Short answer: Current randomized evidence does not show a universal winner among transcrestal osseodensification (OD), osteotome-mediated sinus floor elevation (OMSFE), and lateral-window maxillary sinus floor augmentation (MSFA). A 2025 review of six randomized trials found similarly high short-term implant survival. OD produced higher measured implant stability and generally less operative and analgesic burden, while the lateral-window approach created more endo-sinus bone. The studies were short, often at high risk of bias, and graded as low-certainty, so the choice still depends on residual bone, the amount of vertical gain required, anatomy, and the operator’s assessment.
What the three techniques are trying to accomplish
The posterior maxilla often has limited vertical bone because of sinus pneumatization and bone resorption. Sinus-floor elevation creates space beneath the Schneiderian membrane so an implant can be placed with adequate surrounding bone. The three approaches differ mainly in how they reach the sinus and how much grafted or compacted bone they can create. The intended endpoint is not simply a higher radiographic floor: the clinician must obtain a stable implant in a prosthetically useful position while protecting the membrane and managing the sinus space.

Osseodensification (OD)
OD is a transcrestal procedure: access is made through the implant osteotomy rather than through the side wall of the maxilla. The 2025 three-technique review describes specially designed burs rotated counterclockwise in a pumping motion with copious irrigation. This action compacts autogenous bone particles beneath the Schneiderian membrane and gradually elevates it. The burs are a specific equipment category, often described commercially as osseodensification burs, but the literature reviewed here does not establish that one brand or kit is superior.

The operator still has to judge the sinus shape, the remaining ridge, the implant trajectory and whether the membrane is behaving safely during preparation.

Osteotome-mediated sinus floor elevation (OMSFE)
OMSFE is also transcrestal. Osteotomes are used through the crestal osteotomy to fracture or open the sinus floor and elevate the membrane. Because there is no lateral bony window, the access is more limited than in a lateral approach. Comparisons with OD therefore need to include not only bone gain and implant survival, but also surgical time, postoperative symptoms, and complications. The force, sequence and feel of osteotome use are operator-dependent, so studies performed by different clinicians may not be directly interchangeable.
Lateral-window sinus floor augmentation (MSFA)
MSFA uses a lateral approach. The clinician creates an opening in the lateral sinus wall, elevates the membrane under direct access, and places graft material in the created space. This route can accommodate a larger grafted compartment and, in the 2025 three-technique review, produced greater endo-sinus bone gain than OD. Greater radiographic bone gain does not automatically translate into better implant survival, and the approach involves a larger access site. Direct access may nevertheless be useful when the planned augmentation is extensive or when a transcrestal route would not provide enough working room.
How each operation is approached
A typical OD sequence
- The implant position and sinus anatomy are planned from three-dimensional imaging, including the residual ridge height, ridge width, membrane appearance and any septa or unusual contours.
- The osteotomy is started through the crest. Specially designed burs are used in a counterclockwise pumping motion with copious irrigation, according to the technique description in the 2025 review.
- Bone particles are compacted beneath the Schneiderian membrane while the sinus floor is gradually elevated. The clinician monitors resistance and membrane behavior rather than assuming that a preset depth is safe.
- If the membrane and primary stability are acceptable, the implant may be placed in the same operation. If not, the plan may change to additional grafting, a staged approach or another access route.
This sequence explains why OD can have a relatively small access field and why it may be attractive when the implant can be stabilized immediately. It does not mean every transcrestal case is brief or that simultaneous placement is always appropriate.

- The osteotomy is prepared to the sinus floor while preserving the planned implant axis.
- Osteotomes are introduced through the crest to open or fracture the sinus floor and elevate the membrane.
- Graft material may be used in the created space, depending on the treatment plan and the amount of elevation required.
- The implant is placed immediately only when the clinician can obtain adequate primary stability and is satisfied with membrane and graft conditions.
The transcrestal route avoids a lateral osteotomy, but the membrane is less directly visible than through a lateral window. That limited visibility is an important trade-off when comparing comfort or access with control over a large augmentation.
A typical MSFA sequence
- A lateral opening is designed over the sinus wall at the planned implant site.
- The membrane is carefully elevated through that window, creating a compartment under direct access.
- Graft material is placed into the compartment to support the intended vertical augmentation.
- The implant is inserted at the same appointment or after graft healing, depending on residual bone, primary stability, membrane status and the overall plan.
A lateral window permits a larger grafted space and direct visualization, which helps explain the greater endo-sinus bone gain reported against OD. The price of that access is an additional bony opening and a broader surgical field; the patient should ask how those features affect the planned recovery.
What the best comparative evidence shows
The most direct synthesis is a 2025 systematic review and meta-analysis that searched through March 19, 2025 and included six randomized controlled trials. The trials were short-term, were judged to have high risk of bias, and the certainty of evidence was low. Within those limits, implant survival was comparably high across OD, OMSFE, and MSFA.
OD had statistically higher implant stability quotient (ISQ) values at implant placement and at abutment connection than the osteotome and lateral-window comparators in that review. It was also associated with shorter surgery, fewer reported complications, and fewer days of analgesic use. These are review-level findings, not promises about an individual operation. The review found less endo-sinus bone gain with OD than with MSFA, while the difference between OD and OMSFE was not statistically significant.

The review’s conclusion is appropriately cautious: Long-term randomized controlled trials are, therefore, needed before one treatment may be considered superior to another.
That conclusion matters because high short-term survival can coexist with uncertainty about long-term marginal bone, prosthetic maintenance, biological complications, sinus health and implant loss.

The separate 2025 International Journal of Implant Dentistry review synthesized 13 studies covering 519 sites for the OD-versus-osteotome comparison. Its numerical estimates are useful for understanding the literature, but they do not establish that OD always causes fewer complications. Another 2025 review found heterogeneous designs and inconsistent quantitative comparisons and called for standardized research. Differences in case selection, operator experience, definitions of complications, follow-up, and measurement methods can move a pooled percentage substantially.
The figures should also be kept in context. A mean bone gain of 3.37 mm is a summary of selected study sites, not a promise that a patient will gain exactly 3.37 mm. Likewise, a 2.78% pooled complication estimate cannot be used as a personal risk calculator or compared casually with a clinic’s own rate unless the definitions and follow-up are the same.
How the access routes change the operation
Transcrestal access: OD and OMSFE
Both transcrestal methods use the planned implant osteotomy as the route to the sinus floor. That usually means a smaller access field than a lateral window and can support implant placement during the same surgical episode when the clinician judges primary stability and membrane conditions adequate. The trade-off is limited direct visibility of the membrane and a smaller working space. The amount of elevation that can be achieved safely depends on anatomy and technique; the reviews do not validate one residual-height cutoff that applies to every patient or operator.
Transcrestal access may be less disruptive to the lateral wall, but it is not automatically simpler. A narrow ridge, an unfavorable sinus contour, a septum, poor membrane mobility or inadequate primary stability can change the plan. The surgical team should explain what intraoperative finding would prompt conversion, staging or abandonment of simultaneous implant placement.
Lateral access: MSFA
The lateral window gives direct access to the sinus membrane and a space that can accept a larger volume of graft. That capability explains the greater endo-sinus bone gain reported against OD in the three-technique review. It also adds a lateral osteotomy and a broader surgical field. Implant placement may be simultaneous or staged according to residual bone, primary stability, membrane status and the planned augmentation; the evidence does not support a single timing rule.
Direct access can be an advantage when the desired augmentation is substantial, but greater bone gain is a structural endpoint rather than proof of superior patient experience. The lateral route should therefore be discussed in terms of the amount of space required, not simply as an intrinsically better or worse operation.
What “implant stability” means in these studies
ISQ is a numerical reading used to describe the mechanical stability of an implant. The 2025 three-technique review found higher ISQ for OD at placement and at abutment connection. A higher early reading may be attractive when the treatment plan depends on immediate mechanical stability, but it is not equivalent to a long-term survival endpoint. Implant survival was similarly high across the methods in the short-term randomized evidence, so the ISQ difference should be considered one factor rather than a stand-alone reason to choose OD.
Stability also has a timing dimension. A reading at placement describes initial mechanical conditions; a reading at abutment connection reflects a later stage. Neither measurement by itself answers whether the augmented bone will remain healthy for years, whether the prosthesis will require maintenance, or whether sinus-related problems will develop.

Procedure burden and recovery
The same randomized-trial review associated OD with shorter surgery, fewer reported complications, and fewer days of analgesic use than the comparator techniques. The findings describe averages or pooled comparisons in the included trials. They do not predict how long a particular case will take: sinus anatomy, membrane behavior, need for grafting, implant number, sedation and the surgeon’s experience all influence the actual course.
A smaller transcrestal access may reduce the amount of surgical exposure, but postoperative discomfort is still individual. A lateral approach adds a bony window and grafted space, while OMSFE uses osteotome instrumentation that may produce a different immediate sensation. The appropriate comparison is the full treatment plan, including whether a staged second operation would otherwise be needed.
A randomized trial first published November 28, 2023 (journal issue 2024) enrolled 20 people needing one implant with residual bone height at or below 4 mm. It compared OD with lateral-window elevation and simultaneous implant placement. The protocol measured pain, OHIP-14 quality of life, analgesic intake, symptoms during the first week, surgery duration, complications, and implant stability at baseline and six months. The reported trial design and endpoints do not provide enough results to claim that either technique was better on those outcomes. The eligibility criterion is therefore not a general indication or a universal cutoff.
Choosing a technique: a practical decision framework
A clinician normally weighs several questions together rather than selecting a method from a single millimetre value.
1. How much vertical gain is actually needed?
Start with three-dimensional imaging and the planned implant position. A modest elevation may be compatible with a transcrestal approach, whereas a larger grafted volume may favor lateral access. The reviews do not establish a universal boundary between those situations. Bone height, sinus shape, septa, membrane health, ridge width, implant dimensions and prosthetic position all matter.
2. Is simultaneous implant placement realistic?
Transcrestal OD and OMSFE are often considered when the implant can be stabilized during the same operation. Lateral augmentation can also be combined with placement in selected cases, but staging may be chosen when primary stability or membrane management is uncertain. The correct timing is a case decision, not a guaranteed property of the technique. Ask what minimum stability or membrane finding the clinician uses before proceeding with immediate placement.
3. Which outcome has priority?
- Mechanical stability at placement: OD had higher ISQ in the short-term randomized review.
- Maximum endo-sinus bone volume: MSFA produced more bone gain than OD in that review.
- Smaller access and potentially lighter recovery: The transcrestal route avoids a lateral window; OD was associated with shorter surgery and fewer analgesic days in the review.
- Evidence confidence: All of these comparisons are constrained by short follow-up, small trials, high risk of bias and low certainty.
- Ability to manage an unexpected finding: Direct lateral access may offer more working room, whereas a transcrestal plan may be sufficient when the anatomy and required elevation are favorable.
4. What does the clinician see on imaging?
Residual bone height should be treated as a description of the case, not as a treatment prescription. The 2025 crestal review reported mean study-group heights of 5.94 mm for OD and 5.00 mm for osteotome studies, while the 20-person OD-versus-lateral trial included cases at or below 4 mm. Those figures came from specific study populations and cannot be converted into a rule that one approach is safe or effective above or below a particular number.

Patients should know whether the clinician would stop, graft and return later, change the access route or place a shorter implant if the membrane cannot be elevated as intended. A clear contingency plan is especially important when the treatment depends on simultaneous placement or when the residual bone is limited. The comparative reviews do not prescribe one fallback, so it must be tailored to the anatomy and restoration.

Potential complications and how to discuss them
Sinus-floor elevation can involve membrane perforation, graft displacement, infection, postoperative discomfort, bleeding or the need to change from a planned approach. The published comparisons summarized here do not provide a reliable patient-specific rate for each event. OD’s counterclockwise pumping motion and irrigation are intended to compact bone and elevate the membrane, but they do not make perforation impossible. OMSFE and MSFA have their own technical demands, and the pooled complication percentages should not be used as a personal risk calculator.
Before surgery, ask how the clinician defines and manages a membrane perforation, what would trigger staged treatment, how sinus symptoms are handled and which signs require urgent contact. Ask whether the implant would be removed or left out if primary stability is inadequate. Also clarify who provides after-hours advice and how follow-up imaging or stability checks are scheduled.
After surgery, follow the treating team’s instructions about bleeding control, medications, hygiene and sinus precautions. Contact the team promptly for worsening pain, fever, persistent bleeding, foul drainage or breathing or sinus symptoms. Those aftercare decisions are individualized and are not replaced by a review average.

Cost and treatment burden
The comparative reviews did not report a standardized fee schedule that allows a fair cost ranking among OD, OMSFE and MSFA. The eventual charge can reflect imaging, sedation, graft material, the implant, laboratory work, whether augmentation and placement are simultaneous, and whether a second staged operation is required. A lateral window may involve a larger grafted procedure, while a transcrestal plan may still require additional treatment if primary stability or membrane conditions are inadequate. Ask for a written estimate that separates the surgical, implant, graft, restoration and possible-revision components rather than assuming that a shorter operation always costs less.
What is known about long-term outcomes?
The strongest comparative statement is limited to short-term implant survival. The six randomized trials do not establish long-term equivalence, and the reviews do not justify claiming that OD, OMSFE or MSFA is superior for every patient. Long-term outcomes can include maintenance of augmented bone, peri-implant tissue health, prosthetic complications, sinus health and implant survival beyond the follow-up used in the trials. Those questions require longer, better-controlled randomized studies.
Follow-up duration also affects how complications are counted. A study that records events only through abutment connection cannot answer the same question as one that follows patients through years of loading. The short duration and heterogeneous methods are why apparently favorable differences should be presented as signals, not guarantees.
Questions to take to an implant consultation
- What is my measured residual bone height and sinus anatomy at the planned implant site?
- How much vertical gain is required for the intended implant and restoration?
- Would you recommend a transcrestal or lateral approach, and what finding would make you change plans?
- Can the implant be placed at the same appointment, or is staged graft healing safer in my case?
- What are your definitions of membrane perforation, infection and implant instability, and how would each be managed?
- What minimum primary stability do you require before immediate placement or loading?
- What graft material, instruments and imaging will be used, and why are they appropriate for my anatomy?
- What evidence and follow-up apply to the specific technique you are proposing?
- What symptoms after surgery are expected, and which require an urgent review?
- What is included in the fee, and what additional cost would arise if treatment must be staged?
Bottom line for patients and clinicians
OD and OMSFE are transcrestal routes; MSFA uses a lateral window and graft augmentation. Short-term randomized evidence reports similarly high implant survival. OD shows signals for higher early ISQ and lower operative burden, whereas MSFA provides more endo-sinus bone gain than OD. The numbers come from small, heterogeneous, low-certainty evidence, and one review’s lower complication estimate for OD is not settled proof of superiority. Technique selection should therefore match the required augmentation, residual bone and sinus anatomy, desired timing, cost and recovery priorities, and the clinician’s ability to manage the membrane and obtain stable implant support.

Is osseodensification better than an osteotome sinus lift?
The 2025 crestal meta-analysis reported similar bone gain—3.37 mm with osseodensification and 3.18 mm with osteotome elevation—while a separate randomized-trial review found higher ISQ and less operative burden with osseodensification. The studies were heterogeneous and low-certainty, so they do not establish a universal winner.

Does a lateral-window sinus lift create more bone?
In the 2025 review comparing all three approaches, lateral-window augmentation produced greater endo-sinus bone gain than osseodensification. That radiographic advantage did not produce a demonstrated short-term implant-survival advantage, and it does not by itself determine which route is appropriate for an individual.
Can the implant be placed during the same surgery?
OD and osteotome procedures may permit simultaneous placement when primary stability and membrane conditions are favorable. Lateral-window augmentation can also be simultaneous in selected cases or staged after healing. Residual bone, sinus anatomy and intraoperative findings determine the timing.
What does the 2.78% complication figure for osseodensification mean?
It is a pooled estimate from a 2025 review of 13 studies and 519 sites, compared with 14.32% for osteotome studies. Different definitions, operators, case selection and follow-up can change these percentages, so they are not an individual’s predicted risk.
Is 4 mm of residual bone the cutoff for choosing one technique?
No. A 20-participant randomized trial enrolled sites at or below 4 mm, but that was an eligibility criterion for that study, not a general treatment rule. The reviews do not validate one residual-height cutoff for every patient or operator.
Which technique is likely to hurt less?
The three-technique review associated osseodensification with shorter surgery, fewer reported complications and fewer analgesic days. Those are pooled study findings, not a guarantee. The actual experience depends on anatomy, grafting, number of implants, sedation and the clinician’s technique.
Are these methods proven equivalent over the long term?
No. The randomized evidence reports comparably high short-term implant survival, but the trials were short, often at high risk of bias and graded low-certainty. Longer, better-controlled studies are needed to compare durable bone, sinus health, prosthetic maintenance and implant survival.
How should I compare treatment costs?
The reviews did not provide standardized fees. Request an itemized estimate covering imaging, sedation, grafting, implant placement, restoration and possible staged surgery; a smaller access route does not guarantee a lower total cost.
The Bottom Line
Bottom line: Osseodensification and osteotome elevation approach the sinus through the crest; lateral-window augmentation approaches it from the side and generally creates more endo-sinus bone. Short-term survival is similarly high, with signals for higher early stability and lighter operative burden with OD, but low-certainty evidence and variable anatomy prevent a universal recommendation.
