Probiotics are live microorganisms; prebiotics are substrates—usually particular fibers—that your resident microorganisms selectively use. A probiotic is defined as a live microorganism that confers a health benefit when taken in an adequate amount. A prebiotic is “a substrate that is selectively utilized by host microorganisms conferring a health benefit,” according to the International Scientific Association for Probiotics and Prebiotics (ISAPP). Synbiotics combine a microorganism and a substrate.
Neither category is automatically required for a healthy person. The U.S. National Institutes of Health (NIH) Office of Dietary Supplements (ODS) says there are no official recommendations for probiotic use by healthy people. Your goal, the specific strain or fiber, your tolerance, and your medical circumstances matter more than a broad “gut health” claim.

Prebiotics and probiotics at a glance
| Question | Probiotic | Prebiotic |
|---|---|---|
| What is it? | A live microorganism that provides a demonstrated health benefit when administered in an adequate amount. | A substrate, often a particular type of fiber, selectively used by host microorganisms and linked to a health benefit. |
| What does it do? | Adds or supplies a specific microorganism. | Provides material that selected microorganisms can use. |
| What determines whether it works? | The exact genus, species, strain, dose, product quality, condition and person. | The exact fiber or substrate, amount, resident microbes, outcome studied and person. |
| Common food forms | Some yogurts and other fermented foods, provided they contain a documented beneficial microorganism. | Foods containing fermentable fibers, including onions, garlic, leeks, bananas, wheat, chicory root and artichokes. |
| What should a supplement label show? | Genus, species, strain, colony-forming units (CFUs), expiration or use-by date and storage instructions. | Fiber or substrate type and the amount per serving. |
| Is a supplement generally required? | No official recommendation exists for routine use by healthy people. | No universal requirement; food and clinical purpose should guide a decision. |
The most important practical point is that “probiotic” does not mean “any fermented food,” and “prebiotic” does not mean “any fiber.” A product must meet the scientific definition and have evidence for the benefit it claims.

Probiotics are specific live microorganisms
ODS, citing the ISAPP consensus definition, describes probiotics as “live microorganisms that, when administered in adequate amounts, confer a health benefit to the host.” The definition has three parts: the organism must be alive, the amount must be adequate, and a health benefit must be demonstrated. A microorganism’s presence alone is not enough.

Benefits are usually strain-specific. Two products can contain different strains from the same species and have different evidence, doses and results. A label that says only “Lactobacillus” or advertises a large total CFU number leaves out information needed to judge the product. ODS recommends looking for the genus, species and strain, the CFU amount, an expiration or use-by date and storage directions.
Prebiotics are substrates used by resident microbes
The 2017 ISAPP consensus statement defines a prebiotic as “a substrate that is selectively utilized by host microorganisms conferring a health benefit.” In practice, this often means a fermentable carbohydrate or fiber that particular microorganisms can use. The fiber must be selectively used and associated with a demonstrated health benefit; simply being fermentable does not make every fiber a prebiotic.
Prebiotics do not add a live organism. They alter the material available to microorganisms already living in the digestive tract. Effects can therefore vary with a person’s existing microbiome, the type and amount of fiber, and the outcome being measured.

Commercial products containing a probiotic microorganism and a prebiotic substrate are often called synbiotics. Under the consensus approach, the combination still needs to confer a health benefit. The word on a package is not proof that the particular mixture has been studied or that combining the ingredients is better than using either one alone.
How the difference changes what you buy or eat
Fermented food is not automatically a proven probiotic
ODS lists yogurt, cheese, sourdough bread, pickles, apple cider vinegar, kombucha, miso, kimchi and sauerkraut among fermented foods. Fermentation and probiotic status are not interchangeable. Processing, storage and the organisms used can differ, and some products may not contain a microorganism with a proven health benefit when consumed in the product’s final form.

If a food claims to contain probiotics, its label should identify the microorganism’s genus, species and strain. A food can still be nutritious without meeting the probiotic definition; the distinction prevents a general fermentation claim from being treated as clinical evidence.
Prebiotic foods supply different fibers
A PubMed-indexed overview lists wheat, onions, bananas, honey, garlic and leeks as foods containing fructooligosaccharides. NIH’s Research Matters summary identifies long-chain inulin in chicory root and vegetables such as onions and artichokes. These are examples of food sources, not guarantees of a particular symptom or disease outcome.
Whole foods also bring water, vitamins, minerals and other fibers, while a purified product supplies a more concentrated substrate. Concentration can be useful for a specific plan but also makes digestive side effects more likely if the amount is increased quickly.
Supplement formats are different
Probiotics are sold as capsules, powders, liquids and added ingredients in foods. Prebiotics may appear as a food ingredient or as a purified soluble-fiber product such as inulin powder. “Prebiotic fiber supplement” is a product category, not an individualized recommendation. No specific brand, preferred dose or universally effective formula is established by the evidence summarized here.
What the evidence actually supports
Probiotic benefits depend on the strain and condition
NIH and the National Center for Complementary and Integrative Health (NCCIH) describe promising or condition-specific findings, including research on prevention of antibiotic-associated diarrhea and other outcomes. In most cases, however, researchers still do not know which strain helps, how much is needed, how long it should be used or who is most likely to benefit. “Probiotics” is a category, not a single treatment.

That uncertainty is why a result for one strain should not be generalized to every product containing a similar species. A high CFU number also does not establish a greater benefit. Viable-cell count is one label detail; identity, evidence for the intended purpose, storage and expiration are equally important.
Prebiotic effects vary by fiber and individual
NIH Research Matters reported a tightly controlled NIH-funded study of 18 participants. Participants took arabinoxylan, long-chain inulin and a fiber mixture in separate three-week periods with washout periods between them. The effects varied by fiber and by individual. Long-chain inulin was associated with an increase in Bifidobacterium; in a few participants, high doses were associated with increased inflammation markers and alanine aminotransferase.
This was a small, short exploratory study. Changes in a microbiome measurement are not the same as an improvement in symptoms or health, and the findings do not show that inulin benefits everyone or establish a best dose.
The same NIH summary notes that as little as about 5% of the U.S. population consumes the recommended level of overall dietary fiber. That estimate concerns total fiber intake, not specifically prebiotic intake, and it does not prove that every person should take a prebiotic supplement. Increasing fiber through foods may be a reasonable nutritional goal, but the appropriate amount depends on diet, tolerance and medical context.
Irritable bowel syndrome is a useful warning against broad claims
For irritable bowel syndrome (IBS), NCCIH’s Clinical Digest reports that the 2021 American College of Gastroenterology guideline conditionally recommended against probiotics for global IBS symptoms, based on very-low-certainty evidence. NCCIH also characterizes the evidence for prebiotics in IBS as poor.

A 2019 review summarized by NCCIH included 11 trials and 729 people. Overall, prebiotic supplements did not improve gastrointestinal symptoms or quality of life compared with placebo. Moderate amounts of some non-inulin fructans were associated with improved flatulence, while inulin-type fructans increased flatulence. These findings apply to the studied products, amounts and outcomes; they are not a self-treatment plan for IBS.
Which one do you need?
For a healthy person asking about general wellness, the answer is usually neither as a mandatory supplement. ODS has no official recommendation for routine probiotic use by healthy people, and there is no universal instruction to add a prebiotic product. Start with the reason you are considering one, then match the intervention to evidence and tolerance.
If you want to improve your everyday diet
- Build a varied food pattern first. Include fiber-containing foods such as vegetables, fruit, whole grains and legumes as tolerated. Foods that contain potential prebiotic substrates include onions, garlic, leeks, bananas, wheat, chicory root and artichokes.
- Use fermented foods for what they actually provide. Yogurt, kimchi, sauerkraut, miso and other listed foods can be part of a varied diet, but do not assume that fermentation alone supplies a clinically proven probiotic.
- Change one major variable at a time. A sudden large increase in fermentable fiber can cause gas, bloating, cramps or diarrhea. A gradual change makes tolerance easier to assess.
If you have a defined medical goal
Identify the condition, the outcome you want and the evidence for a specific strain or substrate. Ask a clinician or pharmacist to check interactions, contraindications, dose and duration. A recommendation for one probiotic strain in one situation does not validate every probiotic, and a fiber study in healthy volunteers does not establish treatment for a disease.
If you are considering a probiotic after antibiotics
NCCIH describes some promise for preventing antibiotic-associated diarrhea, but the useful product, dose and patient group remain uncertain. Discuss the choice with the prescriber who knows why the antibiotic was used and whether you have risk factors that make a live microorganism inappropriate.

If you are considering a prebiotic supplement
First check whether you can meet your nutritional objective with ordinary foods. If a clinician has suggested a supplement, compare the named fiber, amount per serving and instructions rather than choosing by a “gut health” headline. An inulin powder or other prebiotic fiber supplement is a concentrated option, not a requirement and not proof of disease prevention or treatment.

How to evaluate a probiotic label
Use this checklist before buying or taking a product:
- Identity: The genus, species and strain should be stated. “Proprietary blend” without organism-level details makes comparison difficult.
- Amount: Check the CFU count and whether it is guaranteed through the expiration date or only at manufacture. A larger number alone does not show greater benefit.
- Purpose: Look for evidence for the condition or outcome you care about, not just a general promise about the microbiome.
- Storage: Follow refrigeration, moisture and temperature directions. Viability can change when a product is stored incorrectly.
- Date: Do not use a product past its expiration or use-by date unless a health professional and the manufacturer provide specific guidance.
- Duration: Select a trial period and stopping point with a clinician when the purpose is medical. Continuing indefinitely is not automatically better.
How to evaluate a prebiotic or fiber label
- Name the substrate: Look for the fiber type, such as inulin, rather than an unspecified “microbiome blend.”
- Check the amount: Compare grams of the named fiber per serving and account for other fiber in your diet.
- Read tolerance warnings: Gas, bloating, abdominal pain, cramps and diarrhea are more likely when larger amounts are introduced quickly.
- Follow preparation directions: Mix and store the product as directed, and avoid treating a concentrated powder as equivalent to a serving of vegetables or whole grains.
- Match the claim to evidence: A change in bacterial abundance is not automatically a clinical benefit. Look for a study of the same substrate, amount and outcome.
Safety, side effects and who should ask a clinician first
Probiotics
ODS says probiotics are unlikely to harm healthy people, although minor, self-limited effects such as gas can occur. Rare severe infections have been reported, with greater concern for people who are severely ill or immunocompromised. Serious illness, a weakened immune system, a central line or intensive medical treatment are reasons to obtain medical advice before using a live-microorganism product.
NCCIH also notes a 2023 U.S. Food and Drug Administration warning about severe, potentially fatal infections in premature infants given probiotics. Do not give a premature infant a probiotic supplement unless the infant’s medical team specifically directs it.

Because prebiotics are fermentable substrates, larger amounts can produce gas, bloating, pain, cramps or diarrhea. Reduce or stop a product and seek medical advice for persistent, severe or alarming symptoms. People with a diagnosed gastrointestinal condition should ask their clinician before using concentrated fermentable fibers, especially if they have reacted to them previously.
Regulation in the United States
NCCIH explains that U.S. regulation depends on intended use. A probiotic may be sold as a dietary supplement, food ingredient or drug. Dietary supplements generally do not require FDA approval before marketing, while products making disease-treatment claims face a stricter drug pathway. “Sold as a supplement” therefore does not mean “FDA-approved treatment.”
Common mistakes to avoid
- Equating “fermented” with “probiotic.” Verify the organism and evidence instead.
- Choosing the highest CFU count. The relevant strain, dose and studied purpose matter more than a headline number.
- Calling every fiber a prebiotic. The consensus definition requires selective use by host microorganisms and a health benefit.
- Assuming a microbiome change is a health outcome. Increased abundance of one bacterium does not by itself prove symptom relief or disease prevention.
- Using one product’s evidence for an entire category. Strain, substrate, amount, population and outcome all limit what a study can show.
- Starting with a large dose. Rapid increases in fermentable fiber commonly worsen gas and bloating.
- Using supplements as a substitute for care. Persistent symptoms, severe illness, immune compromise and infant use require professional guidance.
A practical decision rule
Choose neither supplement by default if you are healthy and have no defined goal. Choose foods first when your aim is a more varied diet. Consider a specific probiotic only when there is a plausible, condition-specific reason and a clinician can help identify the strain and plan. Consider a prebiotic supplement only when a named fiber serves a clear nutritional or clinical purpose and you can introduce it gradually. Stop if symptoms are significant, and reassess the underlying goal rather than switching randomly among products.
