Questions to Ask Your Doctor About an Incidental Brain Aneurysm

An incidental brain aneurysm is an aneurysm seen on imaging performed for another reason. It is not automatically an emergency, harmless, or a reason for immediate treatment. Take the scan report and images to a clinician and ask about the finding’s certainty, size, location, shape, estimated rupture risk, monitoring plan, treatment options, risk-factor changes, family history, and warning symptoms. The answers depend on your images and medical history; no general article can calculate your personal risk.

Start by separating “incidental,” “unruptured,” and “ruptured”

“Incidental” describes how the finding was discovered: imaging was ordered for another reason and showed a possible or confirmed aneurysm. “Unruptured” describes its current state. A ruptured aneurysm is a different emergency condition. An incidental finding therefore needs timely clinical interpretation, but the word itself does not establish that rupture is occurring or that an operation is required.

Bring the actual radiology report and, if the imaging center can provide them, the image files or a link to the image viewer. A report may describe a suspected aneurysm that needs confirmation, or it may identify a definite aneurysm. Your doctor must interpret the images in context rather than relying on a general size rule from the internet.

Prepare for the appointment

  • Write down why the original scan was ordered, when it was performed, and whether you have had earlier brain or blood-vessel imaging for comparison.
  • Bring a complete medication list, including prescriptions, over-the-counter products, supplements, nicotine, alcohol, and recreational drugs. Include the dose and how often you use each item.
  • Record blood-pressure readings if you have them, your smoking history, and any previous diagnosis that your clinician may consider relevant.
  • List relatives who have had a brain aneurysm or aneurysmal subarachnoid hemorrhage, including the relationship and approximate age at the event.
  • Take a support person or use a notes app so you can record the answers and the agreed follow-up date.

Questions about what the scan actually found

1. “What exactly did the scan find?”

Ask whether the finding is definitely an aneurysm or only a possibility, and whether another type of blood-vessel abnormality or an imaging artifact could explain it. Ask the clinician to state the measured size, the vessel and location, the shape, and any other imaging features that affect management. Mayo Clinic’s patient guidance specifically directs patients to ask about an aneurysm’s size, location, and appearance.

Ask which test produced the finding and whether a dedicated vessel study is needed. The AHA/ASA professional summary identifies CT angiography and MR angiography as sensitive approaches for detecting and following intracranial aneurysms. The appropriate test depends on your circumstances, prior images, kidney function, radiation considerations, implanted devices, and the quality of the existing study.

Request a copy of the written report and the images. Ask whether the clinician can show you the area on the scan and explain terms such as “saccular,” “fusiform,” “irregular,” “daughter sac,” or “mural change” if they appear in the report. Do not assume that a word describing shape has the same significance for every aneurysm; ask how it applies to yours.

Questions about your estimated rupture risk

2. “How likely is my aneurysm to rupture?”

This is a central question, but an answer requires your images and history. Ask which features drive the estimate and whether the estimate is qualitative or based on a validated risk tool. Clinicians may consider size, location, shape, documented growth or shape change, age, blood pressure, smoking, general health, family history, and relevant medical conditions. Ask which of those factors apply to you and which are uncertain.

Ask whether prior or future imaging could show growth. A change in size or shape can alter management, so ask what degree of change would be considered meaningful and how it would be confirmed. Also ask whether the aneurysm’s location carries particular technical or natural-history considerations.

There is no single rupture percentage that can be responsibly applied to every incidental aneurysm. Mayo Clinic reports that about 25% of people who experience a ruptured aneurysm die within 24 hours and another 25% have complications that lead to death within six months (Mayo Clinic, 2026). Those figures describe outcomes after rupture; they are not the probability that your unruptured aneurysm will rupture.

Questions about what to do now

3. “What do you recommend at this time, and why?”

Ask your clinician to compare the expected risk of leaving the aneurysm untreated with the potential benefit and harms of an intervention in your particular case. Some unruptured aneurysms are monitored rather than treated immediately. Possible procedures include surgical clipping, endovascular coiling, and flow diversion. They are not interchangeable choices that a patient can select from a menu: suitability depends on the aneurysm’s anatomy, your health, the team’s expertise, and your preferences.

For each option under consideration, ask:

  • What benefit is expected, and what problem is the procedure intended to prevent?
  • What short-term complications and longer-term consequences are possible?
  • How do the aneurysm’s size, location, neck, shape, and vessel anatomy affect technical feasibility?
  • What would recovery involve, including hospital time, activity limits, driving, work, and follow-up imaging?
  • Could additional procedures or long-term medicines be needed?
  • What is the team’s experience with this type of aneurysm, and would another opinion change the decision?

Ask the doctor to explain why observation is safer than intervention if monitoring is recommended, or why intervention is favored if a procedure is proposed. The decision balances two kinds of risk: the risk associated with the aneurysm over time and the risk created by treatment.

Questions about a monitoring plan

4. “If we monitor it, what is the plan?”

Do not leave the appointment with only “come back later.” Ask which imaging method will be used, when the next scan should occur, who will review it, and how you will receive the result. Ask what finding would change the plan, such as growth, a new shape feature, a symptom, or a change in your health.

Mayo Clinic’s patient guidance phrases the issue this way: “If treatment is not indicated currently, how often will I need to have follow-up scans for the aneurysm?” That is a useful prompt, but the sources do not establish one interval suitable for every person. Your clinician should set the timing based on the aneurysm and your circumstances. Put the date or time window in writing and confirm who is responsible for ordering and communicating the study.

Ask whether older images will be compared using the same technique and whether a difference could reflect measurement variation rather than true growth. Keep copies of each report so future clinicians can see the sequence of findings.

Questions about blood pressure, smoking, drugs, and medicines

5. “What should I do about risk factors or medicines?”

Ask for a specific blood-pressure goal and a plan for measuring it. If you smoke or use nicotine, ask about a supported cessation plan rather than trying to stop without help. NINDS patient information also advises addressing use of drugs such as cocaine. Tell the clinician about alcohol and any other substances so advice is based on complete information.

Give the team your full medication list, including medicines that affect blood pressure, clotting, or bleeding. Do not stop, start, or change a prescribed medicine on your own. Ask the clinician who manages that medicine whether an adjustment is needed and how it should be coordinated with aneurysm care.

Ask which lifestyle measures are helpful for your overall vascular health and which claims found online are unsupported. The goal is a practical plan you can follow, with a named clinician for questions and a date to review progress.

Questions about family history and screening

6. “Does my family history change the assessment?”

Tell your clinician about relatives with intracranial aneurysms or aneurysmal subarachnoid hemorrhage. Ask whether the number of affected relatives, their degree of relationship, and the ages at diagnosis change your assessment or justify discussing screening with family members.

The AHA/ASA summary states that screening should be offered to patients with two or more family members with intracranial aneurysm or subarachnoid hemorrhage. That is a specific guideline statement, not blanket advice for every relative of one affected person. Ask whether CT angiography or MR angiography is appropriate for your family situation and how the benefits, limitations, contrast, radiation, and possible incidental findings would be handled.

Questions about specialists and second opinions

7. “Who should be involved in my care?”

Ask whether your case should be reviewed by a neurologist, neurosurgeon, or interventional neuroradiologist. Mayo Clinic identifies these specialties as part of the decision process. You can ask whether the clinicians routinely manage aneurysms like yours and whether a multidisciplinary conference is available.

The AHA/ASA guidance says treatment should be performed at high-volume centers with experience in aneurysm treatment. If the decision is difficult, ask whether a second opinion at such a center would add useful expertise and whether your images can be sent electronically for review. A second opinion is a way to clarify a decision, not proof that the first clinician was wrong.

Ask what symptoms require emergency help

8. “What symptoms mean I should seek emergency care?”

Ask your clinician for individualized written instructions and the correct emergency number for your location. The patient passages used for this guide do not provide a complete, citable emergency-symptom checklist, so do not rely on this article to triage sudden or severe new symptoms. If you develop an abrupt, severe, or otherwise alarming neurological symptom, seek emergency evaluation rather than waiting for a scheduled scan or sending a routine message to the clinic.

Ask what to do outside office hours, who can answer urgent questions, and whether you should tell emergency clinicians that an aneurysm has been identified. Keep the report and medication list accessible.

How to compare observation and procedures with your care team

Use the same questions for every option so the comparison is meaningful. The following framework reflects the factors described in Mayo Clinic and AHA/ASA guidance; it is not a ranking of treatments.

Decision factor What to ask
Risk if untreated What is my patient-specific rupture risk, and which features make it higher or lower?
Procedure benefit and harm What benefit is expected, and what complications can occur from clipping, coiling, or flow diversion?
Anatomical suitability How do size, location, shape, neck, and vessel anatomy affect each option?
Recovery and follow-up What hospital stay, recovery, medicines, and imaging will each option require?
Team experience How often does this center treat aneurysms with similar anatomy, and is it a high-volume program?
Your context How do my age, health, family history, work, preferences, and tolerance for uncertainty affect the choice?

Leave with a written plan

Before you leave, ask the clinician to write down the working diagnosis, the next test or appointment, the responsible office, and the symptoms that require urgent help. Confirm whether you need a blood-pressure follow-up, smoking-cessation support, medication review, specialist referral, or family-history assessment. If the finding is being observed, record the planned imaging method and time window rather than trying to remember it.

Bring your questions to every visit. An incidental aneurysm is a finding that deserves careful interpretation and follow-up, not an automatic prediction of rupture. The safest next step is a documented, individualized discussion with the clinicians who can review your images and medical history.
