Instead of relying only on parameters learned during training, a RAG system finds relevant passages, places them in the prompt, and asks the model to answer from that evidence. A practical system usually combines semantic embeddings, lexical search such as BM25, optional reranking, careful chunk selection, and a token budget that leaves room for the answer.
RAG is useful when knowledge changes, when answers must be grounded in a private corpus, or when citations and traceability matter. A larger context window can hold more text, but it does not guarantee that every passage will be noticed or used correctly. Retrieval quality, ordering, and the amount of context sent to the model still determine the result.

What is RAG and why does it matter?
Retrieval-augmented generation is a two-part process. A retrieval system searches a knowledge base for evidence, and a generative model uses the selected evidence to produce an answer. The retrieved passages are appended to the user’s prompt, so the model can work with documents that were not part of its original training data or that have changed since training.
This architecture separates storage from generation. Documents can be updated, access-controlled, re-indexed, or deleted without retraining the language model. It also makes it possible to show the passages that supported an answer. RAG is not a guarantee of truth: a retriever can select the wrong text, the corpus can be incomplete, and the generator can still misread or contradict its context. Those failure modes must be measured separately.

When RAG is a good fit
- Internal policies, manuals, tickets, contracts, or product documentation that a model cannot safely memorize.
- Frequently changing information where rebuilding an index is cheaper and faster than retraining a model.
- Question answering that needs source passages, document locations, or auditable evidence.
- Large collections in which sending every document on every request would exceed the prompt budget.
What RAG does not solve by itself
- It does not repair inaccurate or outdated source documents.
- It does not ensure that a passage contains the answer merely because it is semantically similar to the query.
- It does not remove the model’s context-length, attention, latency, or generation limits.
- It does not provide reliable citations unless the application preserves source metadata and instructs the generator how to use it.
How a RAG pipeline works
A typical implementation has an offline indexing path and an online question-answering path. The indexing path prepares the corpus; the online path searches it for each query.
- Collect and clean documents. Extract text, remove navigation or duplicate boilerplate, retain headings and other structure, and attach metadata such as title, section, date, permissions, and source identifier.
- Chunk the corpus. Split documents into passages small enough to retrieve and place in a prompt. Boundaries and overlap affect whether an answer remains understandable when a chunk is retrieved without its neighbors.
- Create embeddings. Convert every chunk into a numeric vector that represents its meaning. Store the vectors in an index that supports similarity search.
- Embed the query. Convert the user’s question with the same embedding model or a compatible query model.
- Retrieve candidates. Search for the nearest vectors. If exact words matter, run BM25 or another lexical search in parallel and combine the rankings.
- Rerank when justified. Pass a wider candidate pool to a second-stage model that scores each query-passage pair, then keep only the highest-scoring passages.
- Assemble context. Order the selected passages, include useful titles and source metadata, and enforce a token budget that reserves space for instructions and the answer.
- Generate and cite. Ask the language model to answer from the supplied context, state uncertainty when the evidence is insufficient, and return source references that map to the retained passages.
The first four steps are normally performed when documents change. Retrieval, reranking, context assembly, and generation happen for each question.
Embeddings: how semantic retrieval finds related meaning
An embedding is a vector representation in which texts with related meaning tend to occupy nearby regions. A query about cancelling a subscription can therefore retrieve a passage that says “terminate an account,” even when the words do not match exactly. Similarity is usually calculated with a distance or cosine-similarity function, although the index hides that mathematical detail from most applications.

Where embeddings can miss
Semantic vectors can underweight exact identifiers, product codes, names, error strings, version numbers, and legal phrases. A query containing an incident code may need the exact token match even if a semantically similar passage is otherwise relevant. That is why production systems often combine dense retrieval with lexical retrieval rather than choosing one universally.
Chunking and metadata determine what the vector represents
Embedding a passage also embeds its boundaries. A chunk that starts halfway through a procedure may be hard to interpret; a very large chunk may mix several unrelated topics and dilute similarity. Preserve headings, section labels, and document dates in the indexed text or metadata so that the retrieved result carries enough context to be useful and can be filtered for permissions or recency.
There is no universally correct chunk size or overlap. The right setting depends on document structure, query style, embedding model, and the model that will read the results. Evaluate alternatives on representative questions instead of adopting a fixed number as a rule.
Why hybrid search adds BM25 to embeddings
BM25 is a lexical ranking method. It rewards terms that occur in a document while accounting for term frequency and how common each term is across the collection. It is especially valuable for exact strings: ticket numbers, API parameters, chemical names, filenames, and quoted language.

A hybrid retriever runs dense and lexical searches, then fuses their rankings. Reciprocal-rank fusion is one common approach: a result receives contributions based on its position in each list, so a passage ranked highly by either method can remain in the candidate pool. The exact fusion formula and weighting are implementation choices that should be tuned against your evaluation set.
Hybrid search is not automatically better. It can increase indexing and query complexity, and noisy lexical matches may enter the candidate pool. Its benefit is greatest when the corpus mixes ordinary prose with identifiers or when users alternate between conceptual and exact-term questions.
What a reranker does
A reranker is a second-stage relevance model. The initial retriever favors speed and recall: it returns a broad candidate set that probably contains the answer. The reranker examines each query-candidate pair in more detail, scores relevance, and forwards a smaller top-K set to the generator.
Why reranking can improve answers
- It can distinguish a passage that merely shares vocabulary from one that actually answers the question.
- It can reorder mixed dense-and-lexical results before the context is assembled.
- It can reduce the number of irrelevant tokens sent to the generator, improving focus and sometimes reducing inference cost.
The trade-off
Reranking adds another model invocation, so it increases latency and model expense. A small candidate pool may leave the correct passage out before the reranker sees it; an excessively large pool can erase the speed advantage. Retrieve broadly enough to protect recall, then measure whether the relevance improvement justifies the added delay and cost.

Context windows: more capacity is not guaranteed comprehension
A context window is the maximum token budget available to the complete request: system and user instructions, retrieved passages, conversation history, and the model’s response. Retrieved evidence must fit inside that limit, and reserving output space is essential. Filling the entire window with documents can leave too little room for a useful answer or cause truncation.
Long-context models can process tens of thousands of tokens, but capacity is not the same as effective use. Position, repetition, document order, and distractors influence what the model attends to. A carefully selected and structure-preserving set of passages can outperform a larger undifferentiated dump.
Controlled EMNLP 2025 results reported that a straightforward structure-preserving retrieve-then-read baseline, called DOS RAG, consistently matched or outperformed more intricate methods on several long-context question-answering benchmarks when token budgets were matched. The practical lesson is to compare systems at equal budgets and on the same questions, rather than assuming that a long-context prompt automatically beats retrieval.

How to calculate a safe context budget
Start with the model’s current maximum sequence length for the specific provider and version. Subtract instruction tokens, conversation history, formatting overhead, and a response allowance. The remainder is the budget for retrieved evidence. Keep a margin for tokenization differences and unusually long metadata.
For example, if a request allows 32,000 tokens, your fixed instructions and history consume 2,000, and you reserve 4,000 for the answer, no more than roughly 26,000 tokens remain for retrieved material. That is a planning example, not a universal setting; actual limits and tokenizer behavior vary by model.
How many chunks should go in the prompt?
There is no universal top-K value. The correct number is the largest set that improves answer quality without exhausting the token budget or adding distractors. Treat top-K as an evaluation parameter, not a permanent constant.
| Decision | What to test | Why it matters |
|---|---|---|
| Initial candidate count | Several wider pools from dense, lexical, or hybrid retrieval | A wider pool protects recall before reranking but costs more latency. |
| Reranked count | Small top-K values that fit the evidence budget | Fewer passages reduce noise; too few can omit necessary supporting details. |
| Chunk overlap | Structure-aware alternatives | Overlap can preserve context across boundaries but duplicates tokens. |
| Ordering | Score order versus source order | Score order emphasizes relevance; source order preserves relationships across adjacent sections. |
| Answer reserve | Output allowances matched to the task | A long explanation needs more generation space than a short classification. |
Use a held-out set of real questions and record retrieval recall, answer correctness, citation support, latency, and token usage for each configuration. If adding chunks raises recall but lowers grounded answer quality, the extra context is not helping the reader.

| Design | Strength | Cost or risk | Best use |
|---|---|---|---|
| Dense embeddings only | Strong semantic matching and simple query flow | May miss exact identifiers or quoted phrases | Natural-language questions over conceptually consistent text |
| BM25 only | Reliable lexical and identifier matching | Weak when the query and passage use different wording | Error codes, names, codes, and exact terminology |
| Hybrid dense plus BM25 | Combines semantic recall with exact-term coverage | More indexes, fusion logic, and tuning | Mixed corpora and varied query styles |
| Hybrid plus reranker | Higher precision in the final context | Additional model latency and cost | High-value answers where relevance justifies a second stage |
| Long-context retrieve-then-read | Can preserve more document structure in one request | More input tokens and possible distractors | Tasks where related passages must remain together |
Track retrieval recall, answer faithfulness, exact-term coverage, reranking precision, latency, token and inference cost, context-window utilization, implementation complexity, source-order preservation, and citation quality. NIST TREC RAG evaluations illustrate this multi-stage style of comparison, including BM25, dense embeddings, reciprocal-rank fusion, and rerankers.

2. Establish a simple baseline
Begin with structure-aware chunks, one embedding model, and a straightforward nearest-neighbor search. Log the query, retrieved identifiers, scores, token counts, and final citations. This baseline gives you something measurable before adding complexity.
3. Add lexical retrieval for exact terms
Run BM25 alongside dense search when users ask about identifiers, names, codes, or wording that must match precisely. Fuse the rankings and inspect failures to determine whether the lexical component improves recall without overwhelming the candidate list with noise.

Retrieve a broad candidate pool, rerank it, and pass only the selected passages onward. Compare the relevance gain with the additional model call, latency, and cost. Keep reranking where it improves the target task; remove it where it adds expense without measurable benefit.
5. Assemble evidence with traceability
Include document title, section, date, and a stable source key beside each passage. Preserve original order when a procedure or argument spans adjacent sections, or use score order when independent snippets are easier to compare. Instruct the generator to distinguish supported facts from missing evidence.
6. Evaluate at matched token budgets
Compare a RAG pipeline with a long-context alternative using the same questions and equivalent input-token allowances. Measure both retrieval metrics and end-to-end answers; a high retrieval score is not useful if the generator ignores the passages.
Known results and how to interpret them
Anthropic reported experiments in 2024 on contextual retrieval across several domains and configurations. Adding contextual embeddings reduced the reported top-20 retrieval failure rate by 35 percent; combining contextual embeddings with contextual BM25 produced a 49 percent reduction; adding reranking produced a 67 percent reduction. These are Anthropic’s experimental results, not universal benchmarks. Your corpus, chunking, embedding model, candidate count, and evaluation questions can produce different outcomes.

Anthropic recommends running evaluations for the chosen chunking, embeddings, top-K, and reranking settings. That recommendation matters because retrieval errors are highly corpus-dependent: a policy library, source-code repository, and customer-support archive have different vocabulary, structure, and notions of relevance.
Best Value
Common failure modes and fixes
The answer uses a related but wrong passage
Inspect the candidate list before blaming the generator. If the correct passage is absent, improve chunking, metadata, embeddings, hybrid search, or the initial candidate count. If it is present but ranked low, test fusion weights or a reranker.
Exact codes or names are missed
Add BM25 or another lexical signal, preserve punctuation and identifiers during cleaning, and verify that tokenization has not split or discarded the critical string.
The prompt is too large
Reduce candidate count, rerank before generation, remove duplicated overlap, shorten metadata, or reserve a larger output margin. Do not silently truncate the newest or highest-ranked passage.
The model ignores evidence in a long prompt
Reduce irrelevant chunks, preserve source structure, place the most useful evidence consistently, and compare a shorter retrieved prompt against the long-context version at the same token budget.
Citations do not support the answer
Keep source identifiers attached to every chunk through retrieval and generation. Check that each cited passage actually entails the associated claim, and instruct the model to say when the corpus does not establish an answer.
Bottom line: design for measured relevance, not maximum context
RAG works best as a measured retrieval system rather than a single embedding lookup. Start with clean, structure-aware chunks and metadata; use embeddings for semantic recall; add BM25 for exact language; rerank only when its precision gain is worth the latency; and select context within a deliberate token budget. Long context is useful capacity, but matched-budget evaluations and corpus-specific testing determine whether it improves the answers your users actually need.
