The findings do not show that smartphones caused those symptoms, or that 12 is a risk threshold. The study examined ownership—not phone hours, particular apps, or the content young people saw.
What did the study find?
Nagata and colleagues analyzed data from the Adolescent Brain Cognitive Development (ABCD) Study, a U.S. cohort recruited through 21 study sites. Of the 8,957 adolescents in the analysis, 6,225 (69.5%) owned a smartphone at age 12. The results were published in JAMA Network Open on September 25, 2026.

At age 12, smartphone ownership was associated with higher prevalence of binge-eating symptoms, self-worth tied to weight, and at least one of the symptoms measured. The adjusted association was not statistically significant for compensatory behaviors or worry about weight gain.

For outcomes at age 14, ownership at 12 was associated with higher risk ratios for four measured outcomes. Worry about weight gain was not clearly associated. The study’s adjusted estimates were:

These are adjusted group-level associations, not individual predictions. For descriptive context, 24.2% of adolescents who owned a smartphone at 12 reported at least one measured symptom at 14, compared with 15.5% of those who did not own one at 12. That difference is not evidence that phone ownership caused the symptoms.
What did researchers measure?
A parent or caregiver reported whether the adolescent owned a smartphone at age 12. Adolescents reported symptoms assessed with the Kiddie Schedule for Affective Disorders and Schizophrenia: binge eating, inappropriate compensatory behaviors intended to prevent weight gain, worry about weight gain, and self-worth tied to weight. Researchers assessed these symptoms at ages 12 and 14.

Reported symptoms are not the same as a clinical diagnosis of an eating disorder. The study used weighted, covariate-adjusted statistical models; its age-14 analysis also accounted for the corresponding symptoms reported at age 12. The analysis therefore asks whether ownership was associated with reported symptoms, not whether receiving a phone produced a specific outcome.

No causal conclusion follows from this study. It was an observational analysis: researchers compared outcomes among adolescents based on reported smartphone ownership, rather than assigning phones or testing a phone-access intervention. Other differences between young people who did and did not own a phone could contribute to the associations.

The study did not measure phone-use duration, social-media use, or exposure to particular content. The authors discuss appearance-focused content, social comparison, and sleep disruption as possible mechanisms, but the study did not establish that any of them explains the findings. As the authors put it, “Future research is needed to characterize the mechanisms underlying these observed associations.”
What are the study’s limitations?
- Ownership is a broad measure. It does not show how often a child used a phone, what they did on it, or what content they encountered.
- Some outcomes were uncommon. Worry about weight gain was reported infrequently, which the authors say may have limited the ability to detect a difference.
- The analyzed sample may not represent everyone equally. Participants excluded from the analysis were more likely to belong to racial minority groups and to have parents with lower income and education.
- The findings do not set a universal first-phone age. The study did not test whether delaying ownership prevents symptoms or compare different phone-access policies.
What can parents take from the findings?
The paper suggests that parents and caregivers may benefit from guidance on age-appropriate limits and supervision of early device access, including family media planning. That is a practical implication—not evidence that a particular rule, device, or delay will prevent eating disorder symptoms.

Families weighing a first phone can consider the child’s readiness and practical needs, what supervision and boundaries are workable, and how access to online content will be managed. Those are decision factors, not interventions tested by this study. If a young person shows concerning changes in eating, weight-related distress, or behavior, a qualified health professional can assess their situation; phone ownership alone cannot diagnose a problem.
