Start with Dead of the Brain 2. It is the direct sequel and the clearest follow-up to the original PC-98 zombie adventure. If you want different leads, a PC-98 community discussion points to Marine Philt and Necronomicon, while PC-98.org’s Gore category offers Corpse Party, Ballade for Maria, and Elle as broader discovery picks.
The evidence for these games is uneven: the sequel is firmly connected, but several other suggestions are supported only by community comments or broad category labels. Use the guide below to choose by premise, gameplay, language, access and content rather than assuming every listed title plays like Dead of the Brain.

Dead of the Brain 2 is the closest match
The sequel should be your first stop because it continues the same series rather than merely sharing a platform or horror label. It appears alongside the original in PC-98 adventure indexes, and community members answering requests for similar PC-98 horror games name it as the obvious next game.

That continuity matters. The first game follows Cole after his scientist friend, Dr. Cooger, uses a serum to revive the dead and causes a zombie outbreak. Its format mixes visual-novel-style dialogue with command-based exploration: you inspect scenes, choose actions such as Look, Use and Get, and respond quickly when threats appear. A sequel is therefore the only recommendation here with a documented direct relationship to that premise and structure.

An October 8, 2026 report said the publisher had teased a future English release for Dead of the Brain 2, but gave no release date. Treat that as an announcement, not a currently available localization. The same report described the modern release plans for the series as preserving the pixel art and FM soundtrack while adding optional hints and more forgiving checkpoints. Those changes address friction in the original without promising that the underlying adventure has been redesigned.
Who should choose it
- Choose it if you want the same fictional world and a continuation rather than a new setting.
- Choose it if command-driven exploration, extended dialogue and sudden reaction moments are part of what you enjoyed.
- Wait for confirmed availability if you require an official English version; no sequel date was established in the October report.
How the recommendations compare
The table separates what is actually established from what remains a lead for further investigation. “Not established” means the available descriptions do not verify the point, not that the game lacks the feature.
| Game | Why it appears here | What is established | Language and access notes | Confidence as a Dead of the Brain follow-up |
|---|---|---|---|---|
| Dead of the Brain 2 | Direct sequel; named in PC-98 adventure indexes and community recommendations | Same series; future English release was teased | No sequel release date was given in the October 8, 2026 report | Closest match |
| Marine Philt | Recommended in a PC-98 community discussion | A commenter identifies it as coming from the same developer as Dead of the Brain; other similarities are not verified | Current language and official access were not established | Exploratory lead |
| Necronomicon | Recommended in the same community discussion | A commenter describes it as Japanese-language; a translation was reportedly being worked on | Do not assume an English release or completed translation | Exploratory supernatural-horror lead |
| Corpse Party | Listed by PC-98.org under Gore | Broad gore-category placement only; zombie premise and command adventure format are not established here | Current language and access were not established | Discovery candidate |
| Ballade for Maria | Listed by PC-98.org under Gore | Broad gore-category placement only | Current language and access were not established | Discovery candidate |
| Elle | Listed by PC-98.org under Gore | Broad gore-category placement only | Current language and access were not established | Discovery candidate |
Exploratory pick: Marine Philt
Marine Philt is worth investigating if the appeal of Dead of the Brain includes its developer lineage as much as its zombie story. A commenter in the relevant PC-98 discussion identifies Marine Philt as coming from the same developer. That is a useful connection, but it does not establish that the game uses the same interface, horror subgenre, pacing or subject matter.

Exploratory pick: Necronomicon
Necronomicon is the better lead for readers who are open to supernatural horror rather than specifically seeking another zombie outbreak. The community recommendation identifies it as Japanese-language, and another commenter was reportedly working on a translation. That statement describes an in-progress community effort, not a confirmed finished patch or official English edition.
Its inclusion should therefore set expectations rather than promise convenience. If you do not read Japanese, verify whether a translation is complete, compatible with your copy and legally obtainable before treating the game as your next play. The available evidence does not establish that it shares Dead of the Brain’s command verbs, reaction sequences or dialogue balance.
Broader Gore-category discoveries
PC-98.org places Corpse Party, Ballade for Maria and Elle in its Gore category. That index is useful for expanding a search, but a category label cannot answer the questions that matter when moving on from Dead of the Brain: Is the game about zombies or another threat? Does it use point-and-click commands, menu choices or a different format? Is there an English version? How graphic is it in practice?
Corpse Party
Use this as a discovery candidate if explicit gore is your main filter. The category listing does not establish a zombie premise, the amount of exploration, or the current language and access situation for the PC-98 version.

Ballade for Maria
This title belongs on a wider horror shortlist because of the same Gore classification. Nothing in the available listing confirms that its tone, mechanics or setting resemble Cole’s outbreak story, so investigate those points independently.
Elle
Elle is another index-based lead rather than a verified mechanical match. The listing supports its use as a starting point for browsing PC-98 gore-oriented games, not a claim that it reproduces the original’s command exploration or quick-threat scenes.
What makes Dead of the Brain distinctive
Knowing the reference game’s design helps you decide how much similarity you need. The story premise is exploitation-horror science fiction: Dr. Cooger’s revival serum turns a scientific experiment into a zombie outbreak, with Cole caught in the aftermath. The presentation combines long stretches of illustrated, visual-novel-like conversation with explicit action commands. You examine objects and people, apply items, collect things and occasionally react under pressure when an on-screen threat demands a quick response.
Quick wins for a faster PC:
Clear out junk files and repair common Windows errorsFree Scan →Fix the driver behind crashes, sound loss and screen glitchesFind Drivers →The planned modern edition is described as retaining the pixel art and FM soundtrack while adding optional hints and more forgiving checkpoints. Those additions are quality-of-life features, not evidence that every old interaction has been simplified. Coverage of the original experience cautions that it can feel less streamlined than newer horror adventures, which is relevant when setting expectations for any sequel or neighboring PC-98 title.

Language and access: what is confirmed
As of October 13, 2026, AUTOMATON WEST reported that Dead of the Brain was scheduled for Steam on October 30, 2026 with official English support. October 30 was still in the future on that date, so describe the Steam version as announced rather than available unless a later store or publisher update confirms the launch.
The same report said the publisher had teased English releases of Dead of the Brain 2 and other FairyTale horror titles without supplying a sequel date. Do not convert that tease into a release window.
PC Gamer reported that fan translations of both games existed before the announced official release. A fan patch is separate from an official localization: it may require an original game copy, a specific edition and manual installation, and the available reports do not establish one universal setup. If you use a fan translation, confirm its compatibility and provenance rather than assuming it works with every release.

Best Value
Choose your next game by priority
If continuity matters most
- Try to obtain Dead of the Brain 2 through a confirmed official or otherwise legitimate release.
- Check whether the English localization has actually launched before planning an English-language playthrough.
- Use fan-translation information only as separate technical context, not as proof of official support.
If you want a developer connection
Investigate Marine Philt next. The same-developer identification is the strongest reason to consider it, but verify its mechanics and language before expecting a close match.
If you are comfortable with Japanese or translation uncertainty
Put Necronomicon on the list, checking the current status of any translation and the requirements for the particular release you can access.
If gore is enough of a common thread
Browse Corpse Party, Ballade for Maria and Elle through the PC-98.org Gore listing. Treat each as a separate investigation rather than assuming that the category predicts its story or interface.
Content warnings
Coverage of Dead of the Brain characterizes the series as gory and places it in an exploitation-horror context that includes sexual content and violence. PC Gamer specifically warns that the sequel contains sexual violence. Those warnings are reasons to check a title’s own description and translation notes before playing; they are not a complete content guide for every recommendation in this list.
The Gore category used for three additional titles indicates an emphasis on graphic horror, but it does not establish identical content, intensity or themes. Do not infer that Marine Philt, Necronomicon, Corpse Party, Ballade for Maria or Elle share the sequel’s sexual-violence warning.
A practical play order
- Dead of the Brain 2: the documented direct continuation and the only unambiguous first choice.
- Marine Philt: investigate if same-developer lineage is more important to you than a verified matching premise.
- Necronomicon: consider for a supernatural direction, provided Japanese text or a confirmed translation is acceptable.
- Corpse Party, Ballade for Maria and Elle: use the Gore index to discover further PC-98 horror, then verify each game’s format, language, access and content independently.
This order keeps the strongest evidence first while leaving room for less certain, more exploratory choices. None of the index or community references establishes a ranking of quality, so the list should not be read as a “best games” leaderboard.
