Ovarian Endometrioma Treatment Options for People Who Want to Preserve Fertility

Usually, no. Surgery to remove an ovarian endometrioma is not routinely recommended before in-vitro fertilization (IVF) or another assisted-reproduction treatment simply to improve the chance of a live birth. The 2022 European Society of Human Reproduction and Embryology (ESHRE) guideline found no live-birth benefit and warns that surgery can reduce ovarian reserve. An operation may still be reasonable for severe endometriosis pain or when the cyst prevents access to follicles during egg retrieval.

The right plan depends on your immediate goal (pain relief, trying naturally, starting assisted reproduction, or preserving eggs for later), age and pregnancy timing, ovarian reserve, whether you have had previous ovarian surgery, whether disease affects both ovaries, other infertility factors, and your preferences. A gynecologist experienced in endometriosis and a reproductive endocrinology team should make the decision with you.

What an ovarian endometrioma means for fertility decisions

An endometrioma is an ovarian cyst caused by endometriosis. Treatment is not a single choice between “operate” and “do nothing.” Clinicians balance symptom control and access to treatment against the possibility that operating on ovarian tissue may reduce the number of eggs that remain.

Start by identifying the next objective:

  • Relieve pain: surgery can be considered when pain is significant or inadequately controlled.
  • Try to conceive without treatment: operative laparoscopy may be considered, but comparative studies have not established how much it improves pregnancy rates.
  • Proceed to IVF or another ART cycle: routine removal solely to raise live-birth rates is not recommended.
  • Preserve future fertility: discuss egg or embryo freezing in extensive or higher-risk ovarian disease, while recognizing that the benefit for endometriosis is still uncertain.

Your clinician should also account for previous operations, bilateral endometriomas, recurrence in the other ovary after earlier surgery, ovarian-reserve results, other causes of infertility, and an estimated Endometriosis Fertility Index (EFI) when relevant.

Do I need surgery for an ovarian endometrioma before IVF?

Not routinely. ESHRE’s 2022 recommendation states that clinicians should not routinely operate on an ovarian endometrioma before ART to improve live-birth rates because current evidence shows no benefit and surgery is likely to have a negative effect on ovarian reserve.

The guideline summarizes two meta-analyses rather than new trials conducted in 2022:

Evidence summarized by ESHRE What was compared Finding What it means
Hamdan and colleagues, 2015; five studies, 655 women Surgery before ART versus conservative management No live-birth advantage. Clinical pregnancy, average oocytes retrieved and cycle cancellation also did not differ. Antral-follicle-count and stimulation-dose findings suggested possible reserve harm. Removing the cyst did not improve the measured ART outcomes and may make ovarian stimulation harder.
Nickkho-Amiry and colleagues, 2018; seven studies Surgery before ART versus no surgery Odds ratio for live birth 1.08 (95% confidence interval 0.80–1.45), not statistically significant. The result does not demonstrate a live-birth benefit from routine pre-ART surgery.

These averages cannot predict an individual cycle. A cyst that is painless and does not interfere with follicle access is often monitored while the fertility team proceeds with ART. The decision changes when the cyst causes substantial symptoms, appears concerning for another diagnosis, or physically blocks the needle path needed for egg retrieval.

When surgery before ART can still make sense

Endometriosis-associated pain

ESHRE allows surgery before ART when reducing endometriosis-associated pain is an important goal. Pain severity, impact on daily life, response to medication and your willingness to accept a potential reserve reduction should be discussed separately from the question of IVF success.

Access to follicles during egg retrieval

A large or strategically positioned endometrioma can make it difficult or unsafe to reach follicles. In that situation, an operation may improve physical access for retrieval, but this is a technical and individualized judgment, not a universal cyst-size rule. The 2022 ESHRE framework replaced the older practice of treating 4 cm as an automatic threshold.

Other clinical concerns

Your team may also consider a suspicious appearance, infection or complications, although the decision must be based on your examination and imaging. Do not infer that a cyst should be removed merely because it exists.

Can I get pregnant naturally with an endometrioma?

Natural conception is possible for some people with an endometrioma. ESHRE says operative laparoscopy for endometrioma-associated infertility may be considered because it might increase the chance of spontaneous pregnancy. However, the guideline also notes that comparative studies establishing this benefit are absent. Surgery is therefore an option to weigh, not a guaranteed fertility treatment.

The balance is especially personal when you are trying naturally. Waiting to see whether pregnancy occurs preserves the ovary from an operation but uses time; operating may address pain or disease while potentially reducing reserve. Age, how long you have been trying, ovarian-reserve findings, sperm factors, tubal status and the estimated EFI all influence whether a period of expectant management, surgery or ART is most sensible.

Can endometrioma surgery affect my egg reserve?

Yes. Ovarian surgery can remove or damage healthy ovarian tissue along with the cyst. ESHRE’s evidence review found no ART live-birth gain from routine pre-ART surgery and signals of lower ovarian reserve, including changes in antral follicle count and medication requirements during stimulation. The size, location and laterality of the disease, surgical technique, prior operations and the amount of functioning ovary remaining all matter.

Risk is not identical for everyone. Bilateral disease, a previous operation on one ovary, or recurrence in the opposite ovary leaves less reserve to protect. Ask the team to explain your current reserve, the expected impact of the proposed procedure and how a second operation would be handled if the cyst recurs.

Which operation is used?

Cystectomy versus drainage and coagulation

When surgery is chosen, ESHRE recommends cystectomy rather than simply draining the cyst and coagulating its lining because cystectomy reduces endometrioma recurrence and endometriosis-associated pain. The recommendation must still be balanced against ovarian-reserve risk.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Cystectomy and carbon-dioxide laser vaporization

ESHRE says cystectomy and carbon-dioxide laser vaporization can both be considered. Recurrence rates appear similar beyond the first year, while early recurrence may be lower after cystectomy. The guideline does not establish one technique as best for every ovary; the surgeon’s expertise, cyst characteristics and reserve concerns should guide the choice.

Excision versus ablation in NICE guidance

NICE advises considering excision rather than ablation for endometriomas while taking the person’s fertility wishes and ovarian reserve into account. This complements, rather than replaces, individualized counseling: the least damaging approach for one ovary may not be the same for another.

Should I freeze my eggs before endometrioma surgery?

Discuss fertility preservation before surgery when ovarian endometriosis is extensive or your situation places reserve at higher risk. ESHRE specifically recommends discussing the pros and cons in extensive ovarian endometriosis and highlights examples such as bilateral endometriomas or a recurrence in the opposite ovary after previous unilateral surgery.

Egg or embryo freezing may preserve reproductive options, but ESHRE states that the true benefit of fertility preservation in endometriosis remains unknown. It does not guarantee a future birth or prevent infertility. The decision involves your age, current reserve, how urgently surgery is needed, treatment burden, cost and local availability. A reproductive-medicine specialist can explain whether preservation before an operation would delay needed treatment and what outcome can realistically be expected.

A practical decision framework

  1. Define the immediate goal. State whether the priority is pain relief, spontaneous conception, ART access or preserving fertility for later. Different goals lead to different thresholds for surgery.
  2. Review ovarian reserve. Ask which measurements were used, what they imply for stimulation and how surgery could change them. Request a plan for reassessment if an operation is performed.
  3. Map prior treatment. Tell the team about every ovarian operation, the side treated and any recurrence. A repeat operation can carry different consequences from a first procedure.
  4. Check both ovaries and the rest of the fertility picture. Bilateral endometriomas, tubal disease, sperm factors and other diagnoses may shift the balance toward earlier ART or fertility preservation.
  5. Assess symptoms and access. Document pain severity and whether the cyst would obstruct follicle retrieval. Do not use cyst diameter alone as the decision rule.
  6. Discuss timing and age. Compare the time needed for surgery and recovery with the decline in fertility associated with advancing age and your desired pregnancy schedule.
  7. Compare techniques. Ask why cystectomy, laser vaporization, excision or another approach is proposed, how recurrence and pain are expected to change, and what ovarian tissue might be at risk.
  8. Record your preference. There is no guideline instruction that overrides informed consent. Your tolerance for pain, surgery, delay, recurrence risk and possible reserve loss belongs in the final decision.

How the main pathways differ

Pathway Primary purpose Potential advantages Main trade-offs
Monitor and try naturally Spontaneous conception without immediate surgery Avoids surgical injury and recovery time Pregnancy may not occur; time passes while reserve and age remain important
Operative laparoscopy before trying naturally Address pain or possibly improve natural conception May relieve symptoms; ESHRE says pregnancy benefit is possible No comparative evidence proves a fertility benefit; ovarian reserve may fall
Proceed directly to ART Use IVF or another ART cycle without routine cyst removal Avoids an operation that has not improved live-birth rates in studies Retrieval may be technically difficult if the cyst blocks follicle access; pain remains untreated
Surgery before ART for a specific indication Relieve severe pain or enable follicle access Can address the stated problem before treatment Recovery, recurrence and possible reserve loss; no proven routine live-birth benefit
Fertility preservation before surgery Store eggs or embryos before a higher-risk operation May retain additional reproductive options Benefit in endometriosis is uncertain; adds cost, medication, procedures and possible delay

Questions to take to your specialists

  • Is surgery being proposed for pain, follicle access, a diagnostic concern or a presumed IVF success benefit?
  • What evidence supports operating in my specific situation?
  • How might this procedure affect reserve in each ovary, especially if I have had prior surgery?
  • Would you recommend cystectomy, carbon-dioxide laser vaporization, excision or ablation, and why?
  • What is the expected recovery time, and would it change my planned ART schedule?
  • Do bilateral disease or a contralateral recurrence make fertility preservation worth discussing now?
  • How do my age, other infertility factors and estimated EFI affect the choice between trying naturally and ART?
  • What is the plan if the endometrioma recurs?

What the guidelines do—and do not—promise

ESHRE’s guideline was issued on 2 February 2022 and replaced its earlier guideline. Its recommendation against routine surgery applies specifically to improving live-birth rates before ART; it does not say that surgery is never appropriate. NICE offers complementary advice on choosing excision or ablation with fertility wishes and ovarian reserve in mind. A patient-facing RCOG summary describes expectant and surgical management as broad options for people wishing to conceive and advises discussing fertility preservation.

Guidelines cannot calculate your personal chance of pregnancy or the exact reserve loss from an operation. Bring your imaging, prior operative reports and fertility results to an endometriosis specialist and a reproductive endocrinologist so the two goals—controlling disease and protecting future fertility—are considered together.
