Can AI preserve the evidence? It can help surface and compare conflicting medical-record claims, but current evidence does not show that AI can reliably decide which version is true. To preserve evidence, a system must first link records to the correct patient, keep each claim tied to its source and context, show disagreements rather than silently resolving them, and leave consequential decisions to qualified clinical review.
What does it mean for AI to preserve evidence?
A generated summary is not the source record. Evidence is preserved only when a reviewer can see what each claim came from, when and in what context it was recorded, and whether another source disagrees.

In practical terms, preservation means the system:
- Retains the original documents or an accessible route back to them.
- Attaches provenance to extracted claims, such as the source, author or system, date, and relevant passage.
- Keeps conflicting statements visible and labels them as unresolved until reviewed.
- Does not treat a newer entry, a more fluent account, or an AI-generated summary as automatically more authoritative.
ISO 13606-1 describes EHR communication in terms of preserving the clinical meaning intended by the author and incorporating provenance metadata. That is a useful design principle, not proof that a particular AI product follows it. The ISO reference page describes a third edition and alignment with FHIR and openEHR, but its listing is a draft; publication status and applicability depend on the formal edition and setting. Patient matching asks whether two records belong to the same person. Reconciliation asks how to represent conflicting claims after that identity has been established. These are separate problems: if records are linked to the wrong person, an AI summary can combine unrelated evidence into a convincing but unsafe account.

The U.S. Office of the National Coordinator for Health Information Technology (ONC) describes patient matching as identifying and linking one person’s data within and across health systems to create a comprehensive record. It notes that matching can use multiple demographic fields, including name, birth date, phone number, and address. ONC guidance on patient identification and matching.
Those fields can be incomplete, inconsistently formatted, or shared by different people. The U.S. Government Accountability Office (GAO) reported in 2019 that providers used manual and automated demographic matching, including software that flagged possible matches for staff review. Stakeholders said inaccurate or inconsistent demographic data made matching harder, and warned that incorrect links could affect patient safety or privacy. GAO interviewed 37 selected stakeholders; it explicitly said the findings were not generalizable. Its findings are not a current national prevalence estimate. GAO-19-197, published January 15, 2019.

GAO also reported on a specific formatting effort: 23 providers in Texas implemented demographic-formatting standards in 2017, and representatives from three hospitals said matching improved, with one hospital reporting less manual review. This was interview-based reporting, not a controlled estimate of nationwide impact. Stakeholders’ conclusion was that no single effort would solve patient record matching.

AI can help locate inconsistencies across documents, structure statements into comparable claims, and bring likely conflicts to a reviewer’s attention. But detecting a discrepancy is different from establishing which account is correct. A system may identify two incompatible entries without having enough context to determine whether one is outdated, mistaken, incomplete, or valid in a different situation.

What benchmark testing shows
HealthContradict, published in npj Digital Medicine on January 21, 2026, tests language models on contradictory biomedical documents. Its 920 expert-verified instances pair a health-related question and evidence-supported answer with two documents taking contradictory positions. In this benchmark, MEDITRON3-8B reached 91.1% accuracy when given correct context, 8.7 percentage points above its no-context setting. With incorrect context, its performance fell 21.6 percentage points from control; conflicting context also reduced performance compared with no context.
These are benchmark-specific results, not clinical accuracy rates. The experiments used selected open-source models up to 8 billion parameters, yes/no questions, and web documents. The authors note that the work did not include clinical risk analysis and had limited human evaluation. It does not establish how an AI system will handle a particular patient’s longitudinal EHR. What an early discrepancy-detection study shows
A 2026 paper by Pugh, Yang, Sutherland, and Breschi evaluated a dual-stream architecture that compares patient narrative with structured FHIR data. Its hybrid dataset covered 26 patients across 675 sessions, combining real provider-patient transcripts with synthetic FHIR-grounded scenarios. In isolated tests, the engine detected 84.4% of designed clinical discrepancies and achieved 86.7% safety-critical recall.
The same study found a 13.6% error cascade when extraction and reconciliation were coupled: clinical details lost while extracting information from unstructured conversation affected later comparison. This illustrates why a system’s ability to flag conflicts cannot be assumed to make its end-to-end summaries safe. These results concern one architecture and dataset; they do not show that AI can determine which version of a patient’s account is true. How to evaluate an AI reconciliation workflow
For a patient, clinician, or health-system buyer, the useful question is not simply whether a tool says it can reconcile records. Ask how it handles identity, traceability, disagreement, errors, and evaluation.
| Check | What a safer workflow should make clear |
|---|---|
| Identity assurance | Which identifiers are used to link records, how ambiguous matches are handled, and when a person must review a possible match. ONC and GAO describe matching as a distinct safety and interoperability challenge. |
| Traceability | Whether every summary claim can be followed to its source document, date, and relevant passage, with enough context to interpret it. ISO 13606-1 describes provenance and preservation of intended clinical meaning as design aims. |
| Conflict behavior | Whether the interface shows both contradictory statements, distinguishes a detected conflict from a resolved fact, and avoids silently preferring recency or polished wording. |
| Error handling and governance | Who receives reports of false or inconsistent output, how errors are reviewed, and what qualified staff do before an output affects care. ONC’s SAFER Guides address organizational responsibilities and patient identification; they are safety guidance, not proof of a product’s performance. |
| Evaluation fit | Whether tests match the intended patients, record types, workflow, and clinically consequential errors. Benchmark scores and prototype results should not be treated as guarantees for a deployed system. |
ONC’s 2025 SAFER Guides were updated to focus on common, high-risk EHR safety issues and include guidance on patient identification and organizational responsibilities for AI-enabled EHR systems. They can help organizations examine workflow and governance, but do not certify that a particular AI feature preserves evidence correctly. ONC SAFER Guides.
Why human review remains necessary
Clinical records can conflict for reasons that require context beyond text comparison: a note may be incomplete, a statement may refer to a different time or circumstance, or the underlying identity match may be uncertain. An AI tool can help make these issues easier to find, but the evidence above does not establish broad real-world clinical performance for resolving them.
NHS England’s guidance for health and care professionals, last edited May 11, 2026, says AI can support clinical decision-making but care decisions should be made with the patient or service user using professional judgment. It also directs clinicians to raise false or inconsistent AI output through organizational channels. This guidance applies to England; it is not U.S. law. NHS England AI guidance for health and care professionals.
A practical standard for trustworthy AI summaries
A summary should make it easier to inspect the record, not replace that record or conceal uncertainty. A trustworthy workflow keeps identity checks distinct from clinical reconciliation, attaches source and context to claims, displays conflicts as conflicts, and gives clinicians a route to review and correct errors before they shape care.
