Sudden or rapidly worsening vision while taking semaglutide is an emergency. Go immediately to an emergency department or eye-casualty service for specialist ophthalmologic assessment, even if the change is painless, affects only part of one eye, or improves. Semaglutide has been classified by European regulators as associated with the very rare eye condition non-arteritic anterior ischemic optic neuropathy (NAION), but a causal relationship has not been proven. Do not stop or continue treatment on your own: if NAION is confirmed, regulators advise stopping semaglutide, while eye and prescribing specialists should make the final medication decision together.

What NAION is and why sudden vision loss matters

Non-arteritic anterior ischemic optic neuropathy (NAION) is a problem with blood flow to the front portion of the optic nerve, the structure that carries visual information from the eye to the brain. “Non-arteritic” means the problem is not caused by inflammation of the blood vessels. It is sometimes called an “eye stroke,” although that phrase can be confusing: NAION affects the optic nerve rather than the brain.

The typical presentation is abrupt, painless loss or blurring of vision in one eye, often noticed on waking. The change may be a general cloudiness, a missing area in the upper or lower field of vision, or a substantial drop in sharpness. Partial loss still counts as an emergency. An examination may show swelling of the optic disc, where the optic nerve enters the eye.

Vision lost from NAION is often permanent. The World Health Organization has reported that no effective treatment has been identified that restores visual acuity once NAION occurs. That is why the priority is rapid examination and diagnosis, not trying to treat the problem at home.

What regulators and researchers say about semaglutide

Semaglutide is used for type 2 diabetes and, in particular formulations and indications, for weight management. The medicines include Ozempic, Wegovy and Rybelsus. A safety alert from the World Health Organization on 27 June 2025 relayed the European Medicines Agency’s classification of NAION as a “very rare” adverse effect.

After reviewing nonclinical studies, clinical trials, post-marketing reports and published literature, the EMA’s Pharmacovigilance Risk Assessment Committee concluded that NAION may affect up to 1 in 10,000 people taking semaglutide. In adults with type 2 diabetes, the review found an approximately two-fold increase in relative risk, corresponding to about one additional case per 10,000 person-years. A relative increase does not mean that one in every 5,000 users will develop NAION; the underlying incidence is already very low.

Evidence from observational studies is not uniform. A 2026 cohort of U.S. veterans reported 123 NAION cases per 100,000 person-years among semaglutide users versus 67 per 100,000 person-years among users of SGLT2 inhibitors. After overlap weighting, cumulative risk was 0.29% versus 0.13%, with a hazard ratio of 2.33 (95% confidence interval, 1.54–3.54) over a median 2.1 years. The participants were older, predominantly male and medically higher-risk than the general population, so the results cannot be applied directly to every semaglutide user.

A review summarized by the American Academy of Ophthalmology and the North American Neuro-Ophthalmology Society (AAO/NANOS) covered more than 40 studies. Pooled analyses suggested roughly a two-fold relative-risk increase, but the evidence included a 1.2-million-patient Military Health System analysis that found no increased risk. A JAMA Ophthalmology analysis of the OHDSI network found a modest association and called for more work on mechanism and causality. Diabetes, obesity, sleep apnea, hypertension and vascular disease can each raise NAION risk, making observational results difficult to interpret.

Evidence or estimate What it reports How to interpret it
EMA PRAC review Very rare adverse effect; up to 1 in 10,000 users Regulatory safety classification, not proof that semaglutide directly causes every case
EMA estimate in type 2 diabetes Approximately two-fold relative-risk increase; about one additional case per 10,000 person-years Small absolute increase against a low baseline rate
2026 U.S. veteran cohort 123 versus 67 cases per 100,000 person-years; weighted hazard ratio 2.33 (95% CI 1.54–3.54) Observational result from an older, predominantly male, higher-risk group
AAO/NANOS summary Some pooled analyses suggested about two-fold risk; another 1.2-million-person analysis found no increase Studies are mixed and causation remains unestablished
Reported background incidence 2.3–10.2 cases per 100,000 people per year NAION is uncommon even before considering medication exposure

Therefore, it is accurate to say that semaglutide is associated with NAION in some data and is recognized as a very rare side effect by European regulators. It is not accurate to say that semaglutide has been proven to cause NAION in an individual patient.

Symptoms that need immediate assessment

Typical NAION symptoms

  • Sudden loss of vision in one eye, with or without pain.
  • Sudden blurring or cloudiness, even when the loss is incomplete.
  • A new missing area in the upper or lower part of the visual field.
  • Rapid deterioration in eyesight over minutes or hours.

NAION is usually painless and affects one eye at first, but those features do not make it safe to wait. Sudden visual symptoms can have several causes, and only an examination can distinguish them. Do not rely on a home vision test, online symptom checker or a decision that the symptom is “just blurry vision.”

What to do now

  1. Stop what you are doing and arrange emergency care. Attend an eye-casualty service or emergency department immediately. If those services are not available locally, use the fastest emergency pathway in your region.
  2. Tell the receiving clinician exactly when the change began. Note which eye is affected, whether the loss is partial or complete, and whether it is worsening.
  3. List every medicine and condition. Include semaglutide, the formulation and dose, the date of the last dose, diabetes treatment, blood-pressure medicines and any privately obtained prescription. The U.K. Medicines and Healthcare products Regulatory Agency (MHRA) warns that privately obtained medicines may not appear in the usual medical record.
  4. Request specialist ophthalmologic assessment. An eye specialist, and when appropriate a neuro-ophthalmologist, needs to examine the optic nerve and investigate other urgent causes of visual loss.

The WHO said on 27 June 2025 that people with sudden loss of vision or rapidly worsening eyesight during semaglutide treatment should contact a doctor without delay. The MHRA’s 5 February 2026 advice specifically calls for urgent ophthalmological examination when there is sudden loss, including partial loss.

Risk factors for NAION

Semaglutide exposure is only one part of the risk discussion. The following factors are identified by regulators and professional societies:

  • Type 2 diabetes: Diabetes itself increases NAION risk, regardless of which glucose-lowering medicine is used.
  • High blood pressure and high cholesterol: These vascular conditions can impair the blood supply relevant to the optic nerve.
  • Smoking: Tobacco use adds vascular risk.
  • Untreated sleep apnea: Repeated nighttime breathing interruptions and oxygen changes are associated with NAION risk.
  • Overnight low blood pressure: Excessive nighttime blood-pressure lowering can reduce perfusion of the optic nerve in susceptible people.
  • A very small optic-nerve cup: A crowded optic-nerve anatomy can leave less room for swelling.
  • Previous NAION: The fellow eye becomes an important part of the discussion because risk is no longer limited to the eye already affected.

Some factors can be addressed, but that does not mean a person should change treatment without supervision. Discuss smoking cessation, blood-pressure timing, cholesterol management, diabetes control and evaluation for sleep apnea with the clinicians who manage those conditions. No evidence supports automatically stopping semaglutide in every person who has no visual symptoms, and AAO/NANOS do not recommend a special screening eye examination solely to look for NAION risk. People with diabetes should continue their regular diabetic eye examinations.

Should you stop semaglutide if your vision changes?

When NAION is confirmed

EMA and MHRA product-safety advice says semaglutide should be stopped when NAION is confirmed. That recommendation concerns a diagnosed condition, not every episode of nonspecific blur. Confirmation requires urgent clinical assessment because other eye and neurologic emergencies can look similar.

When symptoms are sudden but the diagnosis is not yet known

Seek emergency care first and bring the medication information with you. Do not delay assessment to decide whether to take the next dose, and do not restart, permanently discontinue or substitute treatment without instructions from the prescribing clinician and eye specialist. The immediate question is what is causing the vision loss.

When you have no visual symptoms but are worried

AAO/NANOS recommend discussing concerns with the clinician who prescribes semaglutide or manages your primary care. A shared decision should weigh:

  • How certain the eye diagnosis is, if any diagnosis has been made.
  • The very low absolute NAION rate versus the relative-risk findings.
  • Your diabetes, blood-pressure, cholesterol, smoking and sleep-apnea risks.
  • Your optic-nerve anatomy and any previous NAION.
  • The cardiometabolic benefit you receive from semaglutide.
  • Available alternative treatments and their expected benefits and risks.
  • The implications for the fellow eye if NAION has already occurred.

For some patients, abruptly stopping a medicine that is controlling diabetes or providing important cardiometabolic benefit can be harmful. That is why professional societies do not endorse a blanket stop for every patient with a concern, while regulators do advise stopping after confirmed NAION.

How clinicians assess suspected NAION

The emergency examination is intended to establish whether the optic nerve is affected and to rule out other urgent causes. Tell the team the exact onset and progression, whether one or both eyes are involved, and all relevant medical and medication history. An ophthalmologist may document visual acuity, visual fields, pupil responses and the appearance of the optic disc, then order additional testing when the presentation is atypical or another diagnosis is possible.

Because NAION-related damage is often permanent and no effective vision-restoring treatment has been identified by WHO, early assessment is valuable even when the likely outcome cannot be reversed. It confirms the diagnosis, identifies risks to the other eye and allows the medication decision to be made with the correct information.

Practical prevention and follow-up conversations

Questions to take to the prescribing clinician

  • What benefit is semaglutide providing for my diabetes, weight or cardiovascular risk?
  • Would another treatment provide a reasonable alternative if semaglutide is stopped?
  • Could my blood-pressure medicines be contributing to low overnight pressure?
  • Should I be evaluated for sleep apnea or have existing apnea treatment reviewed?
  • How should diabetes control be maintained if treatment changes?

Questions for the eye specialist

  • Does the examination support NAION, or is another cause more likely?
  • What is the risk to the fellow eye in my situation?
  • Which vascular or sleep-related factors should be addressed first?
  • What symptoms should trigger another emergency visit?

Do not use supplements, home remedies or self-directed dose changes as substitutes for examination. The available evidence does not establish a home treatment that reverses NAION.

Key distinctions to remember

  • Association is not proof of causation: Some studies report higher NAION rates with semaglutide, while others do not, and many risk factors overlap with diabetes and obesity.
  • Relative and absolute risk differ: A two-fold relative increase can still represent a very small absolute risk when the baseline condition is uncommon.
  • Painless does not mean harmless: Sudden, painless or partial loss is still urgent.
  • Diagnosis determines the medication conversation: Confirmed NAION prompts regulator advice to stop semaglutide; uncertainty requires urgent assessment and clinician-guided shared decision-making.
