Obstructive sleep apnea (OSA) is associated with cognitive impairment and dementia in some observational studies, but that does not prove it causes Alzheimer’s disease. There is also no established evidence that CPAP prevents Alzheimer’s. If you notice loud snoring, breathing pauses, gasping during sleep, or significant daytime sleepiness, talk with a healthcare provider about assessment and sleep testing. Treating diagnosed OSA can address the sleep disorder; it should not be presented as a proven dementia-prevention treatment.

Can sleep apnea cause Alzheimer’s?

Studies have found an association between sleep-disordered breathing, including OSA, and cognitive impairment or dementia. An association alone cannot show that OSA causes Alzheimer’s, predict an individual’s likelihood of developing it, or establish that treating OSA changes dementia risk.

A 2024 meta-analysis of 15 cohort studies reported a pooled hazard ratio (HR) of 1.52 (95% confidence interval [CI] 1.32–1.74) for cognitive impairment and/or all-cause dementia among people with sleep-disordered breathing or OSA compared with people without it. In the subset of studies that diagnosed participants using polysomnography, the pooled estimate for all-cause cognitive impairment was lower and less precise: HR 1.32 (95% CI 1.00–1.74). The authors noted that many studies did not use objective, polysomnography-based diagnosis. These are pooled associations, not proof of cause or a personal risk estimate (Tian et al., 2024).

A 2025 nationwide Danish matched cohort followed 62,928 people with OSA and 62,928 comparison participants. The cohort was 76% male, with a median age of 52. Its estimates were HR 1.10 (95% CI 0.98–1.24) for all-cause dementia and HR 1.16 (95% CI 0.94–1.43) for Alzheimer’s disease; both confidence intervals included 1.0. The study reported differences among subgroups and weaker associations among people treated with CPAP, but treatment was not randomly assigned. Those findings cannot establish that CPAP prevents dementia.

Results vary across studies and can depend on how OSA and dementia are diagnosed, participants’ ages, and other health factors. The available evidence does not support treating a single relative-risk figure as settled or applying it directly to one person.

Does CPAP prevent Alzheimer’s or slow dementia?

No. CPAP is an established treatment for OSA, but it has not been shown to prevent Alzheimer’s disease. Small trials in people who already had Alzheimer’s examined sleepiness or cognition; they do not establish prevention or prove that CPAP reverses dementia or slows cognitive decline.

In a randomized, placebo-controlled study of 39 community-dwelling older adults with mild-to-moderate probable Alzheimer’s disease and sleep-disordered breathing, therapeutic CPAP was associated with lower subjective daytime sleepiness (Ancoli-Israel et al., 2006). A separate randomized study enrolled 52 people with mild-to-moderate Alzheimer’s disease and OSA. Its initial three-week comparison of therapeutic and placebo CPAP did not find significant cognitive improvement. Later pre/post comparisons after therapeutic CPAP suggested some improvement, but the study was too small to make definitive claims about individual cognitive domains (Cooke et al., 2009).

A preliminary follow-up from Cooke et al. compared only five people who continued CPAP with five who stopped. The authors described possible benefits and called for prospective randomized trials. That small comparison is hypothesis-generating, not proof of long-term protection (Cooke et al., 2009).

What are the signs of sleep apnea?

Common clues to OSA include breathing that repeatedly starts and stops during sleep, frequent loud snoring, and waking up gasping for air. A person may not notice these events; someone who shares the bedroom may be the first to report them.

  • Daytime sleepiness or tiredness, and fatigue
  • Dry mouth or headache
  • Insomnia or waking often to urinate
  • Sexual dysfunction or decreased libido

The National Heart, Lung, and Blood Institute (NHLBI) also notes that fatigue, headache, and insomnia are among symptoms more common in women. Symptoms can prompt a conversation with a clinician, but a symptom list cannot diagnose OSA.

How do doctors test for sleep apnea?

A healthcare provider asks about symptoms, risk factors, and family history. Depending on the assessment, they may refer you to a sleep specialist or sleep center for a sleep study. The study helps determine whether sleep apnea is present and, if so, its type and severity (NHLBI, updated 2025).

  1. Describe what is happening. Share symptoms such as snoring, observed breathing pauses, gasping, or daytime sleepiness. If a bed partner has noticed nighttime events, include that information.
  2. Discuss assessment and referral. A provider reviews your symptoms and relevant history and decides whether specialist evaluation or a sleep study is appropriate.
  3. Follow the instructions for the selected sleep study. The clinician or sleep service explains the test they have chosen and how to complete it. Do not assume that all tests are the same or interpret a consumer device as a diagnosis.
  4. Review the results with the clinician. Use the findings to discuss the type and severity of apnea and whether treatment is appropriate.

A sleep diary can help record sleep duration and quality and daytime sleepiness. A wearable or informal phone app may provide observations, but it should not be treated as a diagnosis of OSA.

What treatments are available for OSA?

The appropriate treatment depends on clinical assessment and the person’s circumstances. NHLBI lists positive airway pressure (PAP), lifestyle measures, oral devices, orofacial therapy, and surgery as treatment approaches. The options below are not interchangeable, and suitability, fitting or prescription, and follow-up should be discussed with a clinician.

Approach What it involves How suitability is determined
PAP, including CPAP CPAP provides steady air pressure to help keep the airway open; other PAP types work differently. A clinician recommends and prescribes an appropriate approach. The device uses a mask or other interface and tubing.
Custom oral appliance A dental device may be an option for some people who do not want CPAP or cannot tolerate it. A dental professional fits the appliance and provides instructions; a clinician helps determine whether it is appropriate.
Lifestyle measures Depending on the patient, a clinician may discuss healthy weight, regular activity and sleep habits, limiting alcohol or caffeine, smoking cessation, or sleeping on one’s side. These measures are selected for the person’s circumstances and should not be assumed to replace prescribed treatment.
Orofacial therapy or surgery These are additional approaches listed by NHLBI. More invasive options are selected clinically; a clinician can explain whether an option fits the individual case.

For people already prescribed CPAP, replacement masks and tubing need to match the prescribed setup. Ask the clinician or equipment supplier about compatible supplies and fit; buying equipment is not a way to diagnose or treat Alzheimer’s.
