The company calls this clinical proof-of-mechanism for RNA activation; it is an early target-engagement result, not proof that RAG-18 improves Duchenne muscular dystrophy (DMD) symptoms or slows disease.
What is RAG-18?
RAG-18 is an investigational small activating RNA (saRNA) designed to target regulatory regions of the human UTRN gene and increase production of the body’s own utrophin. Ractigen says it uses its Lipid-Conjugated Oligonucleotide (LiCO) technology to deliver the RNA. The approach is intended to use the body’s transcription machinery, rather than a viral vector or permanent DNA editing.
Utrophin is related in structure and function to dystrophin, the protein deficient or absent in DMD. Increasing utrophin could, in principle, avoid targeting a particular DMD mutation. That is a biological rationale, not evidence that the treatment benefits people across genotypes.
What did Ractigen report at WMS 2026?
At the 31st Annual Congress of the World Muscle Society in Hiroshima on October 3, 2026, Ractigen presented results from its ongoing first-in-human Phase I study, NCT07282652. The late-breaking oral presentation, titled “First-in-human evidence of RNA activation-mediated sarcolemmal utrophin upregulation in Duchenne muscular dystrophy,” was given by principal investigator Professor Yi Dai, MD, PhD, of Peking Union Medical College Hospital.

Who was studied
The company described the trial as an open-label, dose-escalation study of ambulatory boys aged 4–15 with genetically confirmed DMD. The reported Cohort 1 findings concern three participants who received 15 mg by monthly intravenous infusion. The reported observation period extended through Day 169; the company also presented measures collected over 24 weeks. There was no randomized comparator in the reported results.
What the company called proof-of-mechanism
Ractigen reported that paired muscle biopsies taken at baseline and Day 113 showed more utrophin signal at the sarcolemma, the membrane surrounding muscle fibers. Whole-field quantitative immunofluorescence found a 3.5- to 5.3-fold increase in signal density in mature myofibers and a 4.1- to 4.7-fold increase in regenerating myofibers compared with baseline. The company said co-staining confirmed the signal’s sarcolemmal localization.

This is the central mechanistic finding: evidence that the intended molecular target responded in human muscle. The reported fold changes describe biopsy signal, not a comparable increase in muscle strength or function.
What other results did the company report?
Ractigen also reported exploratory tissue, imaging, breathing, motor, and cardiac measures. The figures below are sponsor-reported for the three-person Cohort 1, not results from a controlled efficacy comparison.

What does the safety report establish?
Ractigen reported zero dose-limiting toxicities and zero serious adverse events among the three Cohort 1 participants, and no treatment-emergent adverse events of Grade 3 or higher. The reported adverse events were Grade 1–2, transient, and resolved without medical intervention. The company reported no dose interruptions, reductions, or discontinuations.
Those observations describe only three people at the reported dose and follow-up. They cannot establish a definitive safety profile or rule out uncommon or longer-term risks.

What remains uncertain?
Target engagement is not clinical efficacy
The biopsy findings support the claim that RAG-18 was associated with increased utrophin signal in sampled muscle. They do not show that the change produces durable gains in walking, breathing, or other meaningful outcomes. The functional measures were exploratory and mixed: six-minute walk results ranged from a decline of 62.5 m to an increase of 36.5 m, while NSAA fell by 3 points in each participant.
The evidence is preliminary and company-reported
The results were presented by the sponsor from an open-label first cohort of three, without a control group or an independent statistical analysis identified in the announcement. A larger controlled dataset and independent assessment would be needed to determine whether the reported tissue changes translate into clinically meaningful benefit.

Ractigen said the 30 mg monthly intravenous Cohort 2 was fully enrolled, with safety follow-up continuing. The announcement does not provide outcome results for that cohort.
Is RAG-18 approved or available as a treatment?
No. RAG-18 is described as investigational, and the reported WMS findings do not establish it as an approved treatment. Ractigen has said the candidate received U.S. FDA orphan drug and rare pediatric disease designations in 2024; those designations are not marketing authorization and do not demonstrate efficacy.
