Choose SQL Server on Azure Local when data must remain in your own environment, disconnected operation is required, or you need direct control of the SQL Server virtual machines. Choose Azure SQL Managed Instance when the workload is compatible and you want to run SQL Server in Azure while Microsoft manages much of the database platform. The right choice depends on workload compatibility, networking, recovery needs, licensing, and the cost of operating each option.
How the two options differ
The main difference is who runs the database infrastructure and where it runs—not simply “cloud versus on-premises.”
SQL Server on Azure Local
SQL Server runs inside Windows Server or Linux virtual machines on infrastructure in your organization’s environment. Your organization manages the SQL Server instances and their VM environment, and must plan how to patch, back up, make highly available, and recover them.
Azure Local supports connected and disconnected deployment modes. In connected mode, supported Azure Arc capabilities can provide centralized inventory, governance, monitoring, security, and licensing. In disconnected operations, workloads and the control plane run in the local environment without an ongoing dependency on the public-cloud control plane. The SQL Server extension for Azure Arc is not supported in disconnected mode.

Azure SQL Managed Instance
Managed Instance is a database service that runs in Azure and supports deployment into a virtual network. Microsoft operates platform tasks such as patching, backups, upgrades, and built-in availability. The service has General Purpose and Business Critical tiers with different performance and availability characteristics.
Microsoft positions Managed Instance as a migration destination for SQL Server workloads that need a broad set of instance-level capabilities. That makes it a candidate for some applications that are difficult to move to a database service with a narrower feature set; it does not establish that every SQL Server feature or behavior is identical.

Check compatibility before choosing Managed Instance
Do not base a migration decision on a broad compatibility claim alone. Review the specific engine features and instance-level dependencies the application uses, then confirm that they are supported for the intended Managed Instance configuration. Microsoft’s migration guidance highlights items such as database placement, logins, credentials, SQL Agent jobs and operators, and server-level triggers.
Compatibility is only one part of migration readiness. Confirm the target virtual-network design and plan how to move the databases and associated instance-level objects. The migration method and acceptable downtime depend on the workload; the available evidence does not establish one migration path or downtime figure that applies to every deployment.

Compare operational responsibility and recovery
On Azure Local, resilience is part of your design
Because SQL Server runs in customer-managed VMs, your team must define and test the database and infrastructure arrangements for availability, backup, and disaster recovery. Decide what failure scenarios you must tolerate, what recovery time and data-loss limits are acceptable, and how you will restore service if a local component or site is unavailable.
Managed Instance reduces platform work, but recovery still needs planning
Managed Instance includes platform-managed maintenance and availability architecture. Microsoft documents availability options that include zone-redundancy choices, but the design and applicable behavior depend on the service configuration and region. Select a tier and availability design that suit the workload, and validate the current regional options and service terms rather than assuming all deployments have the same protection.

Microsoft’s SQL Server-to-Managed Instance migration overview states an availability figure of 99.99 percent; the source page does not state a year for that statement. Treat it as a figure to verify against the current service-level agreement, selected region, and configuration—not as a universal commitment for every deployment.

There is no workload-independent cost winner established for these options. Azure Local costs can include infrastructure acquisition and lifecycle, facilities, operations, support, and SQL Server licensing. Managed Instance costs depend on compute, storage, license choice, tier, region, and workload utilization. Migration, networking, and support can also affect the total.

Microsoft documents multiple SQL Server licensing options through Azure Arc, including virtual-core licensing. Verify the current license terms and whether Azure Hybrid Benefit or subscription eligibility applies to your organization’s agreement. Compare the same workload, capacity, availability, support, and operating assumptions on both sides; a hardware quote alone is not comparable to a Managed Instance list price.

A practical decision process
- Write down non-negotiables. Identify whether local data placement, disconnected operation, direct VM control, or reduced platform administration is a requirement or merely a preference.
- Inventory the SQL Server workload. Record databases, instance-level objects, application dependencies, and required engine features. For a Managed Instance path, check each against current support and migration guidance.
- Design connectivity and recovery. For Azure Local, validate local capacity and plan SQL Server availability, backups, and disaster recovery. For Managed Instance, validate virtual-network connectivity, region, tier, and the required recovery design.
- Build a workload-specific cost model. Include infrastructure or service consumption, licensing, operations, migration, networking, and support, using realistic utilization and availability assumptions.
- Validate current product terms before committing. Capabilities, regions, service-level terms, and licensing guidance can change. Microsoft’s Azure Local overview was updated September 29, 2026; recheck current documentation and licensing terms for the intended deployment.
