No Single Healthcare Ransomware Tactic Is Proven to Cut Risk by 70%

No available source establishes that one tactic reduces healthcare ransomware risk by 70%. The number is easy to misread: one report uses 70% for ransomware’s share of successful healthcare cyberattacks in two years, while another reports a 70.0% overall likelihood of paying a ransom after an incident. Neither measures a tactic’s reduction in attack risk. Healthcare organizations should instead use layered risk management and verify that they can restore systems and sustain care when an attack occurs.

What the 70% figure does—and does not—mean

“Risk” could mean the chance of an attack, successful encryption, ransom payment, downtime, or patient harm. A claim that one tactic cuts risk by 70% needs to specify which outcome was measured, among which organizations, over what period, and against what comparison group. The sources available here do not provide that evidence.

  • The American Hospital Association says ransomware accounted for more than 70% of successful cyberattacks on healthcare organizations in each of two years. That is a share of attacks, not a 70% reduction caused by a control. AHA: Ransomware Attacks on Hospitals Have Changed.
  • An Arete and Cyentia Institute report gives 70.0% as the overall likelihood of ransom payment in its dataset. Its control-specific figures concern payment outcomes among affected organizations, not the chance of being attacked across the healthcare sector. Healthcare Sector Ransomware Spotlight.

For example, the report’s MFA row lists MFA in 19.3% of its healthcare dataset, a 34.4% share of demanded ransom paid, and a 52.0% payment likelihood. These are report-specific payment figures, not evidence that MFA or another measure prevents a given percentage of attacks. Because the analysis concerns organizations that experienced ransomware, it cannot establish how a control changes the likelihood of an attack among all healthcare organizations.

Why ransomware preparedness is a patient-care issue

Ransomware can disable systems needed to deliver care, not merely expose information. A 2022 cohort study examined 374 reported attacks on U.S. healthcare delivery organizations from 2016 through 2021, affecting nearly 42 million individuals. It found that 166 attacks (44.4%) disrupted care. Documented effects included electronic-system downtime, canceled scheduled care, and ambulance diversion. Annual reported attack counts rose from 43 in 2016 to 91 in 2021. JAMA Health Forum study.

The study describes reported incidents, not every attempted or unsuccessful attack, and it does not test the effectiveness of a particular preventive tactic. Its findings nevertheless show why resilience planning must account for clinical operations as well as data confidentiality.

What healthcare organizations should put in place

HHS guidance does not prescribe a single universal fix or attach a 70% effect size to one. It points covered entities and business associates toward an organization-wide process for identifying and managing risks to electronic protected health information (ePHI), alongside safeguards, training, recovery planning, and response procedures. Assess and manage risk across the organization

Identify risks to ePHI and implement measures sufficient to reduce them to a reasonable and appropriate level. HHS also identifies procedures to guard against and detect malicious software, and user training. The practical implication is to evaluate how controls apply to the organization’s systems, accounts, clinical workflows, and dependencies rather than assume a single measure covers every path to disruption.

Back up data—and test restoration

HHS says frequent backups and the ability to recover data from them are crucial. It recommends periodic restoration tests and advises considering offline backups because some ransomware variants have disrupted online backups. A backup is useful for recovery only if the organization can restore it when needed.

Plan for clinical operations and incident response

Contingency planning should cover disaster recovery, emergency operations, criticality analysis, and periodic testing. HHS says incident-response procedures should address detection, containment, eradication, recovery, and post-incident review. Exercises can reveal whether staff can keep essential work moving while systems are unavailable and whether response roles and recovery steps are workable.

Include business associates in the risk picture

Healthcare organizations also rely on outside parties that may handle information or support operations. A 2025 study of provider-reported U.S. incidents from 2016 through 2024 identified 831 ransomware incidents; 281 (33.81%) explicitly involved a HIPAA business associate. The analysis found that business-associate-involved breaches were smaller on average across the distribution but larger among very large incidents affecting 100,000 or more individuals. These figures reflect the study’s classification of provider filings, not a complete count of all events. Health and Technology: Third-party risk in U.S. health care ransomware incidents.

The study’s practice implications include tiering vendors by operational criticality, seeking proportionate assurances, coordinating incident response, and exercising with key vendors. These are risk-management recommendations, not experimentally proven ways to achieve a particular reduction in attack risk.

How to judge a ransomware-risk claim

  • Define the outcome: Ask whether the percentage refers to attack likelihood, successful compromise, payment, downtime, or losses.
  • Check the population and comparison: Determine who was studied, what the control group was, and whether the analysis measured outcomes before or after an incident.
  • Separate association from cause: A control reported by organizations with different payment outcomes is not, by itself, proof that the control prevented attacks.
  • Look for operational proof: For recovery and response plans, look for restoration tests and exercises that demonstrate whether the organization can act under disruption.

Until a claim specifies these details and is supported by suitable evidence, treat “cuts ransomware risk by 70%” as unverified rather than as a planning benchmark.
