A PSMA PET scan is a diagnostic imaging test, not a treatment. It uses a small injected radiotracer to show prostate-specific membrane antigen (PSMA) activity, while a CT scan maps that activity to anatomy. The meaningful risks are radiation from both parts of the examination, tracer-specific reactions, and the possibility of an inaccurate or uncertain result. Published dose figures are reference values—not your personal dose or a precise prediction of cancer risk—so ask the imaging team which tracer, injected activity and CT protocol are planned.
What a PSMA PET scan does—and what it does not do
PSMA PET combines two examinations in one visit. A radiotracer that binds to PSMA is injected into a vein. PET detects where the tracer accumulates; CT supplies anatomical detail so the interpreting physician can locate that uptake. The scan may reveal disease in the prostate bed, lymph nodes, bones or other tissues, depending on the clinical situation.

This is diagnostic imaging. It is different from PSMA-directed radioligand therapy, in which a radioactive compound is administered with the intention of delivering treatment to PSMA-bearing cancer cells. A diagnostic scan’s tracer activity and radiation exposure must not be equated with treatment exposure: treatment uses different agents, activities and clinical goals.

Common uses include initial staging when a man is considered at risk for metastatic prostate cancer and evaluation of suspected persistent or recurrent disease after treatment, particularly when PSA rises. The best test depends on the question your team needs answered, your prior treatment and other clinical information.

SNMMI patient guidance says approved PSMA tracers generally perform similarly for detecting prostate cancer, while also noting that detection can be less efficient in some circumstances and that another modality may be preferable. Depending on the case, alternatives can include MRI, CT, bone scintigraphy or PET using another tracer. A PSMA PET scan is not automatically the right examination for every patient.
What the appointment feels like
- IV placement: A technologist places an intravenous line, usually in an arm vein. Tell the team about difficult veins, prior reactions to injections or known allergies before the tracer is given.
- Tracer injection: A small amount of the selected radiotracer is injected. The agent and administered activity vary by center and indication.
- Quiet uptake period: You generally wait about an hour while the tracer circulates. The exact waiting time depends on the agent and local protocol.
- Scanning: You lie on the scanner table for approximately 30 minutes while PET and CT images are acquired. Remaining still helps reduce image blurring. Actual timing can be longer or shorter if additional images or a different protocol are needed.
- Interpretation: A nuclear medicine physician or radiologist reviews tracer distribution, CT findings and your clinical information. The report is not interpreted from tracer uptake alone.
The CT portion may be performed only for attenuation correction and anatomical correlation, or it may be a more detailed diagnostic CT. Ask which type is planned and whether intravenous contrast is included; those choices affect both what the images show and the X-ray dose.
Where the radiation comes from
Internal exposure from the radiotracer
The injected radionuclide emits radiation from inside the body as it decays. The effective dose depends on the tracer, the amount administered and how the agent distributes and clears. The 2023 joint EANM/SNMMI guideline lists effective-dose coefficients from 0.0116 to 0.022 mSv per MBq across the PSMA ligands it evaluates. Those coefficients cannot be converted into your dose without knowing the actual agent and activity.

External exposure from CT
CT adds X-ray radiation. The same guideline gives a broad reference range of 1–20 mSv for the CT component, depending on the protocol. A low-dose CT used mainly for attenuation correction and localization commonly differs from a diagnostic CT designed to characterize anatomy in greater detail. Scanner hardware, settings, body region and whether contrast-enhanced images are obtained all influence the result.
Because the CT dose is protocol-specific, adding a single “typical PSMA PET dose” to every patient is misleading. The imaging service can provide the dose-length product or an estimated effective dose for its protocol if you want a more individualized explanation.

How to think about cancer risk from the radiation
Regulators describe cumulative exposure to ionizing radiation as being associated with an increased lifetime cancer risk. That statement describes a population-level concern; it does not establish a precise added-risk percentage for one person after one PSMA PET/CT. The sources reviewed for this article do not provide a validated individual probability, and a population-average estimate should not be presented as your personal risk.
The clinical reason for imaging matters. A scan may answer whether disease has spread or returned and can change treatment planning. Discuss the expected benefit and the radiation exposure with the ordering clinician, especially if you have had many prior CT or nuclear-medicine studies. Do not decline a clinically indicated CT solely because it is part of a PET examination without first discussing alternatives with your team.

These findings are specific to Pylarify in that FDA summary. They should not be copied as a universal side-effect list for gallium-68 PSMA-11, PSMA-I&T, PSMA-1007, rhPSMA-7.3 or another agent. Ask which product is being used and request its current local labeling. Tell staff about a history of medication or food allergy before injection, and report any new symptom during or after the visit according to the instructions from your imaging service.
Injection and positioning issues
An IV can cause brief discomfort, bruising or, rarely, leakage around the injection site. Lying still for the scan may be uncomfortable for people with pain, claustrophobia or limited mobility. Tell the technologist before scanning so positioning or support can be adjusted. These practical issues are separate from the radiation dose.
Why a PSMA PET scan can be wrong or inconclusive
Normal uptake is not cancer
PSMA expression and tracer distribution are not exclusive to prostate tumors. Normal uptake can be seen in the kidneys, bladder, salivary glands, small bowel and spleen. A bright focus in one of these areas may be physiologic rather than malignant.
Best Value
False-positive results
PSMA binding can occur in certain other cancers and in some non-malignant conditions. Consequently, an area of uptake is not proof of prostate cancer. The CT appearance, location, PSA pattern, prior treatment and other clinical findings help determine whether a focus is suspicious and whether another test or follow-up is needed.
False-negative results
A scan can miss disease. SNMMI patient information notes that tumors smaller than about 0.5 cm may not be visible. Tumor biology, low PSMA expression, image quality and the timing of imaging can also affect detection. A negative scan therefore does not guarantee that no cancer is present when other evidence suggests otherwise.
Questions to ask before the scan
- Which tracer will you use? Ask for the agent’s name, because dosimetry, labeling and known adverse reactions are tracer-specific.
- What activity will be injected? The administered MBq is needed to interpret any radiopharmaceutical dose estimate.
- What is the CT for? Clarify whether it is low-dose attenuation-correction CT or diagnostic CT, and whether contrast is planned.
- What clinical question is being investigated? Ask how the result could affect staging, recurrence assessment or treatment planning.
- What alternatives would answer the same question? Depending on your case, MRI, CT, bone scintigraphy or another PET tracer may be considered.
- How will equivocal findings be handled? Ask whether PSA trends, prior scans, biopsy or follow-up imaging may be needed to clarify an indeterminate focus.
- Who should I contact about a reaction? Obtain the imaging service’s instructions before leaving.
Interpreting the report with your care team
The report should be read alongside your PSA history, biopsy information, treatment history, physical findings and prior imaging. A positive finding may require correlation or another test; a negative result may not exclude very small or biologically different lesions. Ask the interpreting physician or oncologist what each reported focus means for your particular clinical question rather than treating the scan as a stand-alone diagnosis.
Do these 3 things before closing this tab:
1Repair Windows errors before they cause bigger problems2Scan for outdated or missing drivers - takes under a minute3Clear out junk files and repair common Windows errorsThe joint EANM/SNMMI guideline emphasizes that the final decision about the propriety of a procedure belongs to the nuclear medicine physician or medical physicist in light of all the circumstances. In practical terms, the team should balance the expected diagnostic benefit, the tracer and CT exposure, your prior imaging and the alternatives available at that center.
The practical bottom line for patients and caregivers
A PSMA PET/CT usually involves an IV tracer injection, about an hour of circulation time and roughly 30 minutes of scanning. Its radiation comes from two sources: the tracer inside the body and the CT’s X-rays. Published tracer examples range from 3.4 to 6.6 mSv at specified activities, while CT exposure may range from 1 to 20 mSv depending on protocol. Those numbers are reference values, not your personal dose.
Side effects depend on the agent; headache, altered taste, fatigue and hypersensitivity warnings are documented for Pylarify, not automatically for every PSMA tracer. Normal uptake, false positives, false negatives and lesions below about 0.5 cm limit certainty. Ask your imaging team to identify the tracer, injected activity, CT protocol and reason for the examination before you proceed.
