In an 18-month randomized trial of older adults with obesity, adding resistance training to a weight-loss program limited lean-mass loss compared with adding aerobic training. Both exercise groups lost more fat mass than the weight-loss-only group. The result favors resistance training for preserving more lean mass in this specific setting; it does not show that it prevents all muscle loss or is the best workout for every older adult.
What the 18-month trial compared
The study, titled Effect of Exercise Type During Intentional Weight Loss on Body Composition in Older Adults with Obesity, enrolled 249 adults. Participants had a mean age of 66.9 years, 71% were women, and their mean body mass index was 34.4 kg/m². Researchers randomly assigned them to one of three groups for 18 months: weight loss alone (82 participants), weight loss plus aerobic training (86), or weight loss plus resistance training (81). The PubMed record describes the trial’s aim as examining how exercise type during weight loss affected body composition and how those changes related to physical function.

How the three approaches differed
| Program | Fat-mass finding | Lean-mass finding |
|---|---|---|
| Weight loss alone | Less fat-mass loss than either group that added exercise. | The available study excerpts do not establish a numerical change for this group. |
| Weight loss plus aerobic training | More fat-mass loss than weight loss alone. | More lean-mass loss than the resistance-training group. |
| Weight loss plus resistance training | More fat-mass loss than weight loss alone. | Less lean-mass loss than the aerobic-training group; this is a relative advantage, not evidence that no lean mass was lost. |
The reported findings support two distinct points: exercise added to weight loss was associated with greater fat-mass loss than weight loss alone, and resistance training was associated with less lean-mass loss than aerobic training. They do not establish that resistance training produced more fat loss than aerobic training.
What “keep muscle” means here
The headline’s shorthand should not be read as a promise that participants kept all their muscle. The reported comparison concerns lean mass, a body-composition measure, and indicates that the resistance-training group lost less of it than the aerobic-training group while losing weight. It does not establish muscle gain, zero muscle loss, or the exact amount preserved.

The available report describes the resistance intervention as weight-machine workouts. That identifies the kind of training used in the study, but it is not a test of home gym equipment, a comparison of machines with free weights or body-weight exercise, or evidence that a machine is necessary.
Who the findings apply to—and who should be cautious
The participants were older adults with obesity who undertook intentional weight loss as part of a long-term study. The result should not be generalized without qualification to all older adults, people at different body weights, or people with health conditions or mobility constraints that affect exercise choices. The trial’s group comparison also does not replace individualized guidance from a clinician or qualified exercise professional.

What readers can reasonably take away
- For older adults with obesity pursuing weight loss, the trial supports resistance training as a way to limit lean-mass loss relative to aerobic training.
- Adding either type of exercise was associated with greater fat-mass loss than weight loss alone in this study.
- The evidence does not show that one workout is universally best, that resistance exercise eliminates muscle loss, or that participants need to buy exercise machines.
