If the memory write fails, the application still records the resolution as successful but warns the operator that it was not retained. The design is a deliberate boundary around what survives a restart—not a claim that the agent never forgets.
How the incident loop uses memory
Omer describes a three-stage workflow: recall similar past incidents from the current symptoms, reflect on possible causes and fixes using the Hindsight bank, and retain a postmortem after a human marks an incident resolved. In his account, Hindsight does the reasoning over stored memory; a separate LLM call reshapes the result into sections for the interface. Omer summarizes that division as: “The LLM does not diagnose.”

Calls to Hindsight are isolated in a memory.py module rather than spread across the application. That gives the implementation one place for logging, timing, error handling, and adapting to SDK changes. The author also runs recall and reflect concurrently with asyncio.gather(), since those operations are independent in the described flow.

Unresolved incidents live in a process-local list, so a process restart discards them. When a person resolves an incident, the application records its root cause, resolution, and resolved status, then attempts to retain it in Hindsight. From the application’s perspective there is no delete endpoint, and the bank is treated as append-only.

That boundary reflects Omer’s judgment about what is valuable to preserve: symptoms can be submitted again, while rebuilding a human-verified cause and successful fix may be difficult. It is a choice for this implementation, not a universal rule for every incident archive; teams may have different retention, audit, or privacy requirements.
How a failed memory write is handled
A Hindsight error does not turn a successful incident resolution into a failed resolution. The application returns HTTP 200, keeps the incident marked resolved in its own response, and includes a warning with retained: false. That distinction tells the operator the remediation succeeded but the historical record did not reach the memory bank, leaving follow-up to a person.

This is an important reliability detail: the application does not report an unsuccessful retain as if it had succeeded. A consumer of the response must pay attention to the warning and retention flag rather than interpreting the HTTP status alone as proof that memory was updated.

What goes into Hindsight
The retained postmortem uses a labelled text format containing the incident ID and title, severity, affected system, symptoms, root cause, and resolution. The retention call also supplies a document ID, timestamp, and severity/system metadata. Omer says the consistent layout helps Hindsight chunk and embed postmortems consistently; the document ID supports traceability, while the timestamp makes time-oriented questions possible.

The bank is assigned a mission to act as an on-call incident analyst, emphasizing system causes rather than blame. Its configuration includes skepticism 4, literalism 3, and empathy 2. These are Omer’s settings for this project, not established defaults or generally optimal values.
Implementation issues Omer reports

loop.run_until_complete()and causedRuntimeError: event loop is already runningfrom an asynchronous route. He switched to asynchronous variants. - Creating the bank at startup: He reports receiving HTTP 409 when calling
create_bank()a second time. His startup approach therefore became ensuring the bank exists instead of recreating it at each process start.
These are observations about the SDK and implementation state Omer used. They should not be assumed to describe every current release.

Why recall results need regrouping
Omer reports that arecall() on a bank containing 20 incidents returned roughly 45 results. In his example, one incident could contribute multiple facts or fragments—for symptoms, cause, and resolution—so the application groups results back into incident cards using document IDs and sorts the grouped cards by score. The figure is an approximate, author-reported example, not a benchmark or a general measure of recall quality.
The article describes a demo that can compare behavior with and without memory, but it does not provide a controlled measurement of diagnosis accuracy, resolution time, or safety. The implementation illustrates how historical context may be organized and presented; it does not establish that memory improves operational outcomes.

What this persistence design establishes
- Active incidents are ephemeral, while a human-validated resolution is the trigger for persistence.
- The stored record captures the human-entered cause and fix alongside identifiers and time-related metadata.
- A failed retain is exposed to the operator rather than represented as a successful memory write.
- Recall fragments are regrouped so the interface can present incident-level cards instead of disconnected facts.
These points describe the architecture Mohammed Omer reported on DEV Community on September 29, 2026. They are implementation testimony, not an independent evaluation of agent accuracy or a current reference for Hindsight SDK behavior.
