Pneumonic Plague: Symptoms, How It Spreads, and When to Seek Emergency Care

Pneumonic plague is a medical emergency. It is a severe lung infection caused by Yersinia pestis. Fever, headache and weakness can be followed by rapidly developing pneumonia, shortness of breath, chest pain, cough and sometimes bloody or watery sputum. If these symptoms occur after a plausible exposure to plague—such as close contact with a person or animal with pneumonic plague, relevant travel, or contact with infected animals or fleas—seek immediate medical attention. The Centers for Disease Control and Prevention (CDC) says, “If you are experiencing symptoms like those listed here, seek immediate medical attention.”

Do not wait for symptoms to worsen, attempt to diagnose yourself, or delay care while waiting for a laboratory result. Tell the clinician or emergency dispatcher about any possible exposure so testing, treatment and public-health precautions can begin promptly.

What pneumonic plague is

Pneumonic plague is plague involving the lungs. The bacterium Yersinia pestis can reach the lungs in two ways: it may spread from untreated bubonic or septicemic plague, or a person may inhale infectious droplets from someone or an animal with pneumonic plague. CDC identifies pneumonic plague as the most serious form because it can progress quickly and is the only form that can spread from person to person.

Plague is uncommon, and ordinary respiratory symptoms without a relevant exposure do not by themselves indicate plague. The concern rises when a rapidly worsening pneumonia occurs together with a plausible travel, animal, flea or close-contact history. Because the illness can resemble other severe infections, only clinical evaluation and laboratory testing can establish the diagnosis.

CDC’s United States guidance is updated periodically. The symptom and clinical-care pages cited here were dated May 15, 2024; the person-to-person transmission fact sheet was dated April 1, 2026. Follow current instructions from your clinician and local or state health department.

Pneumonic plague symptoms

Symptoms may begin with general infection signs and then progress to serious lung disease. CDC lists the following pattern:

Early or general symptoms

  • Fever
  • Headache
  • Weakness

These symptoms may be followed by rapidly developing pneumonia. The transition can be fast, which is why waiting to see whether an illness “settles” is unsafe when exposure is plausible.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Lung and chest symptoms

  • Shortness of breath
  • Chest pain
  • Cough
  • Bloody or watery mucus (sputum) in some cases

Pneumonic plague does not have a single symptom that proves the diagnosis. Cough and fever occur with many respiratory infections, while bloody sputum and breathing difficulty can have several causes. The combination of a rapidly progressive pneumonia and a credible plague exposure is what requires urgent assessment.

Why the speed of illness matters

Untreated plague may progress quickly. A person can become seriously ill before there is a definitive laboratory report, so clinicians use symptoms, examination and exposure information to decide whether to start treatment immediately. The CDC clinical-care guidance states, “Begin appropriate therapy as soon as plague is suspected.”

When to seek emergency care

Seek immediate medical attention if you have symptoms compatible with pneumonic plague and any plausible exposure. Go to an emergency department or contact emergency services immediately for breathing difficulty, chest pain, rapidly worsening cough, bloody sputum, marked weakness or a high fever after a relevant exposure. Do not use symptom improvement, a lack of swollen lymph nodes or the absence of bloody sputum to rule it out.

Information to give the medical team

Tell the dispatcher, emergency department or clinician:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • When the fever, cough or breathing symptoms began and how quickly they changed.
  • Whether you were within close, direct range of someone with suspected or confirmed pneumonic plague, particularly while that person was coughing.
  • Recent travel, especially to an area where plague is reported, if applicable.
  • Contact with sick or dead wild animals, domestic animals with suspected plague, rodents or fleas.
  • Any diagnosis or treatment for bubonic or septicemic plague in yourself or a close contact.

Call ahead when possible and follow the instructions given by emergency personnel. Healthcare facilities may need to arrange respiratory precautions and notify public-health authorities. These decisions belong to trained responders; do not try to create a medical isolation plan or choose prescription antibiotics on your own.

How pneumonic plague spreads

Spread from plague in another part of the body

Untreated bubonic plague, which often begins after an infected flea bite, or septicemic plague, which involves the bloodstream, can spread to the lungs and become pneumonic plague. This route does not require catching the illness from another person.

Inhalation of infectious respiratory droplets

A person or animal with plague pneumonia can cough droplets containing Y. pestis. Another person may become infected by inhaling those droplets. CDC describes this as a close, direct exposure. In its April 1, 2026 fact sheet, CDC defines close contact for its guidance as being within 6 feet of a patient with pneumonic plague while that patient was coughing.

Person-to-person transmission is rare and is associated with this direct droplet exposure. It is not accurate to describe pneumonic plague as an infection usually acquired from passing someone briefly in a public place, nor to label the mechanism ordinary long-distance airborne spread. Risk depends on the infected person’s illness, coughing and the nature and duration of the contact. Public-health officials will determine who needs evaluation after a confirmed case.

Exposure to an animal

Animals with plague pneumonia may also produce infectious droplets. Handling a sick animal, being close to an animal that is coughing, or contact with infected animal tissues can be relevant exposure information. Flea and animal histories should be reported to clinicians even when the person-to-person route seems unlikely.

Incubation: how soon symptoms can appear

CDC describes a typical incubation period of one to three days, with a reported range of one to six days in its April 1, 2026 person-to-person transmission fact sheet. CDC’s clinical testing information also describes incubation as usually one to three days. These are typical time frames, not a guarantee that every infected person will become ill on the same schedule.

Write down the dates of close contact, animal or flea exposure and symptom onset. A short interval between exposure and rapidly worsening respiratory illness should be reported immediately, but a longer or uncertain interval does not make medical evaluation unnecessary.

How pneumonic plague differs from other plague forms

Form Main site and typical presentation How it is acquired or spread Person-to-person spread
Bubonic plague Often causes painful, swollen lymph nodes (buboes) with fever and other systemic symptoms. Commonly follows the bite of an infected flea; it can progress to other forms if untreated. Not the usual route.
Septicemic plague Infection of the bloodstream with severe systemic illness; obvious buboes or lung symptoms may be absent. May arise directly from an exposure or from untreated bubonic disease. Not the usual route.
Pneumonic plague Rapidly developing pneumonia with shortness of breath, chest pain, cough and sometimes bloody or watery sputum. Can result when plague reaches the lungs or when infectious respiratory droplets are inhaled. Yes, through close, direct respiratory-droplet exposure; this is rare.

These categories can overlap as infection progresses. A person does not need to identify the form at home; the purpose of the distinction is to help clinicians understand symptoms and exposure history.

How clinicians diagnose suspected pneumonic plague

Diagnosis combines the illness pattern, examination, exposure history and laboratory testing. CDC says clinicians may submit blood or material from a swollen lymph gland for testing. For pneumonic disease, respiratory material and other specimens may be collected according to the clinical situation and public-health instructions.

There may be no single obvious sign that separates pneumonic plague from other severe pneumonias. Tell clinicians about travel, animal or flea contact and close contact with a person who was coughing. That history can change which tests are ordered, how specimens are handled and what infection-control measures are used.

Testing takes place in clinical and public-health settings. A suspected case should not wait for a final result before treatment when the clinician considers plague a serious possibility.

Treatment and infection-control decisions

Pneumonic plague requires clinician-directed antimicrobial treatment. CDC identifies gentamicin and fluoroquinolones as first-line treatment options in the United States; the drug, dose, route and monitoring depend on severity, age, pregnancy status, kidney function, allergies and other individual factors. Treatment duration is generally 10–14 days, but the treating team determines the actual regimen.

Do not buy or use leftover antibiotics, share medication, or substitute an over-the-counter product. Inappropriate treatment can delay effective care and complicate evaluation. The key clinical principle is early treatment: CDC instructs clinicians to begin appropriate therapy as soon as plague is suspected rather than waiting for final laboratory confirmation.

CDC emergency-response guidance calls for standard and respiratory-droplet precautions for suspected or confirmed cases during the first 48 hours of antimicrobial therapy and until clinical improvement or plague has been ruled out. Those are healthcare and responder instructions. Patients and household members should follow the specific directions issued by the treating facility and public-health officials.

What to do while arranging care

  1. Act immediately. Contact emergency services or go to an emergency department for compatible symptoms after a plausible exposure.
  2. State the concern clearly. Say that pneumonic plague is a possibility and describe the exposure, date and symptom progression.
  3. Limit close contact while following instructions. Ask emergency personnel or public-health staff what precautions to use during transport and while waiting; do not improvise a substitute for professional guidance.
  4. Bring exposure details. Note travel locations and dates, animal or flea contact, and the names or dates of any known close-contact events.
  5. Follow public-health directions. Health departments may identify and evaluate people who had qualifying close exposure.

Questions people commonly ask

Can a routine cough mean pneumonic plague?

Most coughs are caused by conditions other than plague. Concern is warranted when respiratory illness is rapidly progressive and occurs with a plausible plague exposure. Only a clinician can assess the combination safely.

Can I wait for a test before seeking care?

No. Seek care first. Clinicians may collect specimens, but CDC guidance says treatment should begin as soon as plague is suspected rather than being delayed for final laboratory results.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Is casual contact enough to spread it?

CDC describes person-to-person transmission as rare and linked to direct, close exposure to respiratory droplets, including being within 6 feet of a coughing patient in its 2026 guidance. Public-health officials assess individual contacts after a suspected or confirmed case.
