Before starting an obesity treatment, ask your doctor how it fits your health, goals, medical history, and preferences—and how you will know whether it is helping. Bring a list of your medicines and questions, and agree on follow-up before making changes. Treatment decisions are personal; body mass index (BMI) can inform the discussion, but it does not by itself determine which option is right for you.
Start with your health and goals
You can begin by asking:
- “Am I at a healthy weight?”
- “How is my weight affecting my health?”
- “Will losing weight improve my general health, as well as specific health problems I have?”
Then tell your doctor what matters most to you—for example, improving a health condition, mobility, or day-to-day wellbeing. Ask which factors besides BMI should shape the decision, and what terms you prefer the care team to use when discussing weight. NIDDK recommends asking permission to discuss weight, addressing the patient’s main health concerns, and setting goals collaboratively. NIDDK: Talking with Your Patients about Weight

Compare the treatment choices
Depending on your circumstances, options may include a structured lifestyle program, prescription medication, surgery, or—in some cases—a device. Your plan may combine approaches. Ask your doctor why a particular option may suit your health history and goals, what trade-offs it involves, and what alternatives are available. No average result can predict your individual outcome.
| Approach | What to ask about | Useful context |
|---|---|---|
| Structured lifestyle program | Does it include a reduced-calorie eating and drinking plan, appropriate physical-activity guidance, ongoing support, and a plan to maintain changes? Can staff coordinate with my clinician? | NIDDK describes these as features of a formal program. The U.S. Preventive Services Task Force recommends offering or referring adults with obesity to intensive lifestyle-change programs, as summarized by NIDDK. NIDDK reports an average 5% to 7% body-weight loss through lifestyle changes among adults; this is an estimate, not a personal prediction. | Prescription medication | Which medicine, if any, fits my health conditions and other medicines? What side effects should I expect, how long might I take it, and how will we review whether it helps? | Some people lose 10% or more of starting body weight with weight-loss medicines, but results vary by medicine and person. NIDDK separately reports that, after one year, adults taking prescription medicines as part of a lifestyle program lost 3% to 12% more of their starting body weight than adults in a lifestyle program without medication. These are different estimates and neither is a guarantee. Confirm current approvals, labeling, availability, and cost with your clinician. | Surgery | What does the operation involve? What benefits, risks, eating and activity changes, and long-term follow-up should I expect? Am I a candidate, and what other options should I consider? | NIDDK gives examples of adult candidacy criteria: BMI of 40 or more; BMI of 35 or more with a serious obesity-related health problem; or BMI of 30 or more with difficult-to-control type 2 diabetes. These examples are not a universal rule or a substitute for individual assessment. NIDDK reports about 20% average loss of starting weight after weight-loss surgery, with results varying by person and procedure. | Device | Why might a device be considered for me? What are its risks, expected benefits, costs, and follow-up requirements? | NIDDK notes that devices may be considered in some circumstances and that their long-term risks and benefits are still being studied. Ask about medicines, conditions, and pregnancy
If medication is under consideration, bring a complete list of what you take or use—not just prescriptions. Ask the clinician to review interactions and whether any medical conditions, allergies, or family history affect the choice. Do not start, stop, combine, or substitute medicines or weight-loss products without your health professional’s guidance.

- Could this interact with my prescription or over-the-counter medicines, vitamins, supplements, or herbal products?
- Do my health conditions, allergies, or family history change the risks?
- How could pregnancy plans, pregnancy, or breastfeeding affect this option?
- What side effects or warning signs should prompt a call, and what should I do if they occur?
NIDDK says weight-loss medicines should not be used during pregnancy or when planning pregnancy and are not recommended while breastfeeding. It also advises against combining weight-management medicines with other weight-loss products unless a health professional prescribes them. How long treatment continues depends on whether the medicine helps and whether serious side effects occur. NIDDK: Prescription Medications to Treat Overweight & Obesity
Agree on goals, monitoring, and next steps
Ask what benefit is realistic for you and how you and your clinician will measure it. Decide when to check in, what would count as enough benefit, and what you will do if the treatment is not helping or causes problems. For a program or procedure, ask who will coordinate care with your regular clinician and what follow-up the team provides.

Check costs and access before committing
Ask what the treatment, visits, and follow-up may cost, what your insurance might cover, and whom to contact to verify benefits. Coverage and reimbursement vary, so check directly with your insurer rather than assuming a service or medicine will be covered. If a program, specialist, or procedure is not accessible, ask whether there is another suitable option or referral.
