Only in a limited supporting role. A general-purpose AI chatbot may help you organize your thoughts or prepare questions for a mental health professional, but it is not a therapist, diagnostic service, or crisis responder. Use its answers as suggestions to check—not as the basis for a diagnosis, treatment plan, or major decision.
What a chatbot can—and cannot—do
Chatbots can produce fluent, reassuring replies, but a conversational tone is not evidence of clinical judgment. The American Psychological Association (APA) says there is no consensus that generative AI chatbots and wellness apps have the qualifications needed to provide mental health care, diagnosis, feedback, or even advice in most cases. They may lack important personal context, cannot read nonverbal cues, and can struggle to assess risk or account for cultural differences. The World Health Organization (WHO) has also noted that people are using generative AI for emotional support even though these tools were not designed or tested for mental health care.

That distinction matters: an answer can sound empathetic and still be wrong, miss a serious risk, or reinforce a harmful or delusional belief. Do not interpret confidence, warmth, or a wellness label as proof that a tool understands your situation or is clinically safe.
When it may be useful
Keep the role narrow and supplementary. A chatbot can help you put concerns into words before a visit, brainstorm questions for a qualified professional, or summarize notes you wrote yourself. If you are already working with a clinician, it may help you rehearse an exercise you learned in treatment; ask your clinician whether that use makes sense for you.

| Use | Practical boundary |
|---|---|
| Preparing for an appointment | Use it to organize your concerns or draft questions; check the result with your clinician. |
| Practicing a learned exercise | Use it only to revisit a technique already taught by your clinician, not to create a treatment plan. |
| Seeking a diagnosis or deciding on treatment | Leave this to a qualified professional who can assess your history, context, and needs. |
| Responding to immediate danger or severe symptoms | Seek live human help rather than relying on a chatbot. |
Why extra caution is warranted
APA’s 2026 survey asked more than 1,200 U.S. licensed psychologists involved in patient or client care about chatbots and mental health. Among respondents, 97% felt chatbots may inadvertently reinforce negative behaviors or delusional beliefs, 94% said current chatbots cannot treat conditions with appropriate nuance, and 89% worried chatbots may inadvertently encourage self-harm. Separately, 77% said they had spoken with patients who used AI for support, engagement, or other reasons.
These figures describe psychologists’ reports and concerns, not the percentage of chatbot users harmed. The survey was conducted online in the United States from April 9 to April 26, 2026; it was not a trial measuring treatment outcomes.

Other findings have limits too. APA reports that a 2025 analysis of six popular chatbots found none met clinical standards for responses to simulated mental health crises; APA says the analysis appeared online in 2026. That is a limited test of simulated interactions, not evidence that every chatbot fails every crisis conversation. Stanford’s June 2025 report describes research presented at ACM FAccT 2025 that identified bias and potentially dangerous responses, including in scenarios involving suicidal ideation and delusions. These findings support caution, not a claim that every interaction causes harm.

What to do in a crisis
Do not use a chatbot as your response to suicidal thoughts, urges to self-harm, risk of harming someone else, hallucinations, or strong unusual beliefs. Contact a person who can respond in real time. In the United States, APA advises calling 911, calling or texting 988, or going to the nearest emergency department. If you are elsewhere, contact your local emergency number or crisis service.

Protect your privacy
Do not assume a chatbot conversation is confidential in the way a clinical relationship may be. APA warns that sensitive information may be stored, shared, exposed in a breach, or used for profiling. Data practices vary by service, so check the tool’s current privacy terms before using it; the practices of individual products have not been established here.
- Avoid entering your full name, address, contact details, or other identifying information unless you have a clear reason and understand how the service handles it.
- Share only what is needed for the limited task. For example, ask for help turning a general concern into questions for a clinician rather than pasting an entire private history.
- Do not assume that deleting a conversation removes all stored copies or data; check the service’s terms for its retention and deletion practices.
A safer way to use one for non-urgent support
- Set a limited task. Ask for help organizing your thoughts, summarizing your own notes, or drafting questions for a professional—not for a diagnosis or treatment decision.
- Request evidence-based options. Ask the chatbot to distinguish general information from uncertainty and to offer more than one option. A confident answer is not verification.
- Review before acting. Check any health-related suggestion with a qualified professional, especially if it could affect treatment, safety, or a major life decision.
- Tell your clinician. If you are in care, discuss how you are using AI and bring the chatbot’s output if it would help clarify the conversation.
- Stop and seek a person when risk rises. If the conversation involves immediate danger or severe symptoms, use live crisis or emergency support instead.
What responsible mental-health AI would need
For people assessing a tool intended for mental-health support, a friendly interface is not enough. Relevant questions include whether its clinical purpose and performance have been validated, how it detects and escalates crises, what it does with user data, whether it explains its limitations, who it is suitable for, how well it fits different languages and cultures, and whether a qualified human is accessible.
