How to interpret “less sexual content” accurately
The ESRB is the most consistent public screening tool for this comparison, but its descriptors are not a scene-by-scene inventory. The board defines Sexual Content as non-explicit sexual behavior. Strong Sexual Content means explicit or frequent sexual behavior and may include nudity. A title without either label is a better screening signal than a marketing phrase such as “family-friendly,” yet it does not promise that every joke, relationship reference or optional online interaction will suit every household.

Check the exact product listing and the current ESRB entry before buying, especially when a game has multiple “Definitive,” “Complete” or bundled editions.

LEGO City: Undercover — the clearest low-sexual-content match
What you do
You play an undercover police officer in an open LEGO city. The structure combines crime-scene investigation, driving, collecting, disguises, platforming and mission-based arrests. That gives it the broad loop many people seek in a GTA-like game—moving freely through a city, taking vehicles and responding to crimes—without realistic firearms, adult clubs or graphic depictions.
What the rating tells you
The ESRB rates the game E10+ for Everyone 10+ with Cartoon Violence and Crude Humor. Its official summary specifically describes an open-world LEGO City in which players investigate crime scenes, drive, complete missions and apprehend criminals. No Sexual Content, Nudity or Sexual Themes descriptor appears in that rating summary.
Important limits
“Low sexual content” does not mean “no potentially awkward humor.” The game uses slapstick, bodily-function jokes and bathroom humor. Violence is deliberately comic rather than realistic, but younger players may still need guidance about the age rating and the game’s jokes. If your requirement is a realistic crime sandbox with adult tone removed, this is the trade-off: it is substantially less mature in every direction, including its humor and storytelling.

Just Cause 3 — a mature sandbox without a sexual descriptor
Why it feels like a GTA alternative
Just Cause 3 offers a large open world built around traversal and destruction. Vehicles, weapons, parachute-and-wingsuit movement and explosive objectives create the unscripted freedom that attracts GTA players. The campaign follows an effort to overthrow a dictator, so the setting is political action rather than urban organized crime.
Content profile
The ESRB rates it M for Mature 17+ with Blood, Intense Violence and Strong Language. The summary does not list Sexual Content, Nudity or Sexual Themes. That omission makes it the strongest choice for an adult who specifically wants to avoid sexual material but is comfortable with harsh action.
Why it is not a gentle option
The violence is the defining feature, not a minor side effect. Explosions, shootings and physical attacks are frequent, and one sequence allows civilians or captives to be killed. Anyone replacing GTA because they want a generally calmer or more age-appropriate game should choose LEGO City instead; Just Cause 3 solves the sexual-content concern by leaning harder into destructive combat.

Games that resemble GTA but fail a strict sexual-content filter
Sleeping Dogs: Definitive Edition
This Hong Kong crime thriller combines driving, melee combat, investigations and a tightly directed story. Its ESRB summary lists Sexual Content alongside Blood and Gore, Intense Violence, Strong Language and Use of Drugs. The cited examples include purchases at massage parlors and sexual dialogue. It can fit a player seeking a serious crime narrative, but it should not be presented as sexual-content-free.

Watch Dogs 2
Watch Dogs 2 shifts the formula toward hacking, surveillance systems, modern-city exploration and a satirical activist crew. The ESRB lists Nudity and Sexual Themes, plus Blood, Intense Violence, Strong Language and Use of Drugs. The rating summary specifically mentions a strip club and brief full-frontal nudity. Those details make it unsuitable for a strict no-nudity rule even though much of the main gameplay is technological rather than sexual.
Red Dead Redemption 2
Red Dead Redemption 2 provides a vast western world, detailed exploration, missions and spontaneous encounters. PlayStation describes it as “a sprawling tale of life in America at the dawn of the modern age.” Its ESRB information, however, includes Nudity and Sexual Content, with examples involving a brothel and genital nudity. Blood and Gore, Intense Violence, Strong Language, drugs and alcohol are also listed. Choose it for world-building and western atmosphere, not as a low-sexual-content substitute.

Mafia: Definitive Edition
This remake follows organized crime during Prohibition, with a structured campaign, driving and open-world activities. Its ESRB descriptors include Sexual Themes, and the summary references a brothel mission, suggestive dialogue and sexual moaning. Blood, Drug Reference, Intense Violence and Strong Language round out the warning. The sexual material is more limited than in some mature open worlds, but it is present and identifiable.
Mafia III: Definitive Edition
Mafia III uses an open-world New Orleans setting and a territory-by-territory crime takeover structure. It carries the strongest warning in this group: Strong Sexual Content and Nudity, in addition to Blood and Gore, Intense Violence, Strong Language, drugs and alcohol. It is therefore a poor recommendation for anyone searching specifically for “GTA without sex,” regardless of how appealing its crime-management systems may be.

If sexual content is the hard limit
Pick LEGO City: Undercover first. Its E10+ descriptor set is the clearest evidence of a low-sexual-content design, and its open city, vehicles and police missions preserve the sandbox appeal in a playful form.

If you are an adult avoiding sex but not violence
Pick Just Cause 3. Confirm that the person playing is comfortable with blood, intense violence, strong language and the possibility of civilian or captive deaths. It is not a family recommendation.
If you mainly want a crime story
Sleeping Dogs or Mafia: Definitive Edition offer more grounded crime narratives, but both require accepting the specific sexual scenes or themes listed by the ESRB. Mafia III should be excluded when strong sexual content or nudity is unacceptable.
If you want a modern city and systems-driven play
Watch Dogs 2 is the systems-focused choice, with hacking and urban experimentation. The strip-club setting and brief full-frontal nudity mean it does not pass a strict sexual-content screen.
If you want exploration above all else
Red Dead Redemption 2 has the broadest western exploration and emergent-world emphasis here, but its documented nudity and sexual content place it outside a low-sexual-content list.
Buying and screening checklist
- Identify the exact edition. Match the store listing to the edition named in the rating entry, including “Definitive” or remastered labels.
- Check the platform and region. ESRB ratings cover North American releases; other ratings boards and regional edits may use different labels or classifications.
- Read every descriptor, not just the age symbol. An M or E10+ badge alone does not explain whether the concern is sex, violence, language, drugs or humor.
- Look for concrete examples. Strip clubs, brothel missions, massage-parlor purchases, nudity and sexual dialogue are materially different from a vague “adult themes” tag.
- Separate sexual-content limits from violence limits. Just Cause 3 demonstrates why a game can omit sexual descriptors while still being unsuitable for children or players avoiding graphic action.
- Use parental controls where available, but do not assume they remove scenes. Ratings describe the shipped content; they are not a guarantee that an optional toggle exists.
Bottom line
For the closest GTA-style experience with minimal sexual-content concern, LEGO City: Undercover is the defensible first choice: open-city activities and crime missions under an E10+ rating limited to Cartoon Violence and Crude Humor. Just Cause 3 is the adult alternative when explosive freedom matters more than a low-violence rating. Sleeping Dogs, Watch Dogs 2, Red Dead Redemption 2 and the Mafia games all have documented sexual content, nudity or sexual themes, so they belong on a “check the warnings first” list rather than a clean-content recommendation.
