Gene therapy may help when a person has a specific, treatable inherited retinal mutation and enough living target cells. A retinal implant is considered for a much narrower group with end-stage retinitis pigmentosa and extremely little or no light perception, provided the remaining visual pathway can respond to electrical stimulation. The choice depends on the diagnosis, genetic test, surviving retinal tissue, age, eye anatomy, surgical fitness, rehabilitation capacity and local availability.
For most patients, the first step is not choosing between a device and an injection. It is confirming exactly why vision was lost. A retinal specialist and, when inherited disease is possible, a genetics professional must establish whether either approach is biologically plausible and officially indicated.

What the two approaches actually do
Retinal implants create an artificial electrical signal
A retinal prosthesis uses hardware to bypass damaged photoreceptors. The Argus II system described in the U.S. Food and Drug Administration’s humanitarian device exemption (HDE) consists of an implanted electrode array and external glasses, a camera and a video processor. The camera captures a scene; the processor converts visual information into a pattern of electrical stimulation; and the implant delivers that pattern to retinal tissue. The patient learns to interpret the resulting visual signals through fitting sessions and visual rehabilitation.

This is not a replacement for normal sight. The system is intended for a very narrow end-stage retinitis-pigmentosa population and generally is implanted in one eye, typically the worse-seeing eye. Its indication specifies adults aged 25 or older who have bare or no light perception in both eyes, had useful form vision earlier in life, have an aphakic or pseudophakic eye (the natural lens is absent or has been replaced), and are willing and able to complete follow-up, device fitting and rehabilitation.

Gene therapy changes cellular biology
Gene therapy attempts to alter gene expression or the biological properties of living cells. As the FDA explains, “Gene therapy is a technique that modifies a person’s genes to treat or cure disease.” In retinal medicine, an engineered viral vector can deliver therapeutic genetic material to cells that are still present but cannot perform their normal function because of a mutation.
Luxturna (voretigene neparvovec-rzyl) is the key FDA-approved retinal example. It is approved only for confirmed biallelic RPE65 mutation-associated retinal dystrophy: both copies of the RPE65 gene must carry disease-causing variants, and the retina must retain viable cells that can receive and use the treatment. A diagnosis of retinitis pigmentosa by itself is not enough. Other likely pathogenic variants must be ruled out or judged less likely in the genetic assessment used for treatment eligibility.
Who may be a candidate?
| Question | Retinal implant (Argus II indication) | Gene therapy (Luxturna example) |
|---|---|---|
| Underlying condition | End-stage retinitis pigmentosa meeting the device indication | Inherited retinal dystrophy caused by confirmed biallelic RPE65 variants |
| Baseline vision | Bare or no light perception in both eyes, with useful form vision earlier in life | Determined by the disease and remaining viable retinal cells; the indication is not defined by the Argus-style bare/no-light threshold |
| Age requirement | Adults aged 25 years or older under the FDA HDE | Eligibility is primarily genotype- and disease-based; a specialist must apply the current product labeling |
| Required anatomy or biology | Retina and downstream visual pathways must be capable of responding to electrical stimulation; lens status must be aphakic or pseudophakic | Viable target cells and a treatable RPE65-related mechanism are required |
| Main intervention | Surgery to implant an electrode array, followed by external hardware fitting and rehabilitation | Subretinal delivery of the gene-therapy vector, followed by safety monitoring |
| Typical treatment pattern | Generally one implanted eye, often the worse-seeing eye | CMS coverage criteria describe one treatment per eye per lifetime for qualifying beneficiaries |
| Expected goal | Useful task-level visual signals, such as detecting light or improving performance on selected mobility tasks | Improved visual function in people whose RPE65-related cells can respond; not guaranteed normal acuity or independence |
The table shows why these options are not interchangeable. An implant is a hardware strategy for profound, late-stage loss. Gene therapy is a molecular strategy for a particular genetic cause while responsive retinal cells remain.
How doctors establish the diagnosis before discussing treatment
1. Separate retinal degeneration from other causes of blindness
Vision loss can arise from an inherited retinal degeneration, an acquired retinal disease, optic-nerve damage, brain disease, trauma or several conditions at once. Neither a retinal implant nor Luxturna is a general treatment for every cause of blindness. The specialist will review the history of night blindness and visual-field loss, prior useful vision, family history, examination findings and previous imaging or electrophysiology. If the principal problem is outside the retina, these treatments may not address it.

2. Use validated genetic testing when inherited disease is suspected
A genetic panel can identify variants associated with inherited retinal disease, but a result must be interpreted rather than simply read from a laboratory report. For Luxturna, the critical finding is pathogenic or likely pathogenic variation in both RPE65 copies, with the clinical picture consistent with RPE65-associated retinal dystrophy. Genetic counseling helps explain uncertain variants, inheritance, testing limitations and implications for relatives. A single RPE65 variant, a variant of uncertain significance or a different gene does not establish eligibility.
3. Measure how much usable retina and visual pathway remain
Gene therapy requires living target cells. If the relevant cells have disappeared, delivering a normal gene cannot recreate them. An implant has a different requirement: the retina and the neural pathway from the eye toward the brain must still be able to respond to electrical stimulation. Examination, retinal imaging and functional tests help the team judge whether either biological route remains plausible. Severe optic-nerve or central visual-pathway damage can limit an implant even when the eye itself is profoundly impaired.
4. Check eye anatomy and general surgical fitness
For the Argus II indication, the eye must meet the specified lens-status requirement (aphakia or pseudophakia), and the person must be able to undergo surgery. The team also considers scarring, inflammation, retinal complications, anesthesia risk, other eye disease and the ability to attend repeated programming and rehabilitation visits. Gene therapy involves an ocular procedure as well, so retinal condition, inflammation risk and overall health still matter even though it does not involve an implanted electronic device.

What treatment and aftercare involve
Retinal-implant pathway
- Specialist evaluation: The center confirms that the person meets the narrow indication, reviews prior useful vision and assesses both eyes and the visual pathway.
- Implant surgery: The electrode array is placed on or near the retina in the selected eye. The external glasses, camera and video processor are used after healing.
- Initial fitting: Clinicians adjust stimulation settings and map the device to the patient’s responses. The first signals may be unfamiliar rather than picture-like.
- Visual rehabilitation: Structured training teaches the patient to interpret flashes, edges, movement or contrast cues for practical tasks. Progress depends on practice, residual pathway function and the particular task.
- Long-term follow-up: Device checks, software or hardware support, eye examinations and rehabilitation visits remain part of living with an implanted system.
Gene-therapy pathway
- Confirm the molecular diagnosis: The treating team verifies biallelic RPE65-associated disease and excludes a more likely genetic explanation.
- Assess retinal viability: Imaging and functional testing determine whether enough target tissue remains for treatment to be meaningful.
- Plan ocular delivery: The vector is delivered under the retina by a specialist. The exact operative details and monitoring schedule are set by the treating center and current product information.
- Monitor for complications: The team watches for inflammation, retinal problems, pressure changes, infection and other ocular or immune-mediated events, then treats them according to the product protocol.
- Continue long-term surveillance: FDA guidance emphasizes extended follow-up for gene therapies because delayed adverse events can occur after administration of some products.
CMS criteria for qualifying beneficiaries describe Luxturna as one treatment per eye per lifetime. That is fundamentally different from a medication that can simply be repeated whenever vision changes.

What the evidence shows—and what it does not
Argus II evidence
A five-year clinical-trial report in an American Academy of Ophthalmology journal followed 30 selected participants; 24 remained implanted with functioning Argus II systems at five years. Participants performed significantly better with the system switched on than off on objective visual-function measures and selected real-world tasks. These results demonstrate potential task-level benefit in the studied population, not restoration of ordinary visual acuity and not a guarantee for every person with blindness.
Luxturna evidence
The Centers for Medicare & Medicaid Services evidence summary reports that, at one year, bilateral multiluminance mobility testing improved by 1.8 (standard deviation 1.1) in the Luxturna treatment group versus 0.2 (standard deviation 1.0) in the control group, with a reported p value of 0.0013. This is a measured mobility outcome under the study conditions; it should not be translated into a promise of normal sight, unrestricted driving or complete independence.
FDA states that a gene-therapy product must be tested in clinical studies for safety and effectiveness before marketing, so regulators can judge whether risks are acceptable in relation to potential benefits. Long-term durability, late effects and performance in people outside the studied eligibility criteria remain important parts of specialist counseling.

Potential advantages of gene therapy
- It addresses a defined molecular cause rather than adding an external camera-and-electrode system.
- When viable RPE65-target cells remain, the treatment can improve a functional measure such as navigation under low-light conditions.
- It does not require a permanent electronic implant or daily use of external glasses and a processor.
Important gene-therapy limitations
- Luxturna applies only to confirmed biallelic RPE65-associated disease, not to inherited retinal disease in general.
- Advanced cell loss can limit benefit even when the genetic result is technically eligible.
- The procedure can cause ocular inflammation, retinal complications or other surgical and immune-related problems.
- Follow-up is long term, and CMS criteria describe only one treatment per eye over a beneficiary’s lifetime.
Potential advantages of a retinal implant
- It may provide usable visual cues to a person with profound end-stage retinitis pigmentosa who has no approved gene-specific option.
- The system can be adjusted through fitting and rehabilitation as the patient learns which stimulation patterns are useful.
- Clinical evidence shows measurable performance differences with the system on versus off in a selected study population.
Important implant limitations
- The indication is exceptionally narrow: age, prior useful vision, bilateral bare or no light perception, lens status and rehabilitation ability all matter.
- Surgery introduces risks associated with an implanted device and the eye; the center must explain those risks for the individual case.
- External glasses, a camera and processor are required, and meaningful use depends on repeated training.
- The signals are not normal detailed vision. A device can help with selected detection or mobility tasks without restoring reading vision or independent travel.
A practical decision pathway
- Document the cause of vision loss. Ask whether the diagnosis is an inherited retinal degeneration, acquired retinal disease, optic-nerve disorder or another condition.
- Complete genetic testing and counseling if appropriate. If RPE65 disease is suspected, obtain validated testing that can establish whether both gene copies carry qualifying variants.
- Ask about remaining biology. Have the retinal specialist explain whether enough viable target cells remain for gene therapy and whether the retina and downstream visual pathway could respond to an implant.
- Check formal eligibility. For an implant, review age, prior useful form vision, light perception in both eyes, lens status and ability to complete rehabilitation. For Luxturna, review the confirmed biallelic RPE65 diagnosis and current labeling.
- Compare the lived treatment burden. Discuss surgery, ocular medicines, programming, rehabilitation sessions, travel, caregiver support and long-term monitoring rather than focusing only on the procedure day.
- Define a useful outcome. Decide whether the goal is detecting obstacles, navigating in dim light, locating objects, recognizing larger shapes or another measurable task. “Restore sight” is too broad to serve as a treatment target.
- Obtain authorization and access information. Ask the center and insurer about eligibility documentation, prior authorization, travel, rehabilitation coverage and personal expenses.
- Verify current availability. Confirm that the proposed center is actively providing the therapy and that it is available in the patient’s country. Device programs, treatment centers, payer policies and costs can change.
Costs, insurance and availability
A single canonical source does not establish a current universal price for either approach, and a patient’s out-of-pocket amount depends on country, insurance, eligibility, hospital contracts, surgery, medicines, travel and rehabilitation. Do not rely on an old internet price or assume that regulatory approval guarantees coverage.

Before scheduling, request a written estimate that separates the treatment itself from genetic testing, imaging, surgery, anesthesia, medications, programming, rehabilitation, follow-up and management of complications. Ask which services are considered medically necessary, which require prior authorization and what happens if the patient travels to another region or country.
Because availability is volatile, contact a retinal-dystrophy or retinal-prosthesis center directly. A center should be able to state whether it is currently operating, which diagnoses it accepts, what records it needs and how referrals are handled.
Questions to take to the specialist
- What is the precise cause of my vision loss, and what findings support that diagnosis?
- Do my genetic results show biallelic RPE65 disease, or is another gene more likely?
- How much viable retina remains, and how was that assessed?
- For an implant evaluation, do I meet every element of the FDA indication, including age, prior useful vision, bilateral bare or no light perception and lens status?
- Which visual tasks are realistic for me, and how will improvement be measured?
- What surgery, inflammation, retinal, immune and anesthesia risks apply to my eyes and health?
- How many fitting, rehabilitation and follow-up visits are expected during the first year and later?
- What equipment, medicines or caregiver help will I need at home?
- Is the program currently operating, and can you provide a written estimate of covered and uncovered costs?
- What alternatives remain if I am not eligible or if the expected benefit is too small for the risks?
Common misunderstandings to avoid
“Gene therapy is automatically better because it uses biology.”
Gene therapy is appropriate only when the mutation, viable cells and product indication align. A person with a different inherited mutation or no surviving target cells cannot assume that the same treatment will work.
Do these 3 things before closing this tab:
1Scan for outdated or missing drivers - takes under a minute2Repair Windows errors before they cause bigger problems3Fix the driver behind crashes, sound loss and screen glitches“A bionic eye restores ordinary eyesight.”
An implant supplies artificial electrical cues that must be interpreted through training. The evidence supports selected functional improvements in carefully selected participants, not normal acuity or guaranteed independence.
Best Value
“Any retinitis pigmentosa qualifies for an implant.”
The Argus II indication requires a specific combination of adult age, bilateral bare or no light perception, previous useful form vision, suitable lens status and ability to complete follow-up and rehabilitation. Many people with retinitis pigmentosa will not meet all of these conditions.
“A positive genetic test is enough for Luxturna.”
Eligibility requires confirmation of biallelic RPE65-associated retinal dystrophy and enough viable retinal tissue. Variants of uncertain significance or a single affected gene copy do not establish that indication.
“Low-vision aids are vision-restoration treatments.”
Magnifiers, lighting changes, screen readers and orientation-and-mobility training can improve safety and independence, but they do not repair retinal cells or replace the decision about a retinal implant or gene therapy. They may still be valuable whether or not a person qualifies for either intervention.

Bottom line for choosing between them
Start with the diagnosis, not the technology. Confirm the genetic cause and remaining retinal function; then ask whether your anatomy and health fit the formal indication, whether the expected task-level benefit justifies surgery or biologic treatment, and whether you can obtain the required rehabilitation and long-term follow-up. Luxturna is a mutation-specific option for biallelic RPE65 retinal dystrophy. A retinal implant is an end-stage retinitis-pigmentosa option for a narrowly defined adult population with profound bilateral vision loss. A specialist evaluation is the only reliable way to determine which, if either, fits your eyes.
