Before deciding whether to remove an endometrioma (an ovarian endometriotic cyst), ask your doctor to connect the recommendation to your symptoms, fertility plans, ovarian reserve, previous treatment and acceptable trade-offs. A cyst’s size or an endometriosis stage does not, by itself, determine the right choice. Use the questions below to make the consultation a shared decision rather than a simple yes-or-no discussion about surgery.
This is an appointment guide, not a recommendation that you personally should have an operation. Bring your imaging and treatment history, and ask for a fertility-specialist opinion when future pregnancy is a priority.

First establish why surgery is being considered
Ask the clinician to identify the specific problem surgery is intended to address. Endometriomas can be associated with pain, infertility and reduced ovarian reserve, but the balance between those concerns differs from person to person. NICE says decisions should start with symptoms, preferences and priorities for pain and fertility. Its recommendation 1.9.1 states: “Ask women with suspected or confirmed endometriosis about their symptoms, preferences and priorities with respect to pain and fertility, to guide surgical decision-making.” See the NICE endometriosis recommendations.

- What is the main reason you recommend surgery in my case: pain, the cyst’s appearance, fertility considerations or another concern?
- What improvement in my symptoms do you expect, and how certain is that benefit?
- What are the reasonable alternatives, including monitoring or treatment without surgery?
- How would my priorities about pain, preserving fertility and the timing of a possible pregnancy change your recommendation?
- What would be the consequences of waiting, and what finding would make you change the plan?
Ask the doctor to explain uncertainty plainly. Guidelines identify factors that should be considered, but they do not select one operation for every patient.

Compare the operation with realistic alternatives
If surgery is proposed, ask for the exact technique and a comparison with non-operative or fertility-focused options. NICE recommends considering excision rather than ablation for endometriomas while taking fertility desire and ovarian reserve into account. For people whose immediate priority is fertility, the guideline notes that drainage and ablation may preserve ovarian reserve more than cystectomy. ESHRE recommends cystectomy rather than drainage and electrocoagulation for recurrence and pain prevention when surgery is indicated, again with attention to ovarian-reserve risk. These recommendations describe trade-offs, not a universal winner.
Questions about technique
- Which exact procedure do you propose: cystectomy or excision of the cyst wall, drainage with ablation, or another approach?
- Why is this technique preferable for my cyst, symptoms, ovarian reserve and pregnancy plans?
- How do the options compare for expected pain relief, recurrence and the chance of needing repeat surgery?
- How could each option affect ovarian reserve, spontaneous conception and assisted reproduction?
- What complications are more likely with each approach, and how would recovery differ?
- If the findings during laparoscopy differ from the scan, what decisions could be made during the operation, and will you seek my consent for those possibilities?
| Option discussed | Potential reason to consider it | Questions to resolve before choosing |
|---|---|---|
| Cystectomy or excision | ESHRE favors cystectomy over drainage and electrocoagulation when surgery is indicated for recurrence and pain prevention. | How much ovarian tissue might be affected, and why does the expected benefit outweigh the reserve risk for me? |
| Drainage and ablation | NICE notes that, in fertility-priority decisions, drainage and ablation may preserve ovarian reserve more than cystectomy. | How would recurrence and pain outcomes compare with cystectomy in my situation, and what evidence supports the proposed ablation method? |
| Monitoring or treatment without surgery | May be reasonable when symptoms, fertility timing and imaging findings do not make immediate surgery necessary. | What follow-up imaging and symptom changes would trigger a new decision? |
The table summarizes guideline considerations; your surgeon must explain what applies to your anatomy and goals.
Ask specifically about ovarian reserve and fertility
Both an endometrioma and surgery can affect ovarian reserve. ESHRE reports that severe endometriosis, particularly bilateral endometriomas, is associated with a higher risk of premature ovarian insufficiency and lower AMH, and that surgery can further affect reserve and AMH. A test result cannot predict an individual’s ability to become pregnant.

Questions about your personal fertility outlook
- How might the endometrioma itself, and the proposed operation, affect my ovarian reserve and future fertility?
- Does my age, previous ovarian surgery, bilateral disease or another fertility factor change the balance between operating and waiting?
- What can AMH or other ovarian-reserve tests tell us in my case, and what can they not predict about pregnancy?
- Should I speak with a fertility specialist before deciding?
- Would fertility preservation be relevant to my circumstances, and what remains uncertain about its possible benefit?
- If I am trying to conceive now, how does surgery compare with fertility treatment or proceeding without surgery for me?
- How might the plan differ if I am not ready to try for pregnancy for several years?
ESHRE says the decision should also account for previous surgery, other infertility factors and the estimated endometriosis fertility index. Bring records of prior procedures, pregnancies, fertility treatments, medications and test results so those factors can be considered together.
Find out whether disease extends beyond the ovary
An endometrioma may coexist with endometriosis involving other pelvic structures. Ask what your scans suggest and whether the operating team can manage any unexpected findings.
Questions about imaging and the team
- Do my ultrasound or MRI findings suggest disease involving the bowel, bladder or ureter?
- Would a specialist endometriosis service or another surgical specialist need to be involved?
- How often do you perform endometrioma and complex endometriosis surgery?
- Who would make decisions if disease is found outside the ovary?
- Will the procedure be laparoscopic, and what circumstances could require a different approach?
Ask for the limits of the imaging as well as its findings. A scan can guide planning but may not show the full extent of endometriosis.

Understand complications, recovery and the possibility of further surgery
Ask for a personalized explanation of surgical risks, including effects on ovarian function and fertility. NICE advises discussing what laparoscopy involves, possible symptom effects, benefits and risks, and the possibility of further surgery for recurrence or complications.
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Fix the driver behind crashes, sound loss and screen glitches3Repair Windows errors before they cause bigger problemsQuestions to ask before consenting
- What complications are possible during or after the operation?
- Could bleeding, infection, injury to nearby organs or loss of ovarian tissue affect my recovery or fertility?
- What symptoms or findings would mean I need another procedure?
- How long should I expect to be in hospital, and when can I return to work, exercise and sexual activity?
- What pain control and wound-care plan will I receive?
- Which warning signs after discharge require an urgent call to the surgical team or emergency care?
- When will pathology results and the postoperative review occur?
Ask the surgeon to separate common short-term effects from uncommon but serious complications and to explain how your own prior operations or medical conditions alter those risks.
Plan what happens if you defer surgery
Choosing monitoring is an active plan, not an absence of care. NICE recommends considering follow-up for confirmed endometriosis, particularly when an endometrioma is larger than 3 cm, including for people who choose not to have surgery. The “larger than 3 cm” figure is a follow-up consideration in that guideline, not a universal threshold that means surgery is required.

Questions for a monitoring plan
- What imaging schedule do you recommend, and which type of scan will be used?
- What change in cyst appearance, size or symptoms would prompt reassessment?
- How will we track pain, bleeding, medication effects and impact on daily life?
- Should I have a fertility assessment while monitoring?
- Who should I contact if my symptoms change between appointments?
Ask for the plan in writing, including the next appointment date and the circumstances that should bring it forward.

ESHRE discusses postoperative hormonal treatment for people who cannot or do not plan to attempt conception immediately. Ask whether hormonal suppression fits your goals, which symptoms it is intended to control and how it affects your timeline for trying to conceive.

- If I am not trying to conceive immediately, should I consider hormonal treatment to reduce pain or recurrence?
- Which treatment is suitable with my medical history, and what side effects should I expect?
- When would treatment start, how long would it continue and how would we stop it when I want to try for pregnancy?
- If I decline hormonal treatment, what follow-up will replace it?
Prepare for a more useful consultation
- Write down your priorities. Rank pain relief, preserving ovarian reserve, trying to conceive soon, avoiding repeat surgery and minimizing recovery time.
- Bring your records. Include imaging reports and images, operative notes, pathology, medication lists, previous ovarian surgery and fertility-treatment records.
- Record symptoms. Note pain location and timing, bleeding, bowel or bladder symptoms, missed activities and treatments that helped or failed.
- Ask for numbers with context. If the clinician quotes a risk or test value, ask what population, procedure and timeframe it refers to and whether it applies to your situation.
- Bring support and take notes. A trusted person can help remember options and follow-up instructions; ask whether the clinic allows recording.
- Request a written plan. It should state the proposed operation, alternatives, fertility referral, recovery expectations and follow-up.
Use a decision framework rather than a single test or measurement
When two or more options remain reasonable, compare them on the factors identified by NICE and ESHRE:
Best Value
- Expected effect on pain and daily functioning.
- Likelihood of recurrence or repeat surgery.
- Possible effect on ovarian reserve and AMH.
- Implications for spontaneous conception and assisted reproduction.
- Risk of surgical complications and recovery burden.
- Follow-up requirements.
- Fit with your age, previous surgery, other fertility factors and timing for pregnancy.
Ask the doctor to explain which factor is driving the recommendation and which uncertainties could change it. No population-level percentage for an individual’s fertility, recurrence or ovarian-reserve loss is established here, so be cautious about applying a statistic from another patient or study to yourself.
Reliable guideline sources
For the recommendations on shared decision-making, follow-up and fertility-related choices, read NICE, Endometriosis: diagnosis and management (NG73). For surgical technique, recurrence and ovarian-reserve considerations, see the 2022 ESHRE guideline on endometriosis and its fertility-preservation section in the ESHRE guideline PDF. Guideline wording and local care pathways can change, so confirm that your clinician is using the current version for your location.
Bottom line for your appointment
Leave the consultation knowing why surgery is being proposed, what alternatives exist, how each technique may affect pain, recurrence and ovarian reserve, and what the plan is for fertility and follow-up. A fertility specialist or multidisciplinary endometriosis team can add important input when pregnancy is a priority, especially after previous ovarian surgery or with bilateral endometriomas.
