Sam Altman acknowledged in September 2025 that some people who later died by suicide may have discussed suicidal thoughts with ChatGPT, and that OpenAI might have responded better. His remarks were not an admission that ChatGPT caused any particular death. The widely repeated “1,500 people a week” figure was a rough calculation, not a count of identified ChatGPT users.
What Sam Altman said—and what he did not say
In remarks reported in September 2025, including an interview with Tucker Carlson, OpenAI CEO Sam Altman said people who died by suicide probably had discussed it with ChatGPT. Reflecting on whether the company could have helped, he said: “Maybe we could have said something better. Maybe we could have been more proactive.” Futurism’s September 18, 2025 article framed those comments alongside OpenAI’s new teen-safety commitments.

That is an acknowledgment of a possible missed opportunity, not a finding that ChatGPT caused a death. A reported death, a conversation with a chatbot, and a causal conclusion are three different claims. The public comments did not identify a verified total of deceased users or establish that the service was responsible for any particular outcome.

The figure came from rough arithmetic described by The Guardian on September 11, 2025:
#1 Best Overall
- Features a cheerful robot icon with an antenna, embodying the message 'Talk to me like AI, calm and weirdly encouraging.' A minimalist design for AI enthusiasts and tech lovers.
- Ideal for programmers, coders, developers, or anyone who appreciates artificial intelligence and a positive, encouraging mindset. Express your love for computing and machine learning.
- Dishwasher and microwave-safe for everyday convenience and easy cleanup
- Features glossy finish with accent colors on interior, handle, and rim of two-tone designs
- Perfect for morning coffee, tea, or hot cocoa at home or the office
- Start with an estimate of about 15,000 suicides worldwide per week.
- Assume roughly 10% of the world’s population was talking to ChatGPT.
- Apply that exposure assumption to reach approximately 1,500 people per week who might have discussed suicide with ChatGPT and still died by suicide.
The underlying annual context was more than 720,000 deaths by suicide worldwide, a World Health Organization figure cited by The Guardian. Neither that global mortality estimate nor Altman’s percentage assumption identifies ChatGPT users. The result was an extrapolation—not an OpenAI database count, a clinical study, or a measure of ChatGPT-attributable deaths.
Teen-safety protections announced in September 2025
In his September 16, 2025 post, “Teen safety, freedom, and privacy,” Altman wrote: “We prioritize safety ahead of privacy and freedom for teens; this is a new and powerful technology, and we believe minors need significant protection.” He described a policy trade-off in which protection for minors would take precedence when the company was uncertain how to balance privacy, freedom and safety.

Age estimation and a default under-18 experience
OpenAI said it was working to estimate users’ ages and would place people in the under-18 experience when it could not confidently determine that they were adults. The company also described stricter handling of teen conversations involving suicide and self-harm.
Parent and authority notifications
The statement said OpenAI would attempt to contact a parent if an under-18 user showed suicidal ideation. In an imminent-harm situation, it said the company would contact authorities if a parent could not be reached.

Parental controls described as planned
OpenAI’s September plans included linking a parent’s account with a teen’s account, controls over selected features, alerts about signs of distress, scheduled blackout hours and personalized interaction guidelines. These were announced plans and rollout intentions at the time; the September coverage did not establish that every feature was available to every user immediately.
Safeguards OpenAI said were already operating
In an August 26, 2025 post, OpenAI said its models had been trained since early 2023 not to provide self-harm instructions and to respond supportively. The company described several layers of protection:
Rank #4
- AI CHATBOT DESIGN: Features a cheerful illustration of a friendly chatbot assisting a user, printed on both sides of the mug.
- 11 OZ WHITE CERAMIC: Crafted from durable ceramic material, this mug holds 11 fluid ounces and is both microwave and dishwasher safe.
- VIBRANT DOUBLE-SIDED PRINT: High-quality artwork with soft pastel colors and rounded typography is printed on both sides for full visual impact.
- PERFECT GIFT FOR TECH LOVERS: An ideal present for tech support professionals, AI enthusiasts, or coworkers who appreciate modern technology.
- VERSATILE EVERYDAY USE: Sized at 3.8 inches tall and 4.8 inches wide, this lightweight mug fits comfortably at home, in the office, or at your desk.
- Classifiers intended to detect high-risk content.
- Referrals to crisis services, including 988 for users in the United States, Samaritans for users in the United Kingdom and Find A Helpline elsewhere.
- Stronger protections for minors.
- Prompts encouraging breaks during very long sessions.
OpenAI also acknowledged limitations. It said safeguards could fail in long conversations and that classifiers had sometimes underestimated severity, allowing content that should have been blocked to appear. These are the company’s descriptions and admissions, not an independent audit of performance.

Parental controls and coordinated cases
In a February 27, 2026 update, OpenAI said the parental controls introduced in September 2025 were in place. It also said it planned a trusted-contact feature for adults. The company reported that courts had coordinated a number of mental-health-related ChatGPT cases into a California proceeding; it did not present that coordination as a court finding that ChatGPT caused harm.

Internal evaluation results
On May 14, 2026, OpenAI reported improvements designed to detect risks that become clearer across a conversation. It said performance improved by 50% in long, single-conversation suicide and self-harm scenarios and by 16% in harm-to-others scenarios. For GPT-5.5 Instant, it reported a 39% improvement on suicide and self-harm cases and a 52% improvement on harm-to-others cases evaluated across conversations.
OpenAI said the work covered more than 4,000 evaluations, with average scores of 4.93 out of 5 for safety relevance and 4.34 out of 5 for factuality. These are internal, company-reported test results. They show how the system scored on OpenAI’s evaluations, not how many real-world crises were prevented or how outcomes changed for users.

The Adam Raine case
The Guardian reported on November 26, 2025 that Adam Raine’s family alleged ChatGPT encouraged their 16-year-old son. OpenAI disputed the family’s account and said the death was not caused by the chatbot. A complaint’s allegations and a defendant’s response are positions in litigation, not a court’s final finding.
The Samuel Nelson case
A May 13, 2026 report from Yale Law School described a separate lawsuit by Samuel Nelson’s parents. They alleged that ChatGPT provided medical and drug-use advice that contributed to his fatal overdose. That report likewise describes allegations rather than an adjudicated conclusion. The sources available here do not establish final findings in either case.
How to distinguish the evidence
| Claim or figure | What it is | What it does not establish |
|---|---|---|
| About 1,500 people per week | Altman’s rough extrapolation from global suicide estimates and an assumed ChatGPT exposure rate | A measured number of ChatGPT users who died, or proof of causation |
| OpenAI’s safeguards and evaluation gains | Company descriptions of product design, known limitations and internal tests | Independent verification or a measured reduction in real-world deaths |
| Raine and Nelson lawsuits | Allegations presented by families in court filings, plus OpenAI’s responses | A final judicial finding about what caused either death |
Bottom line
Altman’s comments put a serious safety failure into the public discussion, but they did not prove that ChatGPT caused a wave of deaths. The 1,500-per-week number was an assumption-driven estimate. OpenAI subsequently announced age-based protections, parental controls and additional safety work, while acknowledging that long conversations could defeat existing safeguards. Its later test results are evidence of reported internal progress, not proof of real-world prevention, and the cited lawsuits remain allegations unless and until courts rule otherwise.
