Orthokeratology (Ortho-K) uses specially fitted rigid lenses worn during sleep to temporarily reshape the cornea, so some people can see during the day without lenses. Conventional contact lenses correct vision while they are on your eyes. Ortho-K may suit someone who values daytime freedom from lenses and can keep up with nightly care and follow-up; daytime contacts may suit someone who prefers not to sleep in lenses. An eye-care professional must assess which, if either, is appropriate for your eyes.

How Ortho-K differs from daytime contact lenses

What to compare Orthokeratology Conventional contact lenses
When you wear them Usually overnight; you remove them in the morning, following the prescribed schedule. Often during waking hours. The schedule depends on the prescribed lens.
How they correct vision Specially designed rigid gas-permeable lenses temporarily change corneal curvature to reduce myopia while the lenses are out. Correct vision while worn; the effect does not ordinarily continue once removed.
Daytime without a lens Possible for some wearers; the effect and how long it lasts vary. Not the usual result when lenses are removed.
Ongoing routine Overnight wear on a clinician-prescribed schedule, careful lens handling and cleaning, and follow-up. Wear, cleaning, disinfection, and replacement depend on lens type and practitioner instructions.
Sleeping in lenses Overnight wear is part of the treatment and carries infection risk. Do not sleep in daily-wear lenses. Follow the specific lens’s approved schedule.

The U.S. Food and Drug Administration describes Ortho-K as a fitting procedure that uses specially designed rigid gas-permeable lenses to change corneal curvature and temporarily improve focusing. FDA: Types of Contact Lenses. Neither approach permanently corrects myopia: Ortho-K’s effect requires continued wear on the prescribed schedule, and vision may fluctuate if that schedule is not followed.

What the nightly Ortho-K routine involves

Ortho-K is not simply a regular contact lens worn at bedtime. It requires a professional fitting, instruction in insertion and removal, and care that follows the lens and care-product directions. FDA guidance says to follow the prescribing professional’s instructions and the labeling for lens-care products, including the case. FDA: What To Know If Your Child Wants Contact Lenses.

In the United States, FDA says eye-care professionals must be trained and certified before fitting overnight Ortho-K lenses in practice. This is U.S.-specific regulatory context, not a statement about requirements in other countries. FDA guidance.

Why overnight wear changes the safety discussion

Sleeping in daily-wear contact lenses is not safe; follow the lens’s approved wear schedule and your practitioner’s directions. Overnight contact-lens wear is associated with greater risk of microbial keratitis than daily wear. The available guidance does not establish an individual probability or determine which option is safer for a particular person. American Academy of Ophthalmology, Refractive Errors Preferred Practice Pattern.

Ortho-K’s overnight wear is part of the treatment, so ask the fitter to explain the risks, hygiene steps, follow-up plan, and what to do if you develop redness, pain, irritation, or fluctuating vision. For either modality, do not improvise a wear schedule or care routine.

Daytime contacts may appeal if

  • You prefer not to sleep in lenses.
  • You want correction only while wearing lenses during waking hours.
  • You prefer the wear and replacement routine of a particular prescribed lens type.

These are practical considerations, not a way to determine medical eligibility. Both options require an appropriate prescription, fit, and care instructions. The National Eye Institute recommends consulting an eye doctor about Ortho-K. NEI: Other Types of Contact Lenses.

What prescription limits and approval mean

The American Academy of Ophthalmology’s 2022 Preferred Practice Pattern describes FDA approval for temporary Ortho-K reduction of myopia up to 6.00 diopters in eyes with astigmatism up to 1.75 diopters. Those figures describe stated device or indication limits, not a promise of results or universal eligibility. The applicable limits depend on the specific device labeling; an eye-care professional can assess the current labeling and your prescription. AAO, 2022.
