Start with your provider’s patient portal for results, visit summaries, prescriptions, and routine messages. Use electronic access and sharing tools to gather or send records held by different organizations, and keep a personal health record as a concise summary across care settings. Independent health apps may help with tracking or education, but their privacy protections and oversight depend on who operates them and what they do.

The Office of the National Coordinator for Health Information Technology (ONC) describes using electronic access to find and share records, including when changing doctors or helping care for an older parent. What is available depends on what an organization makes accessible. ONC’s Blue Button guidance explains access, checking records, and sharing them.

Find results and routine details in a provider portal
A portal is a website for personal healthcare. Depending on the provider, it may show test results, visit summaries, medication and allergy history, and appointment details. Some portals also support refill requests, forms, billing, and secure messages. If several of your clinicians use the same connected portal, you may be able to see information from more than one provider there; a portal does not automatically combine every record from every clinician.
Register through the provider that offers the portal, then look for sections such as results, medications, visit summaries, or appointments. If a result or record you expect is missing, contact the provider’s office to ask whether it is available electronically and how to obtain it. Use portal messaging for routine questions, not urgent or time-sensitive concerns. MedlinePlus advises contacting the provider’s office by phone for issues that need prompt attention. If you believe you have an emergency, use your local emergency services rather than waiting for a portal reply.
Gather records when care is spread across providers
To get information from different doctors, hospitals, or health plans, check each organization’s portal or ask its records office what electronic access and sharing options it offers. You can review the information for accuracy and completeness, then share relevant records with a new clinician or a caregiver you trust. A single portal may show records from connected providers, but records held elsewhere may require a separate request or access route.

ONC describes safeguards for provider-held records, including access controls, encryption, and audit trails. Those protections apply to the organization’s systems; once you download a copy, protecting that copy is your responsibility. Use device and account protections, and share only with people you choose. ONC’s Blue Button information explains access and sharing, including the distinction between provider-held records and downloaded copies.

Keep a personal health record as your cross-provider summary
A personal health record helps when important details are spread across multiple clinicians and hospitals. It is a summary you maintain, not a replacement for a provider’s medical record. MedlinePlus recommends keeping the key facts together; a paper binder or digital document are both possible formats.

Useful information to include
- Your name and emergency contacts.
- Dates and results of important tests and screenings.
- Major illnesses and surgeries, with dates.
- Medicines and supplements, including dose and how long you take them.
- Allergies and chronic conditions.
- Relevant family health history.
Update the summary when a medicine, allergy, diagnosis, procedure, or important test result changes. A current medication and supplement list is particularly useful to have available when seeing a new clinician or coordinating care.

Some apps are personal organizers or wellness trackers; others connect to provider systems or offer health education. They are not interchangeable with a provider portal, and an app’s privacy obligations depend on its operator and context. HHS Office for Civil Rights explains that federal and state laws, including HIPAA rules, may apply in different situations. It directs developers and others to an interagency Mobile Health Apps Interactive Tool to help identify which laws may apply. Do not assume every independent health app is covered by HIPAA.

Before entering sensitive information, check who provides the app, what information it collects, how it uses or shares that information, and what account or device protections are available. The protections that apply to a provider’s system do not automatically follow information into every app or downloaded file.
FDA oversight also depends on what a software function does and its risk. Some mobile software functions are not medical devices; some device functions may be subject to enforcement discretion; and some functions are subject to FDA oversight. FDA identifies higher-risk examples such as software that diagnoses conditions or controls the function of a traditional medical device. An app that organizes records or offers general wellness features should not be assumed to be FDA-approved or regulated as a medical device. One connected service has a narrower role: MedlinePlus Connect is a free National Library of Medicine service for health organizations and IT providers. It can return relevant MedlinePlus education links in response to diagnosis, medication, lab, or procedure codes submitted by a portal or electronic health record. It is an integration service, not a consumer record manager.

- Look in the provider portal. Check for the result, medication list, visit summary, or routine detail you need.
- Ask about other records. If information from another provider is missing, contact that provider or its records office about electronic access and sharing.
- Check what you receive. Review records for accuracy and completeness, and ask the relevant provider about anything that appears incorrect or incomplete.
- Maintain your summary. Add important medicines, allergies, conditions, procedures, and test dates to your personal health record.
