, while introducing compatibility, immune-reaction and donor-screening considerations. Those trade-offs must be assessed for the named product rather than inferred from the word “stem.”

How gene therapy may work

FDA describes three broad genetic strategies: replacing a disease-causing gene, inactivating a gene that is causing harm, or introducing a new or modified gene. A therapy may aim to restore production of a missing protein, alter a cellular pathway, make immune cells recognize cancer, or correct another molecular defect. The desired effect can be temporary or persistent depending on the construct, target cells and disease.

Gene delivery does not automatically mean permanent correction. The duration and distribution of expression, immune responses to the delivery system, and the ability of treated cells to persist are product-specific questions. A product’s name or technology class cannot answer them without clinical and regulatory evidence for the particular indication.

Is stem cell therapy FDA-approved?

In the United States, approval applies to a specific product for specific uses. FDA’s consumer information describes approved stem-cell products consisting of blood-forming hematopoietic progenitor cells derived from umbilical cord blood. Those products are approved for disorders that affect blood production, not for every condition promoted in advertisements using the phrase “stem-cell therapy.”

FDA also maintains a list of licensed cellular and gene-therapy products. That list can change, so a patient should check the current entry for the exact product, manufacturer and indication. An approval for one disease, cell source or route does not authorize use for an unrelated disease. Use outside the approved indication may involve a different evidence and oversight situation and should be discussed explicitly with a qualified clinician.

A clinic’s claim that it is registered with FDA, or that a product appears in a clinical-trial registry, is not proof that the product is legally marketed or effective. Registration and investigation are not the same as marketing approval. Ask for the product’s formal name, approval status, indication and the regulator’s documentation rather than relying on a general claim that it is “FDA compliant.”

How does FDA oversight differ for gene therapy?

In the United States, a gene-therapy trial must have an Investigational New Drug (IND) authorization before it starts. The IND process addresses the proposed study, manufacturing, nonclinical information, clinical protocol and participant protections. An investigational product can be scientifically promising without being approved for routine treatment.

Marketing a gene therapy requires an approved Biologics License Application (BLA). Approval is tied to the product, manufacturing controls, proposed use, evidence and labeling reviewed by FDA. A BLA is not a blanket endorsement of every use of the underlying editing method, vector or cell type.

Because gene therapies can have effects that persist, follow-up may extend well beyond the initial administration. The appropriate duration and tests depend on the product, target cells and known or plausible delayed effects. The treating team should explain the monitoring schedule before treatment begins.

Is gene therapy better than stem cell therapy?

There is no universal winner and no general head-to-head ranking established by the FDA pages described here. “Better” only has meaning for a defined disease, patient and approved or properly overseen product. A therapy that addresses a known genetic cause may be inappropriate for a disease without that target; a cell product may be unsuitable if the patient cannot safely undergo collection, conditioning or infusion.

Decision axis Questions to ask about a cellular therapy Questions to ask about a gene therapy
Diagnosis and indication Is this exact cell product studied or approved for my diagnosis? Does the genetic target and indication match my diagnosis?
Mechanism What are the administered cells expected to do, and how long should they persist? Which gene or expression pathway is changed, in which cells, and for how long?
Delivery Are cells infused, injected or implanted, and is conditioning required? Is the treatment delivered in vivo or through modified cells returned ex vivo?
Evidence What outcomes were measured in participants with the same disease and stage? What clinical evidence shows benefit, durability and safety for this construct and delivery system?
Eligibility Are age, disease status, organ function, donor matching or prior treatment restrictions relevant? Are genetic testing, immune status, organ function, conditioning or prior therapies required?
Risk and follow-up What are infusion, immune, infection, graft or manufacturing risks, and what monitoring is required? What are delivery-system, immune, off-target, persistence and delayed-effect risks, and how long is follow-up?

The comparison should end with an individualized recommendation from the clinical team, not with a verdict based on the treatment label.

What are the risks of stem cell therapy or gene therapy?

Every approved product has a product-specific risk profile. Risks can arise from the cells themselves, collection, preparation, conditioning, infusion or injection, immune reactions, interactions with other treatments and the underlying disease. Gene therapies add questions about delivery systems, immune responses to the vector or edited cells, the location and duration of genetic effects, and the possibility of delayed effects.

FDA has warned that unapproved regenerative-medicine products have been associated with serious reported harms, including blindness, tumor formation, neurological events, bacterial infections, reactions at collection or administration sites, unwanted immune or inflammatory responses, unintended tissue growth and contamination. These warnings concern unapproved products; they are not a complete risk profile for every approved cellular or gene therapy.

Be especially cautious when a provider promises a cure for many unrelated diseases, describes a product as “natural” or risk-free, refuses to identify the manufacturer and indication, or treats a trial listing as proof of effectiveness. A legitimate clinical discussion should include alternatives, expected benefits, uncertainties, adverse-event plans and who will provide follow-up.

How do delivery, manufacturing and practical requirements differ?

Practical issue Cellular therapy Gene therapy
Starting material Patient or donor cells, with collection and compatibility requirements determined by the product. A genetic construct, delivery system or cells selected for modification; ex vivo products also require cell collection.
Manufacturing sensitivity Cell identity, viability, sterility, potency and handling time are critical. Those cell controls may apply to ex vivo products, along with genetic construct identity, vector characteristics, editing or expression performance and additional release tests.
Timing Donor availability, collection and processing can affect scheduling. In vivo treatment may avoid patient-specific manufacturing, while ex vivo treatment can require a manufacturing interval before reinfusion.
Administration setting Often requires a specialized center capable of infusion, transplantation or cell handling. Requires a center equipped for the specific vector, infusion or modified-cell process and its emergency monitoring.
Follow-up Monitoring focuses on engraftment or cell activity, immune effects, infection and disease response as appropriate. Monitoring may include gene-expression or cell-persistence effects, immune reactions, disease response and delayed safety concerns.

These are general workflow distinctions, not guarantees. The approved product label and clinical protocol control what actually happens.

How should you evaluate a proposed treatment?

  1. Name the disease precisely. Confirm the diagnosis, subtype, stage and relevant genetic findings. A treatment approved for one subtype may not apply to another.
  2. Identify the exact product. Request the formal product name, cell source or genetic construct, manufacturer, dose, route and whether it is autologous or allogeneic.
  3. Check regulatory status. Determine whether it is FDA-approved for your indication, being offered under an authorized clinical trial, or provided outside those pathways. Do not treat registration as approval.
  4. Understand the mechanism. Ask which cells are affected, what genetic change or cell function is intended, and whether the intervention is in vivo or ex vivo.
  5. Review evidence that matches you. Look for participants with the same diagnosis, disease severity and prior treatment history. Ask which outcomes were measured, how long participants were followed and what uncertainties remain.
  6. List eligibility and preparation requirements. Discuss organ-function tests, immune status, genetic testing, donor matching, conditioning, hospitalization and medication restrictions.
  7. Map risks and alternatives. Ask about common and serious adverse effects, emergency care, standard treatments, other trials and the consequences of declining or delaying treatment.
  8. Plan monitoring. Get a written schedule for laboratory tests, imaging, immune monitoring, disease-response assessments and long-term visits. Clarify which team handles complications after you leave the treatment center.
  9. Verify costs and logistics separately. Confirm collection, manufacturing, travel, hospitalization, caregiver, fertility, insurance and follow-up arrangements with the treatment center and insurer.

What claims should you question?

  • “All stem cells work the same way.” Cell type, source, preparation and indication change the biology and evidence.
  • “Gene therapy always changes every cell permanently.” Targeting, delivery, persistence and duration vary by product.
  • “A trial listing proves the treatment works.” A registered or authorized study shows that an investigation is being conducted under specified oversight; it does not establish effectiveness.
  • “FDA registration means FDA approval.” Registration, an IND for research and a BLA for marketing are different regulatory concepts.
  • “One approval covers every use.” Approval is tied to the product and indication reviewed by FDA.
  • “No reported side effects means no risk.” Rare, delayed or product-specific events may require longer follow-up and careful reporting.

Questions to ask your clinician

  • What is the exact product and is it approved for my diagnosis and location?
  • Are my own cells used, or are donor cells used? If donor cells are involved, how are compatibility and screening handled?
  • For gene therapy, what gene or expression pathway is changed, and is the treatment in vivo or ex vivo?
  • What evidence comes from patients most like me, and what outcomes were measured?
  • What preparation, hospitalization, conditioning or recovery period should I expect?
  • Which short-term and delayed complications require urgent contact?
  • How long will monitoring continue, and who pays for or coordinates that follow-up?
  • What established treatments and properly overseen trials are reasonable alternatives?

Bottom line

Stem-cell or cellular therapy uses cells as the therapeutic product; gene therapy changes genetic material, gene expression or cell properties. An ex vivo gene therapy can therefore use genetically modified stem cells or other cells, making the categories overlap. In the United States, approval and legality depend on the exact product and indication, not on broad labels or promotional claims. Compare a proposed treatment by diagnosis, mechanism, delivery, evidence, eligibility, risks and monitoring, and review those details with a qualified clinical team.
