Osteoarthritis treatment options: medication, physical therapy, and surgery compared

For most adults with osteoarthritis (OA), treatment works best as a sequence rather than a single cure: education, tailored exercise and—when relevant—weight management form the foundation; medicines or injections can make activity more manageable; joint replacement is considered when pain and loss of function continue to substantially affect quality of life despite suitable non-surgical care.

The recommendations below combine the UK National Institute for Health and Care Excellence (NICE) guideline for adults with OA and American Academy of Orthopaedic Surgeons (AAOS) guidance that applies specifically to knee OA. They are general guidance, not an individual prescription. Your joint involved, other medical conditions, current medicines, pregnancy status, goals and local health system all affect the safest choice.

How the main options compare

Option Main job What the guidance supports Important limits
Therapeutic exercise or physical therapy Build or preserve strength, aerobic capacity, movement and day-to-day function NICE recommends exercise tailored to every person with OA; AAOS recommends supervised, unsupervised or aquatic exercise over no exercise for knee OA. Symptoms can temporarily increase when activity starts. Long-term, regular participation matters more than a short course.
Weight management when relevant Reduce load-related symptoms and improve function NICE says any weight loss is likely to help and that 10% loss is likely to help more than 5%; AAOS recommends sustained loss for people with overweight or obesity and knee OA. Body weight should not be used as a prerequisite for referral or as a reason to deny care.
Topical NSAID Local symptom relief, especially for knee OA NICE says offer one for knee OA; AAOS strongly recommends topical NSAIDs for knee OA when not contraindicated. Product labels, skin reactions, other medicines and medical conditions still need checking.
Oral NSAID Additional pain relief when topical treatment is ineffective or unsuitable NICE says consider one after reviewing gastrointestinal, kidney, liver and cardiovascular risks, and use gastroprotection while taking it. Use the lowest effective dose for the shortest possible time, with clinician or pharmacist review when risks or interactions exist.
Corticosteroid joint injection Short-term symptom reduction that may enable exercise NICE and AAOS support selected use for short-term relief. It is not a cartilage-restoring treatment; benefit is temporary and suitability depends on the joint and individual risks.
Joint replacement Replace a severely symptomatic joint when non-surgical care is ineffective or unsuitable NICE supports considering referral for hip, knee or shoulder OA when symptoms substantially affect quality of life. Referral is based on symptoms and function, not an X-ray result or one numerical score alone.

Guidelines differ in scope. NICE NG226 covers adults and several joints, while the AAOS non-arthroplasty and surgical documents focus on knee OA. A knee recommendation should not automatically be presented as evidence for hip, hand or shoulder disease.

Start with the foundation: exercise, education and practical support

Therapeutic exercise is core treatment

NICE’s recommendation is explicit: “For all people with osteoarthritis, offer therapeutic exercise tailored to their needs (for example, local muscle strengthening, general aerobic fitness).” The plan can combine strengthening around the affected joint with general aerobic activity and movement practice. AAOS gives a similar direction for knee OA, recommending supervised, unsupervised or aquatic exercise rather than no exercise.

What you get: a folding cane, a travel bag, a friendly customer service and a brilliant using experience.

A physiotherapist can match the programme to pain, weakness, balance, work demands and available equipment. Supervision is optional rather than mandatory, but it can help with technique, progression and confidence. The useful test is whether the programme is safe, repeatable and improves the activities you want to do—not whether it feels effortless on the first session.

Some people hurt more when they begin or increase activity. That does not automatically mean the joint is being damaged. A clinician can help adjust intensity, range, rest periods and progression so that temporary soreness does not end the programme. Medication is best viewed as support that makes consistent movement possible, not as a replacement for it.

Weight management, without making it a gatekeeper

For people with OA who have overweight or obesity, NICE states that any amount of weight loss is likely to be beneficial and that losing 10% of body weight is likely to be better than losing 5%. This is a recommendation about expected benefit, not a guarantee for an individual. Sustainable nutrition, activity and behavioral support are more useful than a short crash diet.

Weight should be discussed without stigma. NICE says age, sex or gender, smoking, comorbidities and overweight or obesity alone should not exclude someone from referral for joint replacement. A person can work on weight management while still receiving assessment and symptom treatment.

Walking aids and devices

NICE says a walking aid can be considered for lower-limb OA. AAOS describes moderate confirmation that canes can relieve knee-arthritis pain and improve function. A cane is an optional tool, not a universal requirement, and the evidence does not establish that one consumer model is medically superior. Fit, height, hand comfort, the side used and safe technique should be checked by a clinician or therapist.

NICE does not recommend routinely offering insoles, braces, tape, splints or other supports. They may be considered when instability or abnormal loading is present, exercise alone is ineffective or unsuitable without the aid, and the device is likely to improve movement and function. AAOS says a brace could help selected people with knee OA; that narrower knee finding should not be generalized to every joint.

Manual therapy and treatments with insufficient evidence

For hip or knee OA, NICE considers manual therapy only alongside therapeutic exercise; it should not replace the exercise programme. NICE advises against acupuncture or dry needling and listed electrotherapies, including transcutaneous electrical nerve stimulation (TENS), because evidence was insufficient to show benefit. These recommendations do not mean every person experiences the same response, but they do mean such treatments should not displace interventions with stronger guideline support.

Medication: match the drug to the joint and the person’s risks

Topical non-steroidal anti-inflammatory drugs

For knee OA, NICE says to offer a topical NSAID, and AAOS strongly recommends topical NSAIDs for knee OA when there is no contraindication. Applying medicine to the skin can be an appropriate first pharmacological step because it targets local symptoms without automatically exposing the whole body to the same level of medicine as an oral dose.

“Topical” does not mean risk-free. Follow the specific product label, avoid use on damaged skin unless instructed, and ask a pharmacist or clinician about allergies, other NSAIDs, anticoagulants and medical conditions. NICE says topical treatment can also be considered for other affected joints, but the strength and wording of the recommendation are not identical to the knee recommendation.

Oral NSAIDs

If a topical NSAID is ineffective or unsuitable, NICE says an oral NSAID may be considered after accounting for gastrointestinal, kidney, liver and cardiovascular toxicity. Age, pregnancy, previous ulcers or bleeding, high blood pressure, heart disease, kidney disease and interacting medicines can change the balance of benefit and harm. NICE also recommends gastroprotection while taking an oral NSAID.

Use the lowest effective dose for the shortest possible time. A pharmacist or prescriber should check the complete medicine list, including over-the-counter painkillers and supplements, before starting or combining products. Do not assume that a medicine that was safe for a past episode is safe now if your health or other medicines have changed.

Paracetamol, opioids and supplements

NICE does not recommend routine paracetamol or weak opioids for OA. They may be considered only infrequently and for short-term use when other pharmacological options cannot be used or have not worked. NICE advises against strong opioids for OA.

  • Self-Standing: Rehand all terrain pivot quad cane uses a self-standing design, which allows you to easily rest it upright and no longer lean it against the wall whenever you need a break.
  • Easy on The Go: Being constructed with durable & lightweight aluminum frame, with a collapsible design, this walking stick can be wrapped up in seconds and easily can be fitted into a briefcase, wheelchair bag, or even purse. A premium hook & loop fastener tape on the cane shaft comes as standard, which can be used to tie up the cane conveniently. All thoughtful designs make carrying and storage no headache anymore
  • Up To 8 Adjustable Height: Rehand adjustable cane fits for people from 5'(152cm) to 6'5''(195cm). 8 adjustable height with one-inch increments makes almost all seniors, elderly and injured persons can easily adjust the cane to proper height fitting user's height. The adjusting range of walking stick itself is from 2'7'' (78cm) to 3'2'' (96cm)

NICE also advises against glucosamine. There is no guideline basis here for claiming that a supplement restores cartilage or reverses OA. Discuss any supplement with a clinician or pharmacist, particularly if you take anticoagulants or medicines for blood pressure, diabetes or other long-term conditions.

Injections: a limited role, not a cure

Corticosteroid injections

NICE says an intra-articular corticosteroid injection may provide short-term relief lasting about 2 to 10 weeks. It may be considered when other medicines are ineffective or unsuitable, or when temporary relief would help someone take part in therapeutic exercise. AAOS likewise says steroid injections could provide short-term relief for knee OA.

The time-limited benefit is the key qualification. An injection does not rebuild cartilage, correct a deformity or remove the need for a longer-term activity plan. The clinician must assess the target joint, diagnosis, infection risk, diabetes and other factors before recommending it.

Hyaluronan injections

NICE advises against intra-articular hyaluronan injections for OA. AAOS says hyaluronic-acid injections are not for routine knee OA use. Marketing language about “lubricating” or “regenerating” a joint should not be treated as proof of a clinically meaningful or durable result.

When surgery becomes part of the discussion

Referral for joint replacement

NICE says to consider referral for hip, knee or shoulder replacement when joint symptoms—such as pain, stiffness, reduced function or progressive deformity—substantially affect quality of life and non-surgical management, including exercise, weight loss and pain relief, is ineffective or unsuitable.

This is a shared clinical decision, not an automatic consequence of an X-ray. The sources do not establish one age cutoff, imaging grade or numeric severity threshold that applies to everyone. A referral discussion should cover the activities you can no longer do, night or rest symptoms, walking and work limitations, previous treatments, medical risks, expected rehabilitation and your goals.

NICE specifically says not to exclude someone from referral solely because of age, sex or gender, smoking, comorbidities or overweight/obesity. Those factors may affect preparation and surgical risk, but they are not on their own a reason to withhold assessment.

Arthroscopy is not the same as replacement

NICE advises against arthroscopic lavage or debridement for OA. AAOS also advises against lavage and debridement for primary knee OA. That does not mean every arthroscopic procedure is useless: AAOS allows arthroscopic partial meniscectomy for selected people with a meniscal tear and mild to moderate knee OA after physical therapy or other non-surgical care has failed. The indication is the tear in that clinical context, not routine “cleaning” of an arthritic joint.

AAOS has a separate surgical-management guideline for knee OA. It emphasizes that the final procedure decision must account for the patient’s concerns and local resources. Surgery carries operative and recovery burdens, and individual outcomes vary; a consultation is needed to translate general recommendations into a personal choice.

A practical decision path

  1. Confirm the problem and the joint. OA recommendations are joint-specific, and knee evidence should not be transferred automatically to hip, hand or shoulder symptoms.
  2. Set functional goals. Define the activities that matter—walking a particular distance, climbing stairs, working, sleeping or exercising—so improvement is measured in daily life as well as pain scores.
  3. Build a tailored exercise plan. Include strengthening and aerobic fitness, with supervision if technique, confidence or progression is difficult.
  4. Add weight-management support when relevant. Any sustained loss may help; NICE considers 10% more beneficial than 5% while stressing that weight is not a referral gatekeeper.
  5. Use symptom relief to enable activity. For knee OA, discuss a topical NSAID first when appropriate. Consider an oral NSAID only after a review of gastrointestinal, kidney, liver, cardiovascular, pregnancy and interaction risks, with gastroprotection as advised.
  6. Reserve injections for selected short-term needs. A corticosteroid injection may be useful for 2 to 10 weeks when other medicines are unsuitable or when relief is needed to participate in exercise; hyaluronan is not recommended routinely.
  7. Review function and quality of life. If symptoms remain substantially limiting despite suitable non-surgical care, discuss referral for replacement rather than waiting for an arbitrary imaging score or age.

What the guidelines do—and do not—tell you

NICE NG226 was published on 19 October 2022 and covers adults with OA across joints. The AAOS non-arthroplasty knee guideline was adopted on 31 August 2021, and its surgical knee guideline overview was published on 2 December 2022. Recommendations have different strengths and populations; none is a personal prescription.

AAOS reports that its knee guideline evidence process reviewed 15,103 abstracts, recalled 2,426 articles for full review and analyzed 617 abstracts after full-text review and quality analysis. That scale indicates a formal evidence process, not a promise that every intervention works equally well for every patient.
