Do not wait for pain: neuropathy can hide serious infection. Call your local diabetic-foot service, clinician or urgent advice line the same day; seek emergency help immediately for possible sepsis, a rapidly deteriorating deep wound or threatened circulation.
What an infected diabetic wound can look and feel like
Infection is not confirmed by one appearance, symptom or home device. The 2023 guideline from the International Working Group on the Diabetic Foot and the Infectious Diseases Society of America (IWGDF/IDSA) says clinicians diagnose diabetic foot infection from clinical evidence of local or systemic inflammation. A person at home cannot reliably rule infection in or out from a photograph, odor, temperature reading or a single change around the wound.

Changes that become more pronounced, spread or appear together are more concerning than a small, stable change during healing. Kent Community Health NHS Foundation Trust advises watching for a significant increase in one or more of these signs:
- Redness or another change in skin tone that spreads beyond the wound.
- Increasing warmth around the wound.
- Increasing swelling.
- New or worsening pain or tenderness.
- Thick yellow, green or cloudy fluid.
- An unpleasant wound smell.
- The wound becoming larger instead of gradually healing.
- Fever or feeling generally unwell.
None of these findings should be used in isolation as proof. Conversely, the absence of one sign does not make a diabetic wound safe. Diabetes-related nerve damage can reduce sensation, and poor circulation can make it harder for the foot to fight infection and heal.

Healing changes versus changes that need assessment
A wound can look imperfect while it is healing. Kent Community Health NHS Foundation Trust notes that mild redness, slight swelling and a small amount of light-yellow discharge may occur during normal healing. The key concern is a meaningful change from the wound’s previous appearance or a pattern that is worsening rather than settling.
| Observation | How to interpret it | What to do |
|---|---|---|
| Mild, stable redness at the edge | Can occur during healing, especially if it is not expanding or becoming more painful. | Continue the care plan you were given and arrange professional advice for any new or uncertain wound. |
| Slight swelling that is unchanged | May be part of healing; trend matters more than a single snapshot. | Contact your care team if swelling increases, spreads or is accompanied by other warning signs. |
| A small amount of light-yellow fluid | Can occur in normal healing. Thick, cloudy, yellow or green discharge is more concerning. | Seek prompt clinical assessment if the fluid becomes thicker, increases or is accompanied by odor or deterioration. |
| Redness, warmth, swelling or tenderness that is increasing or spreading | Consistent with local inflammation that requires a clinician’s assessment for infection. | Make same-day contact with a diabetic-foot service, GP or urgent advice line. |
| Wound enlarging, foul-smelling or producing pus-like discharge | Suggests deterioration and possible infection, particularly when several changes occur together. | Arrange urgent assessment; do not wait for the next working day if it is worsening. |
| Fever, marked illness or rapidly worsening appearance | May indicate systemic infection or a severe local problem. | Use emergency services when severe illness, sepsis signs, deep tissue destruction or threatened circulation is present. |
Why a diabetic wound can be infected without pain
Neuropathy can blunt pain, heat and pressure sensation. A serious ulcer may therefore feel no worse even while tissue damage or infection progresses. The American Diabetes Association (ADA) advises that every foot ulcer should be examined by a doctor right away, even when it does not hurt.
Do not use “it is painless” as a reason to monitor a new ulcer at home. Also do not press, probe or repeatedly manipulate the wound to test whether it hurts. Report what you can see, how it has changed and whether sensation is reduced.

When to seek help: same-day contact or emergency care
Prompt, same-day clinical contact
Contact your local diabetic-foot or podiatry service, GP, diabetes team or urgent advice line when you suspect infection or have a new or worsening wound. In the United Kingdom, Kent Community Health NHS Foundation Trust directs patients to NHS 111 when its podiatry service is closed and says not to wait until the next working day if symptoms are getting worse. Outside the UK, use the equivalent service in your country; phone numbers and referral routes differ.
Same-day advice is appropriate for spreading redness, increasing warmth or swelling, new tenderness, thick or cloudy discharge, odor, an enlarging wound, fever or feeling unwell when you are not in immediate danger. Tell the service that you have diabetes and describe the change and its timing so the urgency is clear.
Emergency help now
Call emergency services immediately for signs that may indicate sepsis or a limb-threatening problem. Kent NHS lists possible sepsis warning signs including:

St George’s University Hospitals NHS Foundation Trust directs patients to the closest Emergency Department for a foot wound with systemic infection signs or a rapidly deteriorating deep ulcer or necrosis with purulent discharge. Acute critical limb ischaemia—sudden, severely reduced blood flow—also requires emergency evaluation. A cold, pale or discoloured foot with sudden severe deterioration should not be watched at home.
Why hospital decisions are made by clinicians
The IWGDF/IDSA guideline recommends using a formal infection-severity classification and considering hospitalisation for all severe infections. It also recommends considering admission for selected moderate infections when important coexisting conditions make outpatient care unsafe. Readers should not try to assign themselves a severity grade; the classification requires examination and clinical judgement.

What to do while arranging care
- Make contact promptly. Call the service that manages your diabetes or foot care, your GP or the appropriate urgent-care line. Say that you have diabetes, identify the wound’s location and explain what has changed.
- Use emergency services for emergency symptoms. If you have severe illness, possible sepsis, rapid deep deterioration, necrosis with pus or signs of threatened blood flow, call the emergency number rather than arranging a routine appointment.
- Follow your existing clinician-provided wound plan. If you already have written instructions from a diabetes or wound team, follow those while waiting for advice. Do not improvise a new treatment intended to “test” whether the wound is infected.
- Do not start leftover or someone else’s antibiotics. Antibiotic choice, route and duration depend on the clinical diagnosis, severity, circulation, other illnesses and likely organisms. Taking an unsuitable drug can delay appropriate care and does not establish whether infection is present.
- Have key information ready. Note when the wound started, when the change was first noticed, whether redness or swelling is spreading, what the drainage looks and smells like, your temperature if already measured for general illness, your diabetes medicines and any previous foot infections or circulation problems.
- Do not delay because the wound is painless. Explain any numbness or reduced sensation to the clinician; it changes how the wound must be assessed.
What not to use as a home diagnosis
Foot temperature readings
The IWGDF/IDSA guideline advises against using foot temperature, however it is measured, to diagnose infection. A warmer or cooler reading can have multiple causes, and a normal reading cannot exclude infection. Do not postpone assessment while taking repeated measurements.
Odor alone
An unpleasant smell is a warning sign when it is new or worsening, but odor by itself does not prove infection. Dressings, dead tissue and moisture can affect smell. A clinician must interpret odor with the wound’s appearance and your overall condition.
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Fix the driver behind crashes, sound loss and screen glitches3Repair Windows errors before they cause bigger problemsPhotographs and online checklists
A photograph can miss warmth, tenderness, fluctuance, depth, circulation problems and systemic illness. Online checklists are useful for recognizing that advice is needed, not for clearing a wound as safe. If you are uncertain, treat uncertainty as a reason to contact a professional.
Best Value
Dressings and consumer products
A dressing may be part of a clinician’s plan, but it cannot determine whether infection is present. Do not rely on a dressing, thermometer or other product to decide whether to seek care. The urgent question is clinical assessment, not finding a product that promises to identify infection.
Why diabetes raises the stakes
Diabetes can damage nerves, reducing awareness of injury and pressure. It can also impair circulation, leaving tissue less able to resist infection and heal. These factors mean a small-looking wound can become serious without dramatic pain. Every ulcer deserves medical attention, and a wound that changes direction—becoming larger, redder, hotter, more swollen, more tender or more heavily drained—needs reassessment.
Tell the clinician about known neuropathy, peripheral arterial disease, previous ulcers or amputations, kidney disease, immune-suppressing medicines and any recent antibiotic use. These details can influence the urgency and the safest treatment setting.
Questions a clinician will need to answer
Professional assessment usually focuses on whether local inflammation is present, how deep the wound extends, whether there is dead tissue or pus, how quickly it is changing, and whether infection has affected the whole body. The clinician may also assess circulation and sensation because poor blood flow and neuropathy alter both risk and healing.
The IWGDF/IDSA severity framework helps clinicians decide whether outpatient treatment is reasonable or whether hospital care should be considered. A severe infection, systemic illness, rapidly destructive wound or serious comorbidity can change the plan even when the skin opening looks small.
A practical decision rule
- Stable and minor healing change: mild redness, slight swelling or a small amount of light-yellow fluid that is not increasing can occur during healing, but a diabetic ulcer still merits professional review.
- New, significant or worsening local change: contact a diabetic-foot service, clinician or urgent advice line the same day.
- Systemic illness, rapid deep deterioration, pus with necrosis or threatened circulation: use emergency services immediately.
Because infection can be painless and cannot be diagnosed reliably at home, the safest response to a worsening diabetic foot wound is timely clinical assessment rather than waiting for a clearer sign.
