Some Antibiotic Transformation Products Can Drive Resistance Like Their Parent Drugs

Yes—some tested products formed when antibiotics break down selected for antimicrobial-resistance-associated change at levels comparable to, or higher than, the original drugs. A 2026 study found this in wastewater-derived microbial communities, but it does not show that every antibiotic breakdown product behaves this way or quantify effects on human health.

What does the study show?

Lakhey and colleagues compared antibiotic transformation products—chemicals formed as antibiotics are changed—with their parent compounds. Their study, published in Nature Water on 8 June 2026, used growth-based assays and seven-day evolution experiments with complex microbial communities derived from wastewater. The work covered fluoroquinolones, sulfonamides, and macrolide–lincosamide–streptogramin antibiotics. Chemistry World reports that the team tested 15 transformation products and collected wastewater samples from treatment plants in Brisbane, Australia, and Falmouth, UK. Chemistry World’s report describes the study and its context.

How did transformation products compare with their parent antibiotics?

The products did not all behave alike. The study compared them on two measures: growth inhibition and changes in the abundance of intI1, a gene used as an indicator of antimicrobial-resistance-associated change in the bacterial communities.

Raw Milk Antibiotic Residue Test Kit – β-Lactam Screening, 5-Minute Results, Test Kit for Dairy Quality Control 12Tests/Kit
  • Fast 5–10 Minute Results: Visual interpretation for rapid decision-making in dairy operations.
  • High Sensitivity: Detects critical β-lactam antibiotics at or below industry safety thresholds.
  • Multi-Analyte Coverage: Targets penicillin, cephalosporins , and other β-lactams.
  • Food Safety Use Only: Intended exclusively for raw milk screening—not for medical or human diagnostic purposes.
  • High Concordance (98.6%) with ELISA: Trusted accuracy for professional testing environments.
  • Growth inhibition: For several products, the lowest observed effect concentrations were only twofold higher than those of their parent compounds. Moxifloxacin sulfate and descladinose roxithromycin inhibited growth more strongly than their respective parent antibiotics.
  • intI1 enrichment: In the seven-day experiments, desmethyl ofloxacin, N-acetyl sulfamethoxazole, and desmethyl erythromycin enriched intI1 at levels comparable to or above those of their parent drugs.

These results support a specific conclusion: some tested transformation products retained biological activity and exerted selective pressure in the experimental communities. They do not mean all metabolites have the same effect. In this study, the measured outcome was enrichment of intI1; it was not a count of clinical infections or a measure of patient outcomes.

Does “metabolised” mean the products come from the human body?

Not necessarily. The study concerns transformation products: chemicals formed when antibiotics are changed. Environmental transformation can occur through biological or other processes, so the term does not imply that every product was made inside a person. “Metabolite” is often used more narrowly for a product of biological metabolism; the headline’s broader environmental subject is better described by the paper’s term, “transformation products.”

What the findings do—and do not—establish

The experiments show that certain transformation products can select for an AMR-associated genetic indicator in wastewater-derived communities under the conditions studied. They do not establish how much these compounds contribute to resistance in people or waterways, what health burden might result, or whether the same effects occur across other antibiotics and wastewater systems.

The findings also do not demonstrate that a particular wastewater treatment process removes the selective effect. The authors argue that transformation products should be included alongside parent antibiotics in antimicrobial-resistance surveillance, treatment evaluation, and environmental risk assessment.

Why wastewater monitoring matters

Monitoring only the original antibiotic may miss biologically active products formed as it changes. The study therefore points to a need to evaluate both parent compounds and transformation products, while testing their effects in relevant microbial communities. That is an environmental monitoring and treatment-evaluation issue; the results do not support a consumer remedy such as choosing a home water filter or buying a water test kit.

Publication correction

Nature Water published a correction on 30 June 2026 to fix co-author William H. Gaze’s name, which had been rendered as “Willam H. Gaze.” The correction notice reports a name-spelling change, not a change to the study’s results. Read the correction notice.
