Find and replace yours safely.Free scan · under a minuteFor most men who can safely take them, prescription PDE5 inhibitor pills are the usual first treatment for erectile dysfunction (ED). They are not automatically “better” than a vacuum erection device, injections, or counseling: the right choice depends on medical contraindications, the cause of ED, how much spontaneity you want, your tolerance for devices or needles, cost, and your and your partner’s preferences.
What each ED treatment does
ED can result from vascular disease, diabetes, obesity, medication effects, smoking, alcohol use, hormonal problems, anxiety, depression, relationship strain, or a combination of physical and psychological factors. Because ED can also be a marker of cardiovascular disease, treatment should follow a medical evaluation rather than self-treatment.

PDE5 inhibitor pills
Prescription phosphodiesterase type 5 (PDE5) inhibitors increase the likelihood of an erection when sexual stimulation is present. U.S. examples include sildenafil, tadalafil, vardenafil, and avanafil. The American Urological Association (AUA) and the 2024 international consensus describe this class as first-line treatment for most eligible men.

Pills do not create an automatic erection: sexual stimulation is required. An apparently failed treatment may reflect incorrect timing or use, an unsuitable dose, an unrecognized medical cause, or an expectation that the medicine will work without arousal. A clinician may need to review how the medicine is being used and adjust the dose.

Vacuum erection devices
A vacuum erection device (VED) is a non-invasive, drug-free system. A cylinder is placed over the penis, air is removed to draw blood into the erectile tissue, and a constriction ring helps maintain the erection for intercourse. The AUA says the device should include a vacuum limiter to reduce the risk of excessive negative pressure.
Intracavernosal injections
Intracavernosal injection (ICI) therapy places medicine directly into the erectile tissue. Alprostadil may be used alone; compounded combinations are also used in practice. Because the drug acts locally, ICI can work when oral medicines are contraindicated, ineffective, or unwanted, but it requires instruction, dose titration, and comfort with a needle.
Counseling and cognitive behavioral therapy
Counseling or cognitive behavioral therapy (CBT) addresses anxiety, stress, depression, performance concerns, communication problems, and relationship factors that can interfere with erections. It is a medical treatment component, not a suggestion that symptoms are imaginary. It may be used alone when psychological causes predominate or alongside pills, a VED, or injections when causes are mixed.
Quick comparison
| Option | Main advantages | Main burdens and risks | Often fits men who |

Common VED problems include transient petechiae (small spots caused by bleeding under the skin), bruising, discomfort or pain, numbness, difficulty ejaculating, and difficulty using the mechanism. A constriction ring should remain in place only for the period specified in the device instructions. Men taking anticoagulants, people with bleeding disorders, and anyone with a history of priapism should ask a clinician before using a VED.
Injection results
Across ICI studies summarized by the AUA, the percentage of men achieving successful intercourse ranged from 53.7% to 100%. The EAU reports efficacy above 70% for intracavernosal alprostadil in general ED populations and approximately 1% priapism in the cited evidence. Dropout over time is substantial in some studies, often reflecting the burden of injections, discomfort, or changing preferences.
Alprostadil and compounded combinations are not interchangeable from a regulatory standpoint. In the United States, some non-alprostadil combinations may not be formally FDA-approved for ED, so the prescriber should explain what is being used and why.
When pills are the best starting point
Ask a clinician about a PDE5 inhibitor when you want the least complicated treatment that can be taken by mouth and you have no contraindication. The AUA recommends that men with ED be informed about an FDA-approved oral PDE5 inhibitor, including its benefits, risks, and burdens, unless it is contraindicated.

What to discuss before taking one
- All prescription medicines, over-the-counter products, and recreational substances, because interactions and contraindications must be screened.
- Heart and blood-vessel health, prior cardiovascular symptoms, diabetes, blood-pressure problems, and other conditions that may affect sexual activity or treatment safety.
- How and when you took any previous dose, whether sexual stimulation was present, and whether the dose was adequate.
- Side effects, acceptable timing, and whether a different PDE5 inhibitor or dose adjustment should be considered.
Do not buy prescription ED medicines from an unverified seller or change the dose on your own. A clinician can determine whether an apparent failure reflects technique, dose, a medical cause, or the need for another treatment.
When a vacuum pump makes more sense
A VED is a reasonable alternative when you want to avoid systemic medicine, cannot use oral drugs, or prefer a drug-free method. It may also suit couples who are comfortable adding a short mechanical setup to sexual activity.
How to choose and use one safely
- Ask a clinician whether a VED is appropriate, particularly if you take anticoagulants, have a bleeding disorder, or have had priapism.
- Choose a device with an integrated vacuum limiter, as recommended by the AUA.
- Read the manufacturer’s instructions for cylinder placement, pressure, lubrication, and ring use; do not exceed the stated pressure.
- Stop if you develop significant pain, marked discoloration, skin injury, or persistent numbness, and seek medical advice if symptoms do not promptly resolve.
- Remove the constriction ring within the time stated in the instructions. Never leave it on longer to try to prolong an erection.
A pump does not treat the underlying cause of ED. It creates an erection mechanically, so the decision is about reliability, comfort, safety, and convenience rather than curing the disease.
When injections are appropriate
ICI is generally considered after oral treatment is unsuitable, ineffective despite correct use, or unwanted. It can be a strong option for men who value a predictable, locally acting erection and are willing to learn the technique.

What training involves
- Attend an in-office injection test so a clinician can select an initial medicine and dose and observe the response.
- Learn sterile preparation, injection location, needle handling, and safe disposal from the prescribing team.
- Use only the prescribed dose. Do not repeat an injection or increase the amount because an erection seems slower than expected.
- Keep a written record of dose, response, pain, and duration so the prescriber can titrate treatment safely.
- Follow the emergency plan supplied by the clinic. An erection lasting four hours or longer is priapism and requires emergency care.
Injection-specific risks
Penile pain, fibrosis or plaque, and priapism are the principal risks discussed in guidelines. Repeated injury or inappropriate dosing can increase complications. A clinician should explain whether the chosen preparation is FDA-approved for ED in your jurisdiction and how it should be stored and transported.
Where counseling and CBT fit
The National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK) says a health professional may suggest counseling when mental-health or emotional issues affect ED. The EAU considers combining CBT with pharmacological treatment empirical and regards that integrated approach as best practice when psychological factors are involved.
Signs that psychological treatment may help
- Erections occur during sleep, masturbation, or unexpectedly but not reliably with a partner.
- Symptoms began after a stressful event, relationship conflict, depression, anxiety, or a major life change.
- You experience intense performance worry, repeated checking, avoidance, or shame that makes arousal harder.
- Medical treatment helps physically but fear, communication problems, or relationship tension still limits sex.
These patterns do not prove that ED is purely psychological. Physical and emotional causes commonly overlap. A qualified therapist can work on anxiety and communication while a medical clinician evaluates vascular, hormonal, medication-related, and other contributors.

What evaluation should happen first?
The AUA recommends a thorough medical, sexual, and psychosocial history, a physical examination, selective laboratory testing, and measurement of morning total testosterone. NIDDK describes history, examination, blood tests, ultrasound or other tests when indicated, and an office injection test in appropriate cases.

Prepare for the appointment
- List every medicine and supplement, including how often you use it.
- Note when ED started, whether it is constant or situational, and whether erections occur during sleep or masturbation.
- Describe libido, ejaculation, orgasm, pain, penile curvature, urinary symptoms, and any prior pelvic or prostate procedures.
- Bring information about diabetes, blood pressure, cholesterol, heart disease, smoking, alcohol, sleep, mood, and relationship stress.
- Write down what you have already tried, the dose and timing, the presence or absence of sexual stimulation, and the result.
The assessment helps determine whether sexual activity and a proposed treatment are safe and whether an underlying condition needs attention before choosing a device, medicine, injection, or counseling plan.
How to decide between the options
Choose pills first when
- You want an oral, relatively spontaneous option.
- A clinician confirms that no contraindication or interaction makes a PDE5 inhibitor unsafe.
- You are comfortable with the requirement for sexual stimulation and possible dose adjustment.
Choose a VED when
- You prefer to avoid systemic medication or cannot take it.
- You accept mechanical preparation and possible bruising, numbness, or ejaculation difficulty.
- You can obtain a device with a vacuum limiter and use it according to instructions.
Choose ICI when
- Pills are contraindicated, ineffective after correct use, or not acceptable.
- You value a locally acting and often dependable erection enough to accept injections and training.
- You can attend dose instruction and follow the emergency plan for prolonged erections.
Add counseling or CBT when
- Anxiety, depression, stress, relationship strain, or performance concerns contribute to ED.
- You have mixed physical and psychological causes and want better results from medical treatment.
- You and your partner need help discussing expectations, timing, fear, or intimacy.
Common reasons treatment appears to fail
The pill did not work
Check whether the medicine was used as prescribed, whether sexual stimulation occurred, whether the timing was appropriate for that prescription, and whether the dose was reviewed by a clinician. Do not take extra doses without medical direction. Persistent nonresponse warrants reassessment for vascular disease, diabetes, hormone problems, medication effects, or psychological contributors.
The pump is uncomfortable or ineffective
Incorrect cylinder fit, excessive pressure, an unsuitable ring, or rushed setup can reduce comfort and results. Recheck the instructions and ask the clinician or manufacturer about fit. Stop for pain, skin injury, or persistent numbness rather than increasing pressure.

Contact the prescribing clinic for pain, repeated bruising, curvature, or a change in response. An erection lasting four hours or longer is an emergency; do not wait for it to resolve on its own.
Counseling feels irrelevant
ED can have both physical and psychological components. If counseling is not addressing the concerns you have, tell the therapist and medical clinician so the treatment plan can be broadened rather than abandoning evaluation.
Safety checklist
- Have a clinician review oral-drug contraindications and medication interactions before starting a PDE5 inhibitor.
- Use only a VED with a vacuum limiter and obtain medical advice first if you have bleeding risks or prior priapism.
- Receive in-office ICI testing and dose instruction; never improvise the dose.
- Treat an erection lasting four hours or longer as an emergency.
- Include counseling when emotional or relationship factors are present; it is a legitimate part of ED care.
- Seek assessment for cardiovascular and metabolic risk instead of treating ED as an isolated inconvenience.
Bottom line for choosing treatment
No option is universally best. Pills are the guideline-supported first discussion for most eligible men, a VED is the leading non-drug alternative, ICI can provide a reliable next step when pills are unsuitable or fail, and counseling or CBT is essential when psychological or relationship factors contribute. Shared decision-making—balancing safety, effectiveness, burden, cost, timing, and preference—produces a better choice than ranking the treatments for everyone.
