Smartwatch Anxiety: Why Health Alerts Can Worry You—and What Helps

A surprising heart-rate reading or rhythm alert can send you back to the watch for another check. Rechecking may make you more alert to each heartbeat or fluctuation, which can make the next number feel more alarming. That experience is real for some people, but it does not mean the watch caused an anxiety condition—or that a reading diagnoses one.

Why smartwatch readings can become a source of worry

Health data can feel urgent because it turns a bodily signal into a number or notification that is easy to revisit. If a reading surprises you, checking again or seeking reassurance may briefly feel helpful; repeated checking can also keep the concern prominent. The NHS lists repeated checking and reassurance seeking among possible features of health anxiety, and recommends noticing these habits rather than treating every check as proof that something is wrong.

The relationship can run in either direction. Anxiety can involve a faster or more noticeable heartbeat, so a heart-rate reading may become part of a worry loop. That does not mean a higher number proves either illness or anxiety.

What a watch reading can—and cannot—tell you

A smartwatch records signals; it does not explain their cause. Heart rate and activity can vary for many reasons, and an alert is not the same as a clinical assessment. The U.S. Government Accountability Office says wearables may help clinicians, while noting that reliability differs by device and use and that their ability to make clinical decisions autonomously remains unclear (GAO, 2026).

A 2026 systematic review and meta-analysis found limited associations between anxiety symptoms and higher heart rate or lower physical activity. Its meta-analyses found no association between sleep metrics and anxiety. The authors suggest these measures may be more useful alongside self-reports and clinical data than as standalone screening; these findings do not establish that a watch causes anxiety or explain an individual’s reading (Communications Medicine, 2026).

A separate review pooled accuracy across 26 studies of wearable anxiety assessment. It reported 81.94% for approaches combining multiple signals and 76.85% for single-signal approaches; ECG as an individual signal had pooled accuracy of 80.34% across 12 studies. These are research-synthesis results, not real-world accuracy guarantees for consumer watches or a way to diagnose anxiety. The authors noted small samples and methodological variation (Communications Medicine, 2026).

Try a small, reversible change to optional checking

If you use the watch for general wellness and checking is making you more worried, you can experiment with changing how often you look at the data or receive nonessential prompts. This is a practical coping experiment, not a smartwatch setting proven to treat anxiety.

  1. Notice the pattern. For a short period, record when you check, what prompted it, and how you felt before and after. The NHS suggests tracking checking, reassurance seeking, and health-information searches as part of its guidance on health anxiety.
  2. Reduce one optional check gradually. Choose a repeated check or nonessential notification to reduce, then see whether it changes your worry or disrupts your day. NHS guidance says: “Try to gradually reduce how often you do these things over a week.” (NHS: Health anxiety)
  3. Keep medically advised monitoring in place. If you have a diagnosed condition or a clinician-directed monitoring plan, ask your clinician how to interpret the readings and whether any settings should change. Do not dismiss symptoms or ignore an alert because a wellness reading seems reassuring.

When to ask for support

Speak with a GP or qualified health professional if health worries are preventing normal life or self-help is not working. NHS guidance says a clinician may offer cognitive behavioural therapy (CBT) after assessing health anxiety. If you are struggling to cope with anxiety, seek help rather than trying to diagnose yourself from a watch (NHS: Health anxiety; NHS: Generalised anxiety disorder).

People with atrial fibrillation deserve particular care in interpreting alerts: an HHS grant record describes preliminary, cross-sectional findings linking smartwatch use with higher cardiac anxiety and more AF-specific care, especially after heart-rate or rhythm alerts. The record notes possible selection bias and proposes a randomized trial to test causality. It is not proof that alerts cause anxiety or unnecessary care, and it may not apply beyond the group studied (HHS grant record).
