For an urgent hospital update, call the hospital or use its designated urgent-contact route—not a patient portal message or email. Both channels are generally asynchronous, so even a well-protected message may not be read in time. For non-urgent sensitive information, use the hospital’s approved secure channel; email is not automatically prohibited, but its safeguards depend on how it is sent and handled.

Choose the channel by urgency first

Privacy and speed are separate questions. A portal may require a login, but that does not make it an emergency channel. If a delay could affect care, use the hospital’s published phone number, urgent line, or emergency instructions. In the United States, call emergency services for an emergency when appropriate.

Situation Practical choice Why
Medical emergency or a change needing immediate action Emergency services or the hospital’s designated urgent line The VA warns patients not to send emergencies or urgent needs through online messages. NHS England advises considering whether a delay could increase clinical risk.
Important update needing a quick response, but not an emergency Call the hospital or care team using its published contact route; ask when to expect a reply Response targets vary. The VA’s secure-message service gives a three-business-day response expectation for non-urgent messages; that is specific to the VA, not a general hospital standard.
Non-urgent, sensitive health information The hospital’s authenticated portal or another channel it explicitly approves for sensitive information A login-required portal can limit access compared with ordinary email, but protections depend on the actual service and its configuration.
Routine, non-urgent coordination where the provider has invited email Email may be appropriate with safeguards; verify the address and keep details to what is necessary HHS permits provider-patient email when reasonable safeguards are applied. Patients may ask to use another reasonable confidential channel.

What a portal changes—and what it does not

A patient portal commonly requires a username and password to access health information. That authentication step can help restrict access, but the label “portal” is not a universal security guarantee: organizations choose and configure their own systems and policies. The Office of the National Coordinator for Health Information Technology (ONC) notes that safeguards vary by organization. Use the hospital’s own instructions for its portal or approved secure messaging service.

A secure login also says nothing by itself about when staff will read a message. The VA says non-urgent secure messages to its care teams should receive a response within three business days, counting Monday through Friday, 8 a.m. to 5 p.m., excluding federal holidays. That is a VA service expectation, not a deadline for other hospitals. The VA explicitly directs patients with urgent needs to contact their facility or emergency services rather than send an online message.

Email is not automatically forbidden by HIPAA

In the United States, HIPAA does not categorically ban health care providers from communicating with patients by email. HHS says providers may communicate electronically, including by email, if they apply reasonable safeguards. Those safeguards may include checking that the email address is correct and limiting the information disclosed when using unencrypted email. Patients may also initiate email communication with a provider.

This does not make every email account or message private. Ordinary email can be accessible through shared devices or accounts, forwarded, or displayed in a notification preview. Follow the provider’s instructions, confirm the destination address, and avoid including details that are not needed. If you are uncomfortable with email, ask the provider about another reasonable confidential channel.

Reduce exposure when sending a non-urgent message

  • Use the hospital’s portal or the secure channel it specifically approves for health information.
  • Check the recipient and address before sending, especially if a provider has invited email.
  • Include only information needed for the non-urgent request; avoid putting sensitive details in a subject line or notification preview.
  • Follow the organization’s privacy guidance. NHS England notes that message content can appear on a locked screen and recommends minimizing information where appropriate.
  • If the issue becomes urgent or a reply is needed sooner than the channel’s stated response time, switch to the hospital’s phone or urgent-contact route.

Do not confuse a message reply with a records-access deadline

HHS’s HIPAA access guidance gives covered entities up to 30 calendar days to act on a request for access to medical records, with a possible extension under specified conditions. That is a records-access rule—not a response target for portal messages, email, or urgent hospital updates. For an update about care, use the response instructions for the specific hospital and contact route.

Where the guidance applies

The HHS and ONC guidance cited here concerns the United States; the NHS England guidance applies to health and care services in England. Legal obligations, approved communication methods, and response procedures vary by jurisdiction and institution. Your hospital’s published instructions determine which channels it monitors and how quickly it expects to respond.
