Pneumonic plague is a medical emergency. It is a rapidly progressing lung infection caused by Yersinia pestis. Fever with a rapidly worsening cough, chest pain, shortness of breath, or bloody or watery sputum requires immediate professional medical evaluation when plague is possible. Call emergency services or contact an urgent medical provider, and tell them about recent travel, animal or flea exposure, or contact with someone who has a serious respiratory illness.
Do not wait for a test result or try to treat suspected plague yourself. Clinicians use laboratory testing to confirm the infection, but start appropriate antibiotics and supportive care promptly when suspicion is high.

What pneumonic plague is
Pneumonic plague is the form of plague that infects the lungs. The bacterium responsible is Yersinia pestis, which normally circulates among small mammals and their fleas. In people, pneumonic disease can develop in two ways: plague may spread to the lungs from untreated bubonic or septicemic infection, or a person may inhale infectious respiratory particles from an infected person or animal.

Unlike bubonic plague, which is identified mainly by painful swollen lymph nodes, pneumonic plague produces rapidly developing pneumonia. It is also the plague form capable of efficient person-to-person spread through respiratory particles. Because deterioration can be fast, a person with compatible symptoms should be assessed immediately rather than monitored at home.

Symptoms and how quickly they appear
Typical symptoms described by the U.S. Centers for Disease Control and Prevention (CDC) include:
- Fever
- Headache
- Marked weakness or fatigue
- Shortness of breath
- Chest pain
- Cough, which may produce bloody or watery mucus
- Rapidly developing pneumonia
After inhalation exposure, the incubation period can be very short. The World Health Organization (WHO) says symptoms may begin within 24 hours. CDC materials describe a usual incubation of about one to three days, while noting that onset can be as short as one day. The timing varies with the route of infection and the individual; a longer symptom-free interval does not rule plague out if exposure is plausible.
Early symptoms can resemble other severe infections, including ordinary bacterial pneumonia, influenza, or COVID-19. There is no reliable way to distinguish them at home. Bloody sputum, severe breathlessness, confusion, blue or gray lips, fainting, or rapidly worsening weakness are especially urgent warning signs, but a person should not wait for those late features before seeking care.
How pneumonic plague is acquired and transmitted
Spread from another form of plague
Untreated bubonic plague can spread from lymphatic tissue into the bloodstream and then to the lungs. Septicemic plague, which involves the bloodstream, can also progress to lung infection. This progression is possible but not inevitable; plague does not follow one identical sequence in every patient.
Inhalation of infectious particles
A person can develop primary pneumonic plague after inhaling respiratory particles from someone or an animal with pneumonic infection. Close, unprotected contact with coughing secretions is the relevant risk. Ordinary casual contact is not the same exposure, but anyone who may have been near a suspected case should follow instructions from medical and public-health professionals.

Animal, flea, and tissue exposure
People can acquire plague through infected flea bites or contact with infected animal tissue and body fluids. Dead animals, carcasses, and visibly ill wildlife should not be handled. In endemic areas, flea control and avoiding sick or dead small mammals reduce exposure opportunities. A flea bite itself does not prove plague, but it is important information for a clinician evaluating compatible illness.
How pneumonic, bubonic, and septicemic plague differ
| Form | Main site | Typical clue | Person-to-person respiratory spread |
|---|---|---|---|
| Bubonic plague | Lymphatic system | One or more painful, swollen lymph nodes (buboes), often after an infected flea bite | Not the usual route |
| Pneumonic plague | Lungs | Rapid pneumonia with cough, chest pain, shortness of breath, and sometimes bloody or watery sputum | Yes; respiratory particles can transmit it |
| Septicemic plague | Bloodstream | Severe systemic illness from bacteria in the blood; skin and other organs may be affected | Not the usual route unless lung infection also develops |
A person can have more than one form during the course of illness. The table describes the dominant clinical pattern, not a guarantee that symptoms will remain confined to one body system.

Seek immediate medical attention for sudden fever or chills accompanied by a rapidly worsening cough, shortness of breath, chest pain, or bloody expectoration when any plausible exposure exists. The WHO advises people with sudden fever, painful swollen lymph nodes, or breathing difficulty with cough or bloody sputum to contact a medical provider immediately. CDC guidance similarly directs people with suggestive symptoms after living in or recently traveling to a plague-endemic area to seek care without delay.
CDC’s public guidance is direct: “If you are experiencing symptoms like those listed here, seek immediate medical attention.” That instruction applies even if you are unsure whether the illness is plague.
Information to give the medical team
- The exact time symptoms began and how quickly they are worsening
- Recent travel, especially to a location reporting plague activity
- Flea bites, contact with rodents or other small mammals, or handling of a dead animal
- Contact with anyone who has suspected or confirmed pneumonic plague
- Current medicines, allergies, pregnancy status, and significant medical conditions
Call ahead when possible so the facility can arrange appropriate precautions. If breathing is severely impaired, the person is confused, collapses, or cannot safely travel, call emergency services rather than driving.

What to do while arranging care
- Contact emergency or urgent medical services now. State that pneumonic plague is a concern and describe the respiratory symptoms.
- Limit close contact. Keep distance from household members, avoid visitors, and do not share a room if another safe option exists.
- Cover coughs and follow professional instructions about masking. A properly used mask may reduce spread in some situations, but the right type and use depend on the exposure and the person’s ability to wear it.
- Do not handle potentially contaminated animals, tissues, or secretions. Leave cleaning and disposal to trained personnel.
- Do not self-prescribe antibiotics. The appropriate medicine, route, duration, and monitoring depend on clinical severity, age, allergies, organ function, and other conditions.
These steps do not replace evaluation. They are interim precautions while qualified clinicians and public-health officials determine the diagnosis and required isolation.

Diagnosis combines the clinical picture, exposure history, examination, imaging and other assessments, and laboratory testing. Possible specimens include blood, sputum, or pus from a bubo. Testing may identify Yersinia pestis or an immune response to it, depending on the specimen and test used.
Laboratory confirmation is important because several common diseases can look similar at first. However, CDC guidance says treatment should not be delayed while waiting for final laboratory results when plague is strongly suspected. A clinician may collect specimens before treatment when that can be done safely, but preserving the opportunity to treat promptly takes priority.
Treatment: why professional care cannot wait
Pneumonic plague is treated with prescription antimicrobial therapy selected by clinicians. The choice and delivery method depend on disease severity, age, medical history, allergies, pregnancy status, underlying conditions, and local clinical guidance. Severe illness may require intravenous therapy and hospital monitoring.
Supportive treatment can include oxygen, fluids, management of complications, and respiratory support when needed. Hospital teams monitor breathing, blood pressure, kidney function, and other organ systems because the infection can progress quickly.

WHO’s 2021 management guideline discusses rapid diagnostic tests, antimicrobial options, and protective equipment for healthcare and post-mortem settings. CDC treatment tables are intended for clinicians and may require individual adjustment. They are not a basis for choosing a drug or dose without medical supervision.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Contagiousness, isolation, and public-health precautions
Suspected or confirmed pneumonic plague is managed as a potentially contagious respiratory infection. In healthcare settings, patients are isolated and cared for using respiratory droplet precautions. CDC emergency guidance says those precautions continue during the first 48 hours of antimicrobial treatment and until clinical improvement occurs or plague has been ruled out.
The exact room arrangement, mask or respirator selection, visitor policy, and duration of precautions are decided by the treating facility and public-health authorities. People who have had a significant exposure may be contacted for evaluation, monitoring, or preventive treatment. Do not identify or confront a possible source yourself; provide the exposure details to clinicians so officials can handle contact assessment safely.
Prevention in areas where plague occurs
- Avoid close contact with anyone who is coughing and seriously ill during an outbreak or suspected cluster.
- Do not touch dead animals or carcasses, particularly small mammals, and keep pets away from wildlife and fleas.
- Use appropriate flea-control measures for pets and living spaces in endemic areas.
- Follow local public-health notices about animal die-offs, travel, and suspected cases.
- If you become ill after travel or an exposure, disclose the location, dates, and activities immediately.
WHO notes that masks can reduce spread when worn correctly by sick people and can protect healthcare workers, but protection depends on the exposure and correct use. A mask is not a substitute for urgent evaluation, isolation instructions, or prescribed treatment.

Untreated pneumonic plague can be fatal very quickly. WHO’s 2026 fact sheet reports that an untreated pneumonic infection may reach a fatal outcome in 18–24 hours in severe cases. This is a description of an extreme untreated risk, not a prediction of what will happen to a particular patient or a deadline for deciding whether to seek care.
Best Value
For context, WHO reports a 30%–60% case-fatality ratio for untreated bubonic plague and a 30%–100% range for plague overall when left untreated. Those figures are not a pneumonic-plague survival estimate and should not be applied to an individual. Prompt clinical treatment materially changes the situation, which is why suspected disease is handled as an emergency.
Between 2019 and 2025, WHO reports that most human plague cases occurred in the Democratic Republic of the Congo and Madagascar. Plague is not limited to those countries, however; exposure risk depends on local animal and flea transmission, travel, and specific contacts.
Common misconceptions to avoid
“It only matters if I have a swollen lymph node.”
No. Buboes are characteristic of bubonic plague, but pneumonic plague primarily causes lung symptoms and may begin without a noticeable swollen node.
Recommended Free Tools
“A negative first test means I can wait at home.”
No. Test selection, timing, and specimen quality affect results. If clinical suspicion is high, CDC guidance supports beginning treatment before final laboratory confirmation.
“Antibiotics left over from another illness are adequate.”
No. An unsuitable drug, dose, route, or duration can fail to control a rapidly progressive infection and may delay proper care. Treatment must be selected and monitored by a clinician.
“Any cough after travel is plague.”
No. Most coughs are caused by other conditions. Travel or animal exposure does not establish a diagnosis, but it is crucial context when symptoms are sudden, severe, or rapidly worsening.
What should I do if I suspect I have plague?
Contact a medical provider immediately, explain the symptoms and any travel or animal, flea, or respiratory exposure, and follow instructions about transport, masking, and separation from others. Do not wait for symptoms to become dramatic, and do not attempt self-treatment. Because pneumonic plague can progress and spread, rapid professional assessment protects both the sick person and the people around them.
