They require brain surgery, a long-term electrode and connector system, and an external computer that converts neural activity into text or synthesized speech. The main hazards are surgical and neurological complications, skin or connector problems, infection, hardware failure, uncertain durability, and errors that can make communication unreliable.
Early studies are encouraging but too small and varied to establish universal complication rates. A 2024 field review found no iBCI approved by a regulatory agency for the medical-device market, so any prospective user must weigh trial-specific risks against unproven benefits.

What a speech-restoration brain implant is
A cortical speech neuroprosthesis records activity from brain areas involved in speech planning or movement. Electrodes detect neural signals, an external decoder analyzes those signals, and software produces text, a computer voice, or another communication output. The implant does not restore normal biological speech and does not eliminate the need for backup communication methods.

Why the evidence is still limited
A Nature Reviews Bioengineering review covering 1998 through 2023 identified 28 clinical trials involving 67 implanted participants across 21 groups. It concluded that no implanted brain-computer interface had regulatory approval for the medical-device market at the time of publication in 2024. The participants, electrode designs, diseases, surgical techniques, follow-up periods, and outcome measures differed substantially, so an event observed in one study is not a universal rate.
What happens during implantation and use
- Neurosurgery: Surgeons open the skull and place electrodes on or in the cortex. Leads are routed to a connector, sometimes a percutaneous pedestal that passes through the skin.
- Signal recording: The electrodes capture electrical activity associated with attempted speech or related movements.
- Calibration: The user repeats or imagines words, and the decoder is trained to associate neural patterns with intended output.
- External processing: A computer receives the signals, runs decoding software, and generates text or synthesized speech.
- Ongoing maintenance: The system may require recalibration, software updates, troubleshooting, caregiver assistance, and management of cables or the external computer.
Each step creates a different failure point. Surgery can cause immediate medical complications; the skin interface can cause chronic wound problems; electrodes and leads can degrade or shift; and the decoder can become less accurate even when the hardware still works.
Major risks and side effects
Neurosurgical and perioperative complications
Implantation carries the ordinary hazards of cranial surgery and anesthesia. Possible complications include incisional pain, bleeding, infection, seizures, stroke or other neurological injury, cerebrospinal-fluid leakage, wound-healing problems, and anesthesia-related events. The available publications do not support one risk percentage that applies to every speech implant, because the devices and participants differ.

Skin, pedestal, and infection problems
A percutaneous pedestal creates a permanent skin opening around the connector. Friction, moisture, pressure, or bacterial growth can cause irritation, drainage, wound breakdown, or infection. Treatment may require wound care, antibiotics, temporary cessation of use, or another operation.
In the BrainGate feasibility safety study, 14 adults implanted between 2004 and 2021 contributed 12,203 days of safety follow-up. Investigators recorded 68 device-related adverse events, including six serious adverse events. “The most common device-related adverse event was skin irritation around the percutaneous pedestal,” according to the 2023 Neurology publication. The study reported no intracranial infections, deaths, explantation-required safety events, or permanent disability attributable to the investigational device under its one-year safety definition. Those results show what occurred in that selected cohort; they do not remove the possibility of infection or wound complications with another design or longer use.
Hardware failure, revision, and explantation
An implant is a system, not just an electrode. Electrodes, leads, connectors, cables, batteries or power components, and the external decoder can fail, become damaged, or become impractical to maintain. A connector can loosen, a cable can break, or signal quality can fall because the electrode-tissue interface changes.
If troubleshooting cannot restore useful operation, clinicians may consider revision or explantation. Either procedure can require another operation, with new anesthesia, bleeding, infection, seizure, and neurological risks. The absence of an explantation-required safety event in the BrainGate publication should not be generalized to all implant models, all follow-up periods, or future clinical use.

Neurological and functional risks
Because electrodes contact brain tissue, a complication can affect more than communication. Bleeding, swelling, seizure, stroke, or injury along the surgical path could produce weakness, sensory changes, cognitive effects, or other disability. The cited speech report and BrainGate study did not observe such a serious device-attributable outcome in their reported periods, but small cohorts cannot exclude rare events.
Why performance can decline without a hardware malfunction
A successful operation does not guarantee permanent communication benefit. Neural signals can vary with fatigue, illness, medication, electrode position, tissue response, calibration quality, or changes in the underlying disease. The decoder may need retraining, and an external computer remains necessary for many systems.
A seven-year New England Journal of Medicine report illustrates the distinction between physical integrity and useful control. At-home use eventually stopped because control became unreliable. Neural-signal amplitude declined, and CT imaging showed progressive brain atrophy. Investigators found no technical malfunction and considered disease-related neurodegeneration the likely explanation, while acknowledging that other explanations remained plausible. Durability is therefore condition-dependent rather than a fixed number of years.

Decoding errors
A decoder can omit words, substitute words, or produce unintended output. Errors may be merely frustrating during casual conversation, but they can become consequential if a caregiver, clinician, or emergency responder assumes that every output is accurate. Users need a way to confirm important messages and communicate when the decoder is unavailable.

Privacy and consent
Neural recordings and decoded language can reveal sensitive information. Prospective users should ask who stores raw signals, who can access decoded text or audio, how long data are retained, whether data leave the clinical site, and what happens if a sponsor or software supplier changes. Consent should cover future analyses and sharing, not only the initial implantation.
Uneven performance
Results from one speaker, disease, language, or laboratory setup cannot establish consistent accuracy for everyone. Published feasibility demonstrations do not prove normal-speed conversation in every environment, nor do they show that an implant will work equally well as disease progresses. Assistive communication support and a backup method remain important.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. How long do speech implants last?
There is no established service-life guarantee for cortical speech implants. Longevity depends on the electrode and connector design, tissue response, maintenance, the user’s medical condition, and the continued availability of decoding hardware and software. A device can remain physically implanted while losing practical usefulness.

The seven-year report demonstrates this point: the participant’s declining neural signals and brain atrophy made control unreliable even though investigators did not identify a technical failure. Conversely, a hardware or skin problem could require revision earlier. Trial documents should state the planned follow-up period, replacement policy, and what happens if the system stops working.

Do not confuse cortical speech implants with auditory brainstem implants
Auditory brainstem implants (ABIs) target hearing pathways rather than decoding intended speech from the cerebral cortex. A 2024 Neurochirurgie analysis of the FDA MAUDE spontaneous-report database identified 55 qualifying ABI reports. The reports included 27 cases describing failure to provide hearing benefit, 10 implant failures or malfunctions, and six cases of non-use.
Postoperative reports included local skin infection (three), cerebrospinal-fluid leak (three), elevated intracranial pressure (one), wound dehiscence (one), seroma requiring drainage (one), and meningitis (two). Thirty-five reports involved full explantation. These are counts from voluntary reports, not incidence rates, and they must not be transferred to cortical speech implants.
Questions to resolve before joining an implant trial
- Exact device and procedure: Ask where electrodes will be placed, whether the connector crosses the skin, how long surgery usually takes at that site, and what monitoring is used.
- Trial-specific events: Request the actual adverse-event history for the device, including minor skin events, serious events, withdrawals, revisions, and explantations.
- Alternatives: Compare the implant with non-invasive or assistive communication options on invasiveness, expected speed and accuracy, maintenance, reversibility, privacy, eligibility, and dependence on caregivers or external computers.
- Failure plan: Ask what happens after infection, signal loss, hardware failure, or loss of funding; who pays for revision or removal; and whether removal is recommended or optional.
- Long-term support: Confirm access to calibration, software updates, replacement computers, rehabilitation, emergency troubleshooting, and a backup communication method.
- Data governance: Read policies for raw neural data, decoded speech, recordings, storage, sharing, retention, and withdrawal of consent.
- Independent advice: Discuss the decision with an independent clinician who is not part of the implant study, especially if you have seizure risk, bleeding risk, infection risk, or progressive neurological disease.
Bottom line for prospective users
Cortical speech-restoration implants can provide a path to communication for carefully selected people, but they remain experimental. The known burden includes brain surgery and a chronic implanted system, while the potential benefit can be reduced by infection, skin problems, hardware failure, decoder errors, recalibration demands, privacy concerns, or progression of the underlying disease. Small studies have reported reassuring outcomes in particular participants, yet they cannot establish definitive rates or guarantee durable, accurate speech for an individual.
