For most adults with chronic primary low back pain, start with education and an active plan built around exercise, often with guidance from a physical therapist. Medication can be a time-limited add-on when symptoms remain disabling, non-drug care is unavailable or unsuitable, or a clinician identifies a specific reason to use it. Average medication benefits are small, and opioids, traction and routine back braces are generally poor default choices.
What “chronic back pain” means
Chronic primary low back pain is pain in the lower back lasting more than three months without an identifiable disease such as cancer, infection or a major inflammatory disorder causing it. The World Health Organization (WHO) estimates that this category accounts for at least 90% of chronic low-back-pain presentations in primary care. It is not a minor problem: low back pain produced 8.1% of all years lived with disability worldwide in 2020. As WHO Assistant Director-General Bruce Aylward put it, “To achieve universal health coverage, the issue of low back pain cannot be ignored, as it is the leading cause of disability globally.”

These approaches are not mutually exclusive. Physical therapy is a clinician-delivered process; exercise is the active training you perform and continue; medication is a symptom-modifying tool with potential systemic harms. WHO recommends a person-centred package that can combine education, exercise, selected physical therapies, psychological therapy and medicines rather than relying on one intervention.

| Approach | What it can change | Time to benefit | Durability | Main risks or burdens | Access and fit |
|---|---|---|---|---|---|
| Physical therapy | May improve pain, movement confidence and function through an individualized program; manual or other directed treatments may be added selectively. | Some people notice changes during the first few sessions, but meaningful functional gains usually require several weeks of practice. | Best maintained when skills and exercises continue after visits end; protocols vary and no single format is clearly superior. | Time, travel and visit cost; temporary soreness or symptom flares can occur; quality depends on assessment and progression. | Useful when pain, fear of movement, weakness or a complicated history makes self-directed exercise difficult. |
| Independent or group exercise | Builds strength, endurance, aerobic capacity and confidence in daily activity; can target function even when pain does not disappear. | Often gradual over weeks; a short-lived flare does not necessarily mean harm. | Requires ongoing participation. Benefits can fade when activity stops. | Adherence, uncertainty about technique and choosing too much load too soon; an unadapted routine may aggravate symptoms. | Usually the lowest ongoing cost and can be home-based, supervised, aquatic or community-based. |
| Medication | May reduce symptoms enough to permit sleep, work or participation in rehabilitation; it does not correct most mechanical contributors to persistent pain. | NSAIDs can act within hours to days; other medicines require a clinician-defined trial. The exact response is individual. | Effects commonly stop when the medicine stops. Long-term use adds cumulative risk and monitoring. | NSAIDs can cause gastrointestinal, kidney and cardiovascular complications; other drugs have their own adverse effects. Opioids carry dependence and other harms. | May be considered when non-drug care is insufficient, contraindicated, inaccessible or needed briefly as an adjunct. |
Trial averages do not predict an individual response. A person may improve substantially with a modestly effective option, while another experiences no change. Reassess function, sleep and ability to do valued activities—not pain intensity alone—when deciding whether to continue.
What physical therapy actually involves
Assessment before treatment
A physical therapist reviews the pain history, work and home demands, sleep, activity levels, previous treatment, medications and relevant medical conditions. The examination may include movement, strength, sensation, balance and tasks that reproduce or relieve symptoms. The goal is to identify modifiable limitations and screen for signs that need medical investigation, not to label every pain episode as a structural “misalignment.”
Exercise prescription and education
The core of therapy is usually a graded plan matched to current tolerance. Education can cover pain mechanisms, pacing, lifting and sitting strategies, sleep positions, and how to distinguish an expected training response from a concerning change. A therapist may begin with short bouts and low loads, then increase repetitions, resistance, range or complexity as function improves.

Manual and other directed treatments
Hands-on techniques or other physical modalities may be used for short-term symptom relief or to help someone move enough to exercise. They should support an active plan rather than replace it. WHO’s recommendations favor selected physical therapies as part of a package and advise against routine reliance on traction or lumbar supports.
Who benefits most from supervised therapy?
- People who are unsure how to exercise safely or who have repeatedly stopped because of flares.
- People with substantial deconditioning, movement fear, weakness, balance limitations or demanding work tasks.
- People whose symptoms coexist with another condition, recent surgery or a complex medication history.
- People who need progression, accountability or adaptation when home exercises are not working.
Access varies by insurance, geography, referral rules and therapist availability. If visits are limited, ask for a written home program, clear progression rules and a way to contact the clinic when symptoms change.

How exercise helps—and which type to choose
The Academy of Orthopaedic Physical Therapy’s 2021 guidance supports exercise training for chronic low back pain, including trunk strengthening or endurance work, multimodal programs, aerobic exercise, aquatic exercise and general exercise. It does not identify one clearly superior type. That finding matters: the best program is usually the one that addresses your limitations and that you can perform consistently.
Trunk strengthening and endurance
Exercises may train the muscles that help you brace, extend, rotate and stabilize the trunk. Start with a load that permits controlled breathing and technique. Progress one variable at a time—duration, repetitions, resistance or complexity—rather than increasing everything in a single session.

Walking, cycling, swimming or another tolerable aerobic activity can improve stamina and reduce the amount of daily activity that feels threatening. Begin with a duration you can repeat on most days, then add time in small increments. Frequent short walks can be as practical as one long session for someone whose symptoms rise with prolonged activity.
Aquatic exercise
Water buoyancy can reduce loading while allowing movement. It is an option for people who cannot yet tolerate comparable land-based exercise, although pool access, transport and fees may be barriers.
Multimodal programs
A multimodal plan can combine strengthening, aerobic work, mobility, education and behavioral strategies. It is useful when pain affects several areas of life, but it requires coordination and may be harder to schedule than one type of exercise.

How to handle a flare
- Reduce the most provocative movement or volume temporarily instead of stopping all activity.
- Keep gentle, tolerable movement and ordinary daily tasks going when safe.
- Return to the last manageable level, then progress more slowly.
- Contact a clinician if the flare is severe, persistent, associated with new neurological symptoms or unlike your usual pattern.
A 2023 systematic review of structured exercise informed the WHO guideline for adults with chronic primary low back pain. The evidence includes varied protocols and often short-term group differences, so it supports offering exercise without proving that one branded method or style is best.
What medication can and cannot do
Nonsteroidal anti-inflammatory drugs (NSAIDs)
The American Academy of Family Physicians (AAFP) describes nonpharmacologic care as first-line and NSAIDs as the initial medication choice when a drug is appropriate. Cochrane’s 2023 review found that NSAIDs may provide a small benefit for chronic low back pain, with no high- or moderate-certainty evidence that investigated drugs produce a large or medium pain effect.
NSAIDs are not suitable for everyone. A clinician or pharmacist should review kidney disease, previous gastrointestinal ulcer or bleeding, cardiovascular disease, uncontrolled blood pressure, pregnancy, interactions with anticoagulants or other medicines, and duplicate use of over-the-counter products. Use the lowest effective exposure for the shortest practical period, and seek advice promptly for black stools, vomiting blood, severe abdominal pain, swelling, breathlessness or markedly reduced urine.
Duloxetine
AAFP identifies duloxetine as a possible option for some patients when non-drug care and NSAIDs are inadequate or unsuitable. It requires an individualized prescription, review of other medicines and a plan to assess benefit and adverse effects. Do not stop it abruptly without medical guidance. If there is no meaningful improvement in function or tolerable symptom control after the agreed trial, continuing indefinitely is difficult to justify.
Opioids
WHO advises against routine opioid painkillers for most people with chronic primary low back pain because potential harms can outweigh benefits. Cochrane found that opioids may increase adverse events, and AAFP describes long-term opioid benefit-risk profiles as uncertain or unfavorable. Dependence, overdose, sedation, constipation, falls and impaired driving are practical concerns. An opioid should not be the default response to an exercise flare or a normal imaging finding.

Other drug classes
Several other medicines used for pain have uncertain or unfavorable evidence in this setting. A clinician should explain the specific rationale, expected endpoint, interactions and stopping plan rather than adding drugs indefinitely because pain persists.
Do not extrapolate acute-pain findings
A 2023 BMJ network meta-analysis examined acute nonspecific low back pain and found increased adverse events with tramadol-containing regimens and baclofen compared with placebo. That evidence is acute, not chronic; it should not be presented as direct proof of long-term effectiveness or harm in chronic pain. It does, however, reinforce the need to distinguish short episodes from persistent symptoms when weighing medicines.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Choosing a starting point in common situations
You have chronic pain without red flags and have not tried a structured program
Begin with education and a tolerable exercise plan. A physical therapist can individualize the starting point, especially if you are deconditioned or uncertain about technique. Reassess after several weeks using walking tolerance, work, sleep and daily tasks as outcomes.
You can exercise but pain still limits participation
Check load, recovery, sleep, mood, work demands and progression before assuming the exercise has failed. A brief, clinician-guided medication trial may make participation possible, but it should have a defined goal and review date.
Recommended Free Tools
You cannot access physical therapy
Ask a primary-care clinician for a basic activity plan, credible written instructions and criteria for escalation. Community walking, home strength work, aquatic sessions or supervised group classes may provide alternatives. Limited access is a reason to adapt delivery—not a reason to default to long-term medication.
Best Value
You have kidney, gastrointestinal, cardiovascular or other important comorbidities
Medication selection requires a clinician’s review. NSAIDs may be inappropriate, and interactions or monitoring needs can change the balance. Exercise can often be adapted, but the therapist needs the complete medical history.
You have numbness, weakness or radiating leg pain
Some radiating symptoms occur without an emergency, but distribution, severity and progression matter. Arrange medical assessment rather than treating escalating neurological symptoms solely with home exercise or analgesics. Progressive weakness, new bowel or bladder difficulty or saddle-area numbness requires urgent evaluation.
A practical sequence for the first 12 weeks
- Confirm the problem. Arrange an assessment if pain has lasted more than three months, is worsening, or has not been evaluated. Screen for red flags and review medicines and comorbidities.
- Set functional targets. Choose two or three measurable goals, such as walking for 20 minutes, returning to a work task, sleeping through more of the night or lifting a specified everyday load.
- Start below the flare threshold. Use a manageable exercise dose and normal daily movement. Record what you did and how you felt later that day and the next morning.
- Progress gradually. Increase only one training variable at a time. A therapist can modify technique, range, resistance or frequency when progress stalls.
- Review at a planned interval. If function is improving, continue and build independence. If not, reassess the diagnosis, adherence, psychosocial barriers, work demands and exercise fit rather than simply adding another medicine.
- Use medication selectively. If symptoms still prevent sleep or rehabilitation, discuss an NSAID or another evidence-based option, contraindications, duration and stop criteria. Do not combine products casually.
- Escalate when the pattern changes. New systemic symptoms, severe unremitting pain, trauma or neurological deterioration warrants medical review.
Costs, convenience and adherence
There is no universal price comparison. Physical-therapy costs depend on the health system, insurance, referral requirements and number of visits. Exercise usually has little ongoing financial cost once a safe program is learned, but equipment, pool access, transport or coaching can add expense. Medicines may be inexpensive at the point of purchase while creating monitoring, adverse-event or dependency costs over time.

A short home routine performed four days a week may outperform an ideal program that requires an hour-long commute. Conversely, someone who repeatedly abandons unsupervised exercise may gain more from a few supervised visits and scheduled check-ins.
What to avoid as routine solutions
- Routine lumbar braces or supports: WHO advises against routine use for most chronic primary low back pain.
- Traction devices: WHO advises against routine traction because expected benefit does not justify making it a default treatment.
- Long-term opioids: potential harms and uncertain durable benefit make them unsuitable as a standard pathway.
- Passive treatment without progression: temporary relief is not the same as restored capacity; pair any passive modality with active rehabilitation.
- Chasing a single “best” exercise: evidence supports several formats, and no one type is clearly superior across patients.
When self-management is not enough
Seek urgent care for new loss of bladder or bowel control, saddle-area numbness, rapidly progressive leg weakness, severe pain after significant trauma, fever with back pain, or a very unwell appearance. Arrange prompt medical evaluation for unexplained weight loss, a history of cancer, immunosuppression, intravenous drug use, persistent night pain or symptoms that steadily worsen. These features can indicate a condition that needs diagnosis and targeted treatment rather than routine exercise progression.
The decision in plain terms
For uncomplicated chronic primary low back pain, an active plan—education plus progressive exercise, with physical-therapy guidance when useful—is the most defensible starting point. Medication can be layered in selectively to remove a barrier to activity, but average effects are modest and safety screening matters. Make decisions with a clinician when symptoms, comorbidities or access constraints complicate the picture, and judge success by durable function and participation rather than by seeking zero pain immediately.
