Fluctuations may improve after adjusting timing or the combination of medicines. If oral treatment remains inadequate, clinicians may discuss pump-delivered medication, deep brain stimulation (DBS), or a selected lesion procedure, alongside rehabilitation and supportive care. The right choice depends on which symptoms are limiting you, how you respond to levodopa, other health factors, and what is authorized and available where you receive care.
First establish what “no longer works” means
Parkinson’s is progressive, and the benefit from a dose can become shorter or less predictable. You may experience “off” periods as a dose wears off, sudden changes between mobility states, or troublesome dyskinesia (involuntary movements) when medicine is working. A change in control does not by itself prove that every medicine has failed or that surgery is required.

Before choosing an advanced treatment, the specialist will usually clarify the timing of symptoms, dose timing, side effects, daily function, and the response to levodopa. Keep a short diary for several days if your team asks for one: record each dose, when it starts helping, when symptoms return, involuntary movements, falls or freezing, meals that appear to affect absorption, sleep, and activities you could or could not do. Bring the actual medication list, including over-the-counter products and supplements. Do not stop or alter Parkinson’s medicines without instructions from your care team.
Medication review and optimization
Best medical therapy remains the foundation of care, including for people with advanced disease. A clinician may alter dose intervals, formulations, or combinations to reduce off time or dyskinesia, while balancing sleepiness, hallucinations, low blood pressure, nausea, impulse-control problems, and other adverse effects. The plan is individualized and may need repeated adjustment as symptoms change.

What to discuss at the review
- Whether symptoms consistently return before the next dose and whether a timing or formulation change could help.
- Which symptoms are medication-responsive and which may need a different approach.
- Whether dyskinesia, fluctuations, blood-pressure symptoms, sleepiness, mood, thinking, or hallucinations are limiting treatment.
- How exercise, physiotherapy, occupational therapy, speech therapy, nutrition support, and mental-health care can be integrated.
Advanced drug-delivery options
When tablets or capsules alone do not provide consistent control, clinicians may offer medication delivered intermittently or continuously. These treatments still require specialist selection, training, monitoring, and practical support.
Intermittent apomorphine injection
Apomorphine is a dopamine agonist that can be injected under the skin when an “off” episode occurs. It is intended as an as-needed rescue option for selected people whose oral treatment is not controlling episodes reliably. A specialist team assesses suitability and teaches administration; anti-nausea management and monitoring may be needed.
Continuous subcutaneous apomorphine infusion
A small pump delivers apomorphine continuously through a subcutaneous line. Continuous delivery can reduce fluctuations for some people who respond to dopaminergic treatment but have inadequate control with oral schedules. It involves wearing and managing a pump, caring for the infusion site, adjusting settings with the clinical team, and attending follow-up. Eligibility and local service availability vary.

Levodopa–carbidopa intestinal gel
Levodopa–carbidopa intestinal gel is delivered through a tube into the small intestine by an external pump. By providing continuous levodopa exposure, it may be considered for severe motor fluctuations when tablets do not last reliably. Tube placement, stoma or site care, pump operation, and troubleshooting are part of the treatment burden, so the team must explain the procedure and ongoing support requirements.
Continuous foslevodopa–foscarbidopa
NICE technology appraisal TA934, published on 29 November 2023, recommends foslevodopa–foscarbidopa as an option for adults with advanced, levodopa-responsive Parkinson’s disease who have severe motor fluctuations and hyperkinesia or dyskinesia when medicines are not working well enough, and who cannot have apomorphine or DBS or whose symptoms are no longer controlled by those treatments. NICE describes administration by continuous subcutaneous infusion. This is a specific UK recommendation with stated conditions; it should not be assumed to apply to every patient or health system. Confirm current authorization, commissioning, coverage, and local availability with your clinical team.
Deep brain stimulation (DBS)
DBS uses electrodes implanted in targeted brain areas and connected to a pulse generator. Electrical stimulation can be programmed after implantation and adjusted as symptoms and needs change. It is primarily used for selected movement symptoms, not every feature of Parkinson’s.

NICE recommendation 1.8.3 states: “Consider deep brain stimulation for people with advanced Parkinson’s disease whose symptoms are not adequately controlled by best medical therapy.” NICE also advises not offering DBS when symptoms are adequately controlled by best medical therapy. A movement-disorders service evaluates likely benefit, levodopa response, cognition, psychiatric health, balance, speech, general medical fitness, goals, and ability to manage follow-up. A good levodopa response can support candidacy, but the assessment is broader than a single test.

- It can improve selected medication-responsive movement problems, such as fluctuations, tremor, rigidity, or slowness, depending on the target and the individual.
- It is less likely to correct symptoms that do not respond to levodopa, and it does not cure Parkinson’s or stop progression.
- Speech, balance, thinking, mood, swallowing, and autonomic symptoms require separate assessment; some may not improve and can occasionally become more difficult.
- Implantation carries surgical and device-related risks, and ongoing programming, battery management, and medication adjustments are required.
Focused ultrasound and other lesion procedures
Focused ultrasound directs energy through the skull to a small brain target, creating a lasting lesion. Other lesion procedures also permanently destroy a selected target. Unlike DBS, a lesion is not adjustable or reversible in the same way, so target selection and risk discussion are crucial. These procedures are aimed at selected symptoms and are not a cure.
The Parkinson’s Foundation describes focused ultrasound as an option for selected movement symptoms, including medication-resistant tremor. Its US-focused information reports FDA milestones for tremor-dominant Parkinson’s tremor in 2018, other Parkinson’s movement symptoms in 2021, and treatment on both sides with procedures at least six months apart in 2025. These statements are specific to US regulatory decisions and should be checked against current FDA information and local rules. Access, approved targets, evidence, and insurance coverage differ by country and center. Focused ultrasound and other lesion procedures are less common than DBS or intestinal levodopa delivery, and not every technique has the same evidence or suitability.

How clinicians compare the options
There is no universal ranking. A specialist weighs the following dimensions together:
| Decision factor | Questions to answer |
|---|---|
| Symptoms and goals | Is the main problem off time, tremor, dyskinesia, freezing, falls, or another symptom? Which are likely to respond? |
| Levodopa response | Do movement symptoms improve when levodopa is working? Non-responsive symptoms may need a different strategy. |
| Delivery method | Would tablets, an intermittent injection, a continuous pump, implanted stimulation, or a lesion procedure fit daily life? |
| Medical and neuropsychiatric status | Are cognition, mood, hallucinations, balance, speech, sleep, blood pressure, or other illnesses likely to alter benefit or risk? |
| Treatment burden | Can you manage a pump, tubing, a device, programming visits, wound or site care, or long-term follow-up? |
| Access | Is the option authorized, offered by a qualified center, and covered in your health system? |
Questions to take to the specialist appointment
- Are my symptoms returning between doses, and could timing or the treatment combination be adjusted?
- Which of my symptoms is each option most likely to improve, and which is unlikely to respond?
- Am I a candidate for intermittent or continuous apomorphine, intestinal levodopa delivery, foslevodopa–foscarbidopa, DBS, or a lesion procedure?
- What examinations, medication-response testing, imaging, psychological assessment, or other evaluation would establish candidacy?
- What are the common and serious adverse effects, practical demands, and follow-up schedule?
- How would the option affect current medicines, driving, work, travel, exercise, sleep, and caregiver responsibilities?
- Which treatments are available and covered where I receive care, and what happens if the device or infusion fails?
Rehabilitation and supportive treatment still matter
Advanced medication or a procedure does not replace rehabilitation. Parkinson’s Foundation and Parkinson’s UK identify exercise, physiotherapy, occupational therapy, nutrition support, communication therapy, and mental-health care as parts of ongoing management. A physiotherapist can address gait, balance, strength, and falls prevention; an occupational therapist can adapt activities and the home; speech and language therapy can address communication and swallowing; dietetic support can help match nutrition to symptoms and medication routines. Mental-health support is relevant when anxiety, depression, apathy, or caregiver strain affects quality of life.
Supportive therapies should be matched to the symptoms that remain after medication changes or an advanced treatment. Keep reviewing goals with the team rather than assuming a procedure will address every difficulty.
Best Value
When to seek a second specialist opinion
A movement-disorders specialist can provide a second assessment when fluctuations are difficult to characterize, proposed options differ, or you are unsure whether a pump, DBS, or lesion procedure fits your goals. Ask what evidence supports the proposed target or treatment, what symptoms are unlikely to improve, and how the center handles complications and long-term follow-up. Do not delay prescribed medicines while arranging another opinion.
Bottom line for a changing treatment plan
Loss of reliable control calls for a structured reassessment. Medication timing or combinations may help; if they do not, selected people may benefit from intermittent or continuous apomorphine, continuous levodopa-based infusion, DBS, or a lesion procedure. Candidacy is individual, and regulatory status and coverage are location-dependent. Continue exercise, rehabilitation, and supportive care, and make the decision with the treating team rather than changing medication independently.
