Nicotine is not the main source of smoking’s deadly toxic exposure, but it is not harmless. Burning tobacco produces thousands of chemicals that drive much of smoking’s cancer and lung-disease burden; nicotine is highly addictive and poses health risks, particularly during pregnancy and adolescence. In January 2025, the U.S. Food and Drug Administration proposed limiting nicotine in cigarettes and certain other combusted tobacco products—not banning nicotine in every product.
What makes smoking so harmful?
The central distinction is between nicotine and the smoke created when tobacco burns. Cigarette smoke contains more than 7,000 chemicals, and more than 70 have been linked to cancer, according to the FDA’s current Chemicals in Tobacco Products and Your Health page. Burning tobacco exposes users to a toxic mixture that causes much of smoking’s severe disease burden, including cancer and chronic obstructive pulmonary disease (COPD).
The FDA puts it this way: “This toxic mix of chemicals—not nicotine—cause the serious health effects among those who use tobacco products, including fatal lung diseases, like chronic obstructive pulmonary disease (COPD) and cancer.” That distinction does not mean nicotine has no health effects. It means nicotine’s best-known role is driving dependence, while combustion produces many of the toxins responsible for smoking-related disease.
The scale of the harm is substantial: the World Health Organization says tobacco causes more than 7 million deaths per year, including over 1.6 million deaths among nonsmokers exposed to secondhand smoke. Those are global figures, not an estimate of deaths caused by nicotine alone. (WHO, Tobacco and nicotine, 26 June 2026.)

Is nicotine harmless?
No. Nicotine is addictive, can make tobacco products difficult to stop using, and carries health concerns of its own. WHO identifies cardiovascular risk; exposure during pregnancy can harm fetal development, and nicotine can harm the developing adolescent brain. The risks depend on who is exposed and how nicotine is delivered, but “not the main cause of smoking’s cancer burden” is not the same as “safe.”
Nicotine also sustains continued tobacco use. Dependence can make quitting difficult even when someone understands the risks of smoking. That is one reason nicotine is present in cessation medicines as well as in tobacco products: the purpose and context of use matter.

How do tobacco and nicotine products compare?
Risk depends on the product and on the person using it. The FDA says combusted, or smoked, tobacco products such as cigarettes are the most harmful type. Some noncombusted products may expose adult smokers who switch completely to fewer harmful chemicals than cigarettes do, but that does not make them risk-free or suitable for people who do not use tobacco.

These comparisons are not a general endorsement of alternative nicotine products. For a person who does not smoke, starting nicotine adds avoidable risks. For an adult who smokes, completely stopping cigarettes is the key distinction in the FDA’s comparison; combining products while continuing to smoke may preserve much of the danger.

In January 2025, the FDA proposed a product standard that would limit nicotine in cigarettes and certain other combusted tobacco products to minimally or nonaddictive levels. This is a proposed limit on nicotine levels in specified products, not a proposal to ban nicotine across the board, prohibit smoking outright, or outlaw nicotine replacement medicines.

The FDA announcement explicitly excluded e-cigarettes, nicotine pouches, noncombusted cigarettes (including heated tobacco products that meet the definition of a cigarette), waterpipe tobacco, smokeless tobacco products, and premium cigars. The agency said public comments would inform its consideration of future action. The FDA’s Tobacco Product Standards page describes its standards authority and lists the 2025 proposal. These sources establish a proposal; they do not establish that it has been finalized.
This is a U.S. federal policy example. It should not be generalized into a claim that governments everywhere are banning nicotine. The policy distinction is between reducing nicotine in certain combustible products and prohibiting nicotine in all products or medicines.

What can someone do if they want to quit smoking?
Quitting all tobacco is the best health outcome. Adults trying to stop smoking can consider FDA-approved cessation medicines, including nicotine patches, gum, or lozenges, alongside behavioral counseling. These medicines are designed to support quitting; they are different from recreational tobacco or nicotine products. A clinician or pharmacist can help identify an appropriate option, especially during pregnancy or when health conditions or other medicines may affect the choice.
